Antibodies to Thyroid Peroxidase Arise Spontaneously with Age in NOD•H-2h4 Mice and Appear after Thyroglobulin Antibodies

Hashimoto's thyroiditis, a common autoimmune disease, is associated with autoantibodies to thyroglobulin (Tg) and thyroid peroxidase (TPO). TPO, unlike abundant and easily purified Tg, is rarely investigated as an autoantigen in animals. We asked whether antibodies (Abs) develop to both TPO and Tg in thyroiditis that is induced (C57BL/6 and DBA/1 mice) or arises spontaneously (NOD center dot H-2h4 mice). Screening for TPOAbs was performed by flow cytometry using mouse TPO-expressing eukaryotic cells. Sera were also tested for binding to purified mouse Tg and human TPO. The antibody data were compared with the extent of thyroiditis. Immunization with mouse TPO adenovirus broke self-tolerance to this protein in C57BL/6 mice, but thyroiditis was minimal and TgAbs were absent. In DBA/1 mice with extensive granulomatous thyroiditis induced by Tg immunization, TPOAbs were virtually absent despite high levels of TgAbs. In contrast, antibodies to mouse TPO, with minimal cross-reactivity with human TPO, arose spontaneously in older (7-12 months) NOD center dot H-2h4 mice. Unexpectedly, TgAbs preceded TPOAbs, a time course paralleled in relatives of probands with juvenile Hashimoto's thyroiditis. These findings demonstrate a novel aspect of murine and human thyroid autoimmunity, namely breaking B cell self-tolerance occurs first for Tg and subsequently for TPO. Summary: Specialists are skilled in managing urinary incontinence, but primary care clinicians play a crucial role in early diagnosis and initiation of management. This study found low rates of adherence to quality indicators for women with urinary incontinence, with female clinicians performing significantly better than male clinicians. Increased years of experience was associated with worse overall care.

Health Confidence Is Associated With Disease Outcomes and Health Care Utilization in Inflammatory Bowel Disease: A Nationwide Cross-sectional Study

Summary: This study aimed to examine the associations between health confidence, inflammatory bowel disease (IBD) outcomes, and health care utilization among adults with IBD. The study found strong associations between health confidence and patients' well-being, symptomatic disease activity, opioid use, and glucocorticoid use. Patients with low health confidence (<8) were more likely to have frequent interactions with their healthcare provider, IBD-related emergency department visits, and hospitalizations.

Outcomes of the Supris Sling in an Urban Latina Population

Summary: This study utilized noncontact mapping to analyze global conduction patterns in paroxysmal and persistent atrial fibrillation (AF) and their modification during pulmonary vein (PV) ablation. The results showed different conduction patterns between persistent and paroxysmal AF, with PVs remaining a critical region. PV isolation was effective in eliminating PV-related functional phenotypes and affecting the recurrence of other patterns.

Seeking the Truth About Primary Elective Cesarean Delivery and Pelvic Floor Disorders: A Systematic Review and Meta-Analysis

Summary: A better understanding of the effect of elective cesarean delivery (CD) on pelvic floor disorders is important for obstetricians to counsel pregnant women. This systematic review and meta-analysis found that among primiparous women, elective CD may decrease the risk of urinary incontinence compared to CD after trial of labor or vaginal delivery. However, there was no protective effect against fecal incontinence, anal incontinence, or pelvic organ prolapse. More data on elective CD's effect on pelvic organ prolapse are needed.

Summary: This study demonstrates that human neural progenitor cells modified to release GDNF can be safely transplanted into the spinal cord of ALS patients, providing support cells and GDNF delivery for over 3 years. The transplantation of CNS10-NPC-GDNF has shown to protect spinal motor neurons and have no negative impact on motor function in the treated leg. Furthermore, the transplanted cells have been found to survive and produce GDNF in tissue analysis.

A 24-week feasibility study will be conducted, involving virtual personal training via Zoom and follow-up exercise using recorded sessions. Physical assessments and surveys will be conducted at multiple time points. Further research is needed to understand the feasibility and effectiveness of virtual exercise programming.
