Summary: Corticotroph tumors are pituitary neoplasms arising from the Tpit lineage, with functioning CTs causing Cushing's disease and non-functioning or silent CTs and Crooke's cell subtypes more frequently manifesting as macroadenomas. Tumorigenesis involves disruption of cell-cycle regulators through genetic, epigenetic, and post-transcriptional mechanisms, potentially becoming therapeutic targets. Treatment options include targeted therapies like EGFR inhibitors and HSP90 inhibitors, as well as BRAF inhibitors for specific mutations.

Summary: This study investigates the impacts of urea degradants on urea degradation and disinfection byproduct formation. It finds that while the combination of urea degradant and chlorine can effectively remove urea, it leads to a significant increase in the concentration of disinfection byproducts, particularly brominated DBPs. Therefore, the use of urea degradants together with chlorine should be carefully considered due to potential DBP-related health risks.

Summary: The efficacy of radiation therapy (RT) for recurrent or residual pituitary macroadenoma (PMA) invading extrasellar regions was evaluated in this study. The results showed that conventional RT is safe and effective for the local control of recurrent or residual PMA. Immediate postoperative RT may be beneficial for recurrent or residual PMA.

Summary: This study performed a systematic review and meta-analysis on the therapeutic management of recurrent CD patients. It found that transsphenoidal surgery had a higher remission rate compared to radiotherapy and medical therapy. However, caution should be taken when interpreting the data of medical therapy due to the small number of included cases and wide confidence interval range.

Summary: This study evaluated the sleep parameters of patients with Cushing syndrome (CS) and found that their sleep structure was disturbed, partially reversible after treatment. However, the risk of obstructive sleep apnea remained even a year after treatment, emphasizing the importance of early diagnosis and treatment.

Summary: This case discusses the complex neuropsychiatric manifestations, including complex visual hallucinations and visual impairment, in geriatrics due to recurrent pituitary macroadenoma. Remission of hallucination occurred within 2 weeks of oral administration of 1.5 mg of haloperidol once daily; this is a rare case of Charles Bonnet syndrome after recurrent pituitary macroadenoma.

Summary: Patients with a novel POMC gene variant presented with obesity, hyperphagia, and hypocortisolism, related to an ACTH defect at the prohormone convertase 2 cleavage site. The study demonstrated that adequate cleavage of ACTH to alpha-MSH and d-alpha-MSH is also crucial for feeding control in humans.

Summary: This study reveals the immunoregulatory functions of hypothalamic AgRP neurons in negative energy balance and endotoxemia. Endotoxemia sustains the activity of AgRP neurons and reduces their sensitivity to inhibitory signals. The study also highlights the role of endogenous glucocorticoids in this process.

Summary: This study investigates the role of FURIN in hypothalamic neurons controlling glucose and feeding. The researchers found that FURIN plays an important role in the cleavage of POMC and is involved in the regulation of feeding behavior and glucose homeostasis. The deletion of FURIN in mice resulted in hyperphagia, increased body weight, and altered POMC processing. These findings challenge the previous understanding of POMC cleavage and have implications for understanding the mechanisms of glucose and feeding regulation.
