Summary: The gold standard method for diagnosing and staging chronic liver diseases is liver biopsy, although breath analysis represents an attractive non-invasive alternative. Studies comparing breath samples of patients with chronic liver diseases to control subjects have identified candidate biomarkers, with the most effective compounds being of exogenous origin. The use of an exogenous volatile organic compounds (EVOCs) probe approach may help overcome the limitations of random dietary exposure and has shown potential in healthy subjects.

Summary: The liver plays a crucial role in systemic metabolic processes, with liver mitochondria being key in degrading pathways and providing energy for cells. Nonalcoholic fatty liver disease (NAFLD) is a common chronic liver disorder associated with obesity and metabolic disorders, and its progression can lead to cirrhosis and hepatocellular carcinoma. Evaluation of liver function in NAFLD patients using C-13 breath test (BT) can assess mitochondrial function and overall liver health, providing a cost-effective and indirect method for dynamic liver function testing. Further applications of C-13-BT are anticipated in clinical medicine.

Summary: This study established noninvasive models for assessing liver reserve function (LRF) based on liver stiffness measurement (LSM) and evaluated their clinical performance. The new models showed good predictive performance for LRF and could replace the indocyanine green (ICG) clearance test, especially in patients who are unable to undergo ICG testing.

Summary: Impaired phenylalanine metabolism and its relationship with gut microbiota in alcohol-related liver disease (ALD) were investigated. Elevated levels of phenylalanine and its metabolites were found in the intestines of alcohol-fed mice, negatively correlated with the abundance of Lactobacillus. Phenylalanine levels were correlated with colon inflammation and intestinal barrier dysfunction, while oral supplementation of L. acidophilus improved phenylalanine metabolism and protected mice from ALD.

Summary: The urea breath test is a non-invasive method for diagnosing Helicobacter pylori infections. Raman spectroscopy shows potential for more accurate measurements than commonly used infrared sensors. Measurement errors, including equipment error and uncertainty, can affect the accuracy of the test. A Raman scattering-based gas analyzer capable of measuring d(13)C in exhaled air was developed. The total error measured was 6%, below the analytically calculated limit of 10%.

Summary: Chronic liver diseases lack effective pharmacological therapies and fibrosis, a major precursor of liver cirrhosis, is the focus of our research group. Fibrosis is a progressive disturbance of liver sinusoidal architecture caused by connective tissue deposition. Understanding the multifactorial events and various cell types involved in fibrosis initiation and progression is crucial. Developing experimental models of liver fibrosis and identifying critical factors that lead to cirrhosis will aid in the development of more effective therapies. This review provides an overview of hepatic fibrosis processes and therapeutic approaches based on a deep understanding of molecular regulation of fibrogenesis in the liver.

Summary: This study found that 29 volatile organic compounds (VOCs) differed significantly between cirrhosis patients and controls in breath samples. The seven best performing VOCs were sufficient for classification. Furthermore, 11 VOCs were correlated with liver function metrics and could separate patients by cirrhosis severity using principal component analysis. These results suggest that these VOCs have promise as a panel for liver disease detection and monitoring.
