Long-term follow-up of ulcerative colitis in Taiwan

Background: The incidence of ulcerative colitis (UC) has been increasing in Asia recently, but little long-term follow-up data is available. We aimed to understand the clinical characteristics of UC patients in the National Taiwan University Hospital (NTUH), a tertiary referral center in Taiwan. Methods: A retrospective study was conducted to review data from January 1, 1988 through December 31, 2008 compiled at NTUH. Patients' clinical information, demographic data, endoscopic pictures, treatment regimens, pathologic, and outcome details were reviewed, recorded, and analyzed. Results: A total of 406 patients were included (233 males and 173 females; median age at diagnosis was 36 years). The follow-up period ranged from 0.25 to 40.8 (mean, 7.3) years. The prevalence of UC in Taiwan was at least 7.4/100,000 in 2008. Bloody stool was the most common presentation (77.3%). Total colon was the most common (41.0%) disease involvement and proctitis the least common (21.1%). Six patients (1.5%) died during the follow up. Most of the UC patients (72.4%) could be controlled with 5-aminosalicylic acid alone, but about one third (30.9%) were admitted for treating the UC or UC-related complications. Twenty-three patients (5.5%) were treated surgically. Extra-gastrointestinal tract manifestations were noted in 4.5% of the UC patients, with primary sclerosing cholangitis (6 in 406, 1.5%) the most common. Colon cancer/severe dysplasia occurred in six (1.5%) of the patients. Conclusion: The incidence of UC has increased in Taiwan. Interestingly, CRC/dysplasia and PSC occur more frequently here than in other Asian nations. Copyright (C) 2012 Elsevier Taiwan LLC and the Chinese Medical Association. All rights reserved.

Summary: This study demonstrated the upregulation of TL1A, IFN gamma, and two MLCK variants in human IBD biopsy specimens. TL1A was found to induce MLCK-dependent changes in gut barrier function and bacterial internalization. Different MLCK isoforms were shown to orchestrate divergent pathways of bacterial infiltration in IBD, suggesting a potential mechanism for symptom flares in the disease.

Summary: This study investigated the clinical course and liver dysfunction risks in HBV-infected IBD patients undergoing anti-TNF-alpha therapy. The results showed that liver dysfunction due to HBV reactivation was more common in patients with chronic HBV infection. Antiviral prophylaxis significantly reduced the risk of liver dysfunction in patients with chronic HBV infection.

Association of young age and male sex with primary sclerosing cholangitis in Taiwanese patients with inflammatory bowel disease

Summary: This study aimed to evaluate the prevalence, clinical manifestation, and outcomes of primary sclerosing cholangitis (PSC) in Taiwanese patients with inflammatory bowel disease (IBD). The results showed a prevalence rate of 1.57% for PSC in Taiwanese IBD patients. The majority of patients with IBD and PSC were men and were diagnosed at a younger age. Routine evaluation of biliary enzymes and liver imaging is recommended in young male patients with IBD.

Impact of the COVID-19 pandemic on inflammatory bowel disease care in Taiwan: A multicenter study

Summary: This study evaluated the impact of the COVID-19 pandemic on the perceptions of patients with inflammatory bowel disease (IBD). The results showed that patients were concerned about adverse events from vaccination, higher risks of developing severe COVID-19, and COVID-19 infection compared to the general population. Some patients self discontinued IBD medication during the pandemic, leading to worse symptoms. However, after receiving education, the anxiety levels of the patients were mitigated.

Pancreatic cancer-derived small extracellular vesical ezrin activates fibroblasts to exacerbate cancer metastasis through STAT3 and YAP-1 signaling pathways

Summary: Cancer-associated fibroblasts (CAFs), a major component of the tumor microenvironment (TME) in pancreatic ductal adenocarcinoma (PDAC), play an important role in tumorigenesis, metastasis, and chemoresistance. Tumor-derived small extracellular vesicles (sEVs), which mediate cell-to-cell communication between cancer cells and fibroblasts, are also critical for cancer progression and metastasis. In this study, we found that ezrin (EZR) in PDAC cell-derived sEVs can activate fibroblasts and promote PDAC cell proliferation, invasion, and liver metastasis through the STAT3 and YAP-1 signaling pathways.

Vaccination in patients with inflammatory bowel disease-Asian perspectives: results of a multinational web-based survey in the 8th Asian Organization for Crohn's and Colitis meeting

Summary: This study investigated the current approaches and practices of physicians regarding vaccinations for inflammatory bowel disease (IBD) patients in different Asian countries/regions. The results showed similarities among countries/regions in terms of recognizing the importance of vaccination, but there were differences in the selection and recommendation of certain vaccines, reflecting each country's vaccination guidelines and health insurance. Therefore, more communication among doctors and a consensus regarding IBD vaccination differences in the Asian region are needed.

Comparison of the magnetic resonance scoring systems for Crohn's disease activity: MaRIA, simplified MaRIA, and Nancy scores
