Biomarkers have been sought after in the field of schizophrenia research for decades. In this paper, we discuss some of the concepts around developing biomarkers in an effort to understand why the use of biomarkers for schizophrenia has not been realized. In particular, we address the following 4 questions. Why would we need a diagnostic biomarker for schizophrenia? How is a biomarker typically defined and how does that influence the discovery of biomarkers in schizophrenia? What is the best use of biomarkers in schizophrenia? Do any biomarkers for schizophrenia currently exist? Thus, while we suggest that no biomarker currently exists for schizophrenia, the heterogeneity associated with schizophrenia will most likely need to be taken into account which will result in multiple biomarkers that identify the multiple underlying pathophysiological processes involved in schizophrenia. Therefore, much additional work will be required prior to obtaining any well-established biomarkers for schizophrenia.

Summary: High levels of glucocorticoids are damaging to the brain, but DHEA has neuroprotective properties. People with schizophrenia have higher levels of DHEA and lower cortisol/DHEA ratios, which are negatively correlated with hippocampal and dorsolateral prefrontal cortex volumes. This suggests that the cortisol/DHEA ratio could be a blood signature of hippocampal and cortical damage in schizophrenia.

Summary: The study identified shared neurobiological processes associated with differences in cortical thickness among multiple psychiatric disorders, suggesting a common role of prenatal development and postnatal functioning of the cerebral cortex. The analysis of data from 145 cohorts revealed that greater expression of genes specific to pyramidal cells was associated with greater group differences in cortical thickness across all 6 disorders.

Summary: The study found no significant interactions between smoking and cognitive or brain morphology in SSD patients, with the exception of a thinner left posterior cingulate in smokers compared to non-smokers. While several main effects of smoking were initially significant in cognitive, volume and thickness analyses, they did not survive FDR correction.

Summary: A genome-wide association study identified significant genetic loci associated with suicide attempts, particularly one locus on chromosome 7 which is implicated in risk-taking behavior, smoking, and insomnia. The study also revealed strong genetic correlations between suicide attempts and psychiatric disorders, as well as other nonpsychiatric traits like smoking, pain, and lower socioeconomic status. Conditioning on psychiatric disorders reduced the genetic correlations between suicide attempts and psychiatric disorders, indicating a shared biology with nonpsychiatric risk factors.

Summary: Elevated levels of pro-inflammatory cytokines and CRP are associated with impairments in cognitive abilities and more severe psychiatric symptoms in schizophrenia. NF-kappa B pathway genes are increased in the brains of schizophrenia patients and are closely related to neuroinflammation. IL-1 beta and IFN-gamma mRNA levels were increased in patients, while IL-6, IL-8, IL-18, and TNF-alpha mRNA levels did not differ. TLR4 and RelB appear to be more related to inflammatory status, while IKK beta and NIK are unique to high inflammation patients.

Summary: The study found that group differences in cortical area for attention-deficit/hyperactivity disorder, schizophrenia, and Child Behavior Checklist were dominant in multimodal association cortices across 11 cortical regions. These differences were associated with interregional profiles of prenatal cell-specific gene expression, implying that prenatal cell-specific processes may lead to deviations from typical brain development and vulnerability to mental illness. Additionally, genes coexpressed with radial glia and endothelial and mural cells were enriched with genes related to known pre/perinatal risk factors for psychosis, supporting a neurodevelopmental model of mental illness vulnerability.

Higher peripheral inflammation index scores are associated with reduced cortical thickness in the temporal lobe. Summary: Left-right asymmetry is an important feature of the healthy brain that may be altered in schizophrenia. This study used a large sample size and consistent methods to investigate structural brain asymmetries in schizophrenia, finding significant differences. These differences may reflect changes at the molecular, cytoarchitectonic, or circuit levels that have functional relevance for the disorder. Reduced left middle temporal cortical thickness is consistent with altered left-hemisphere language network organization in schizophrenia.
