Persistent low microcirculatory vessel density in nonsurvivors of sepsis in pediatric intensive care

Objective: To investigate the time course and predictive value of microvascular alterations in children with severe sepsis. Design: Single-center, prospective observational study. Setting: Intensive care unit of a level III university children's hospital. Patients: Patients with septic shock, requiring the administration of fluid and vasopressor agents and/or inotropes after the correction of hypovolemia, who were intubated and ventilated, were included. Interventions: None. Measurements and Main Results: The microcirculation was assessed in the buccal mucosa, using orthogonal polarization spectral imaging, within 24 hrs after admission. Subsequent measurements were performed every 24 hrs for 3 days. The measurements were discontinued when the patient was extubated. There were no significant differences in the functional capillary density or microvascular flow index for all vessel types between survivors and nonsurvivors on day 1. In the survival group, the functional capillary density increased significantly between day 1 and day 2 from 1.7 cm/cm(2) (0.8-3.4) to 4.3 cm/cm(2) (2.1-6.9) (p = .001). Functional capillary density values in nonsurvivors did not change (day 1: 3.2 cm/cm(2) [0.8-3.8]; day 2: 1.9 cm/cm(2) [1.0-2.1]). The median functional capillary density on days 2 and 3 were significantly lower in nonsurvivors (day 2: 1.9 cm/cm(2) [1.0-2.1] vs. 4.3 cm/cm(2) [2.1-6.9], p = .009; day 3: 1.8 cm/cm(2) [1.0-2.0] vs. 4.7 cm/cm(2) [2.1-8.6], p = .01). The microvascular flow index for all vessel types improved in survivors and did not change in nonsurvivors. Differences in microvascular flow index values between survivors and nonsurvivors were not significant. Conclusion: Persistent microcirculatory alterations can be prognostic for survival in children with septic shock. (Crit Care Med 2011; 39:8-13)

Quantification of stress exposure in very preterm infants: Development of the NeO-stress score

Summary: This study developed an instrument to validly determine the cumulative stress exposure for preterm infants in a Neonatal Intensive Care Unit (NICU). Through two consecutive questionnaires, NICU nurses and physicians rated the relevance and stressfulness of 77 potential stress-inducing procedures. The resulting NeO-stress score consists of 38 items with a stressfulness index, providing a quantification of stress in preterm infants.

COMFORTneo scale: a reliable and valid instrument to measure prolonged pain in neonates?

Summary: The reliability and validity of the COMFORTneo scale, used to measure prolonged pain in neonates, were studied. A prospective observational study was conducted to evaluate four clinimetric properties of the COMFORTneo scale based on assessments by NICU nurses. The results indicated that the COMFORTneo scale can reliably and validly assess pain in NICU patients.

Hospitalized Patients' Sleep Quality Compared Between Multioccupancy Rooms and Single-Patient Rooms

Summary: This study aimed to compare the sleep quality of patients in a former hospital with two-and four-bedded rooms to that in a new hospital with exclusively single-patient rooms (SPRs) designed as a healing environment. The results showed that the self-reported sleep quality of patients in the hospital with 100% SPRs was slightly better than that of patients staying in multioccupancy rooms with fellow patients.

Parenthood after severely endangered pregnancy and delivery of a prematurely born child: An interview study with fathers and mothers

Summary: This study aims to gain insight into the development of parenthood six months after delivery of a preterm infant due to the mother's preeclampsia or HELLP syndrome. In this qualitative interview study, we conducted individual interviews with the Working Model of the Child Interview to tap into the subjective experiences of parents who went through preterm delivery due to preeclampsia or HELLP. The interview data were analyzed using thematic analysis. Many statements made clear that these parents are undergoing a distinctive development, and psychological support should help parents regulate the shock, process traumatic experiences and integrate the frightening life event into parenthood development.

Supporting early parenthood of hospitalised women with severe pregnancy problems and their partners: rationale, principles and exploration of a psychotherapeutic programme

Summary: This paper describes a psychotherapeutic programme for pregnant women with pre-eclampsia or HELLP syndrome and their partners, aiming to improve the experience of early parenthood and address the psychological consequences of these conditions. The programme provides continued in-hospital support at the obstetric and neonatology ward and includes mentalization-based psychotherapeutic work. A small study presented in this paper showed that participating parents were committed and no dropouts occurred.

Integrative nursing in Europe-A competency profile for nursing students validated in a Delphi-study

Summary: This study aims to develop and validate integrative nursing learning outcomes and competency profile for bachelor nursing students. Through experts' ratings and comments, 35 learning outcomes were identified, with 24 of them deemed appropriate. These learning outcomes include general knowledge about integrative nursing, safety regulations, communication skills, ethical competencies, and the ability to apply evidence-based complementary therapies and non-pharmacological interventions. The results of this study are important in structuring teaching modules for nursing students in integrative nursing practice.

The self-regulation skills instrument in transplantation (SSIt): Development and measurement properties of a self-report self-management instrument

Summary: The objective of this study was to develop a self-management instrument for organ transplant recipients that incorporates self-regulation skills and determine its measurement properties. The instrument was evaluated based on various guidelines and showed good validity and reliability. The final version consists of 21 items divided into two scales: "Setbacks" and "Successes". This instrument can provide valuable insights and support for self-management after transplantation.

This study aimed to investigate the impact of implementing a new evidence-based protocol on nurses' knowledge, attitude, and adherence to oral care.

Low-value home-based nursing care: A national survey study
