The nail is considered to be a specialised modification of the skin - an 'epidermal appendage'. However, the phrase 'hanging on by your fingernails' belies a vital aspect of nail micro-anatomy that has been overlooked, namely that the nail is functionally integrated with the musculoskeletal system. The current article reviews how the nail is functionally linked to the distal phalanx and several distal interphalangeal joint structures, including extensor tendon fibres and the collateral ligaments. New histological images exploring this link are provided. The extensor tendon in particular continues from its bony insertion to envelop the nail root, and the collateral ligaments form an integrated network on the sides of the joint, helping to anchor the nail margins. This virtual continuum of connective tissue structures merges with a thick periosteum on the distal phalanx and with the numerous cutaneous ligaments that anchor the fatty pads of the finger pulp to the skin. Summary: The physical function of patients with idiopathic inflammatory myopathies (IIMs) is significantly poorer compared to those with non-IIM autoimmune rheumatic diseases (AIRDs) or healthy controls (HCs). Even in patients with inactive disease, physical function remains impaired. Factors such as older age, female gender, longer disease duration, and specific diagnoses were identified as independent factors contributing to lower PROMIS PF-10a scores.

Summary: In this study, hesitancy in patients with autoimmune rheumatic diseases (AIRDs) towards COVID-19 vaccines was investigated. The results showed a decrease in hesitancy, but an increase in concerns over the long-term safety of the vaccine. Patients with idiopathic inflammatory myopathies (IIMs) expressed more skepticism towards vaccine science and had more concerns over long-term safety.

Summary: This study aimed to compare the occurrence of adverse events (AEs) after COVID-19 vaccination in patients with SLE, AIRDs, nrAIDs, and healthy controls. The results showed that AEs and hospitalization rates were similar among SLE patients compared to other groups, and the majority of AEs were minor. The frequencies of AEs were similar across different vaccine types, and the frequency of chills was lower in SLE patients using antimalarial drugs.

Summary: Using global surveys, this study investigated the incidence, predictors, and patterns of flares following COVID-19 vaccination in individuals with autoimmune rheumatic diseases (AIRDs). The results showed that flares occurred in nearly 1 in 10 AIRDs patients after vaccination, with higher vulnerability in patients with comorbidities, mental health disorders, and those receiving the Moderna vaccine. The presence of multiple autoimmune diseases and mental health disorders, as well as receiving the Moderna vaccine, were predictive of flares, while the use of MMF and glucocorticoids were protective. Further research and optimization of vaccine strategies are needed.

Summary: This study investigated the safety of COVID-19 vaccines in pregnant and breastfeeding women with autoimmune diseases. The results showed that adverse events were more frequent in pregnant women compared to non-pregnant patients, but there was no significant difference compared to pregnant healthy controls. There was no difference in adverse events between breastfeeding women and healthy controls. Summary: Limited evidence on long-term COVID-19 vaccine safety in patients with idiopathic inflammatory myopathies (IIMs) contributes to vaccine hesitancy. This study analyzed delayed-onset vaccine adverse events (AEs) in patients with IIMs, other systemic autoimmune and inflammatory disorders (SAIDs), and healthy controls (HCs). The findings showed that vaccination is safe in individuals with IIMs, with AEs and hospitalization rates comparable to SAIDs, and mostly limited to those with autoimmune multimorbidity and active disease. These observations can inform guidelines for identifying high-risk patients in the post-vaccination period.

Summary: The safety profile of COVID-19 vaccines in patients with systemic sclerosis (SSc) was compared with other rheumatic and non-rheumatic autoimmune diseases, as well as healthy controls. The study found that short-term adverse events following vaccination did not differ significantly between the different groups, and background immunosuppression and disease activity did not influence these events.
