Summary: This study analyzed the cost-effectiveness of screening and surveillance strategies for gastric cancer in Brazil, France, Japan, Nigeria, and the United States. The most cost-effective strategies were identified as screening every 10 years, surveillance every 5 and 10 years for high- and low-risk patients, and screening every 5 years. Starting age of screening, cost of endoscopy, and baseline probability of local gastric cancer were found to be important variables affecting the overall model.

Summary: While clinical practice guidelines exist for hereditary diffuse gastric cancer, there is no consensus on how to approach familial intestinal gastric cancer. A multicenter observational study was conducted in nine Spanish hospitals to investigate the yield of gastric cancer surveillance, as well as identify risk factors for development of gastric cancer and precursor lesions in these families. The study found that Helicobacter pylori infection was independently associated with an increased risk of developing precursor lesions or gastric cancer.

Summary: Studies revealed that guideline-specified endoscopic surveillance for patients with non-dysplastic Barrett's esophagus (NDBE) was not cost-effective. Instead, a strategy of deselecting low-risk NDBE patients from surveillance may be a cost-effective approach.

Summary: The optimal strategy for screening esophageal adenocarcinoma varies by race and sex. White men with GERD symptoms can potentially be screened more intensely than is currently recommended, while screening women may not be cost-effective and could even cause net harm for black women.

Summary: This study aimed to elucidate the endoscopic causes and characteristics of missed gastric cancers (MGCs) after endoscopic submucosal dissection (ESD) for early gastric cancer (EGC). The results showed that the endoscopic causes of MGCs included perceptual errors, exposure errors, sampling errors, and inadequate preparation. Logistic regression analysis revealed the risk factors for perceptual errors and the sites of exposure errors.

Summary: The study found that extending screening mammography to age 80 may have benefits for some women, but the absolute reduction in death rates is small for women with comorbidities. Therefore, women over 75 should weigh the risks and benefits when considering screening mammography.

Summary: Extending mammography screening to age 80 appears to be cost-effective in certain scenarios, but the absolute number of deaths averted is small, especially for women with comorbidities. Women considering screening beyond age 75 should carefully weigh the potential harms of overdiagnosis against the potential benefits of averting breast cancer death.

Summary: This study compared the costs and cost-effectiveness of PTED and open microdiscectomy for patients with sciatica. The results showed that PTED was more effective and less costly than open microdiscectomy in terms of leg pain and quality-adjusted life years. From a societal perspective, PTED was found to be more cost-effective.

Summary: True penicillin allergy is rare and commonly incorrectly reported. Testing shows that less than 5% of patients who report a penicillin allergy will have an active hypersensitivity when appropriately tested. Penicillin allergy testing is safe and cost-effective, and inaccurate penicillin allergy status may lead to inappropriate antibiotic use.

Summary: This study evaluated the cost-effectiveness and utility of posttransplant hepatocellular carcinoma (HCC) surveillance compared to standard follow-up. Although surveillance slightly improves survival and quality of life, it also increases costs and does not appear to be cost-effective in this setting.

Summary: The study analyzed the cost-effectiveness of FDA-approved drugs for CIC from the perspectives of insurers and patients. Prescription drugs increased insurer costs but decreased patient costs, with drug tolerability and impact on quality of life being key factors in treatment choice from the patient perspective. Addressing costs at a policy level may help optimize treatment choice for CIC patients.

Summary: The study identified SOF/LDV as the most cost-effective treatment for HCV genotype 1, producing more QALYs at a lower cost compared to SOF/VEL and the traditional pegIFN + RBV regimen. SOF/VEL was also economically dominant to pegIFN + RBV, suggesting DAA regimens could be a promising strategy for treating HCV.

Summary: The study evaluated the cost-effectiveness of glecaprevir/pibrentasvir (GLE/PIB) for chronic HCV genotype 1 and genotype 2 infection in China, finding that GLE/PIB was a cost-saving regimen with increased quality-adjusted life years compared to the pegylated interferon (pegIFN) plus ribavirin (RBV). The probability of GLE/PIB being cost-effective was high, especially for HCV genotype 1 infection. Influential factors included age of starting DAA treatment, price of pegIFN + RBV, cost of cirrhosis treatment, and duration of the GLE/PIB regimen.
