Summary: This study aimed to describe the radiological features of chronic thromboembolic pulmonary disease (CTEPD) and compared them with chronic thromboembolic pulmonary hypertension (CTEPH) patients. The results showed significant differences in vascular obstruction, hypoperfusion, mosaic attenuation, and signs of pulmonary hypertension between the two groups, suggesting that the absence of peripheral microvascular disease may be the reason for the absence of pulmonary hypertension in CTEPD patients.

Summary: This study found that 15% of patients with acute pulmonary embolism (PE) showed signs of chronic PE or chronic thromboembolic pulmonary hypertension (CTEPH) on CT pulmonary angiography (CTPA). Specific radiologic parameters, such as intravascular webs, arterial narrowing or retraction, dilated bronchial arteries, and right ventricular hypertrophy, could help identify preexisting CTEPH. Early diagnosis of CTEPH can be facilitated by targeted follow-up strategies and risk-adapted CTEPH screening based on these parameters.

Summary: This study investigated the association between hepatorenal dysfunction and right heart function as well as survival in patients with chronic thromboembolic pulmonary hypertension (CTEPH). The results showed that hepatorenal dysfunction mainly depends on right atrial strain and tricuspid regurgitation, rather than right ventricular systolic dysfunction. Additionally, hepatorenal dysfunction can predict mortality at baseline and follow-up.

Summary: Acquired thrombophilia, specifically antiphospholipid antibodies (aPL), may contribute to the development of chronic thromboembolic pulmonary hypertension (CTEPH). Young patients who experience an unprovoked pulmonary embolism (PE) or PE triggered by mild risk factors should be tested for aPL. If positive, close monitoring and lifelong anticoagulant therapy should be considered because aPL-induced thrombophilia increases the risk of recurrent PE leading to CTEPH. The presence of lupus anticoagulant (LAC), anti-cardiolipin (aCL), and anti-& beta;2-glycoprotein I (a & beta;2GPI) antibodies indicates the highest risk of recurrence and necessitates treatment with warfarin for maximum protection.

Summary: This study systematically assessed the late outcomes of acute pulmonary embolism (PE) and investigated the clinical implications of post-PE impairment (PPEI). The results showed that the cumulative incidence of chronic thromboembolic pulmonary hypertension (CTEPH) was 2.3%, while the incidence of PPEI diagnosed by standardized criteria was 16.0%. The presence of PPEI increased the risk of re-hospitalization and death, as well as worsened quality of life.

Summary: This systematic review focuses on the pre-operative evaluation, intra-operative procedure, and post-operative results of pulmonary endarterectomy (PEA) in patients with chronic thromboembolic pulmonary hypertension (CTEPH). The study found that PEA is an acceptable treatment option with satisfactory short- and long-term outcomes in highly experienced CTEPH centers. The decision to perform PEA should be made by a multidisciplinary team consisting of surgeons, pulmonologists, and radiologists.

Summary: Patients with chronic thromboembolic pulmonary hypertension (CTEPH) exhibit significant impairment in all steps of the O-2 transport cascade during exercise, resulting in markedly impaired exercise capacity. Pulmonary vascular intervention increases peak Vo(2) by partly correcting O-2 delivery but has no effect on abnormalities in peripheral O-2 extraction. This suggests that current interventions only partially address patients' limitations and that additional therapies may improve functional capacity.
