Implementing these features in the existing classification system has the potential to improve prognosis estimation and patient selection for adjuvant treatment.

Summary: In 2022, it is projected that around 600,000 deaths from cancer will occur, with over 50,000 of them being due to colorectal cancer (CRC). The mortality rate for CRC in the US has seen a significant decrease over the past few decades, dropping by 51% from 1976 to 2014. This decrease can be attributed to advancements in therapeutic approaches, particularly since the 2000s, as well as increased awareness of risk factors and improvements in diagnosis. The introduction of several drugs since the 1960s, such as five-fluorouracil, irinotecan, capecitabine, and later oxaliplatin, has been instrumental in the treatment of metastatic CRC (mCRC). Furthermore, the emergence of precision oncology, which utilizes patient and tumor characteristics to guide treatment decisions, has opened up new opportunities for targeted therapies. This review aims to summarize current literature on targeted therapies for CRC, with a focus on the molecular biomarkers involved and their pathways.

Summary: Oligometastatic disease is a clinical state between localized cancer and widespread metastatic disease. Recent studies have demonstrated improved survival in patients with oligometastases when aggressive local treatments are combined with systemic therapies. This review outlines the latest treatment options for oligometastatic colorectal cancer (CRC), taking into account its molecular patterns. Various local treatment strategies, such as surgery, stereotactic body radiation therapy (SBRT), thermal ablation, trans-arterial chemoembolization (TACE), and selective internal radiotherapy (SIRT) are discussed. The role of modern radiologic techniques in identifying patients suitable for potentially curative locoregional treatments is also highlighted.

Summary: The study aimed to investigate whether transanal reinforcement of the suture line could prevent anastomotic leakage after rectal cancer surgery. Summary: Evolving evidence suggests that cancer-derived extracellular vesicles (EVs) play a crucial role in facilitating directional metastasis in colorectal cancer to the peritoneal surface. These EVs can induce cell apoptosis, reduce invasiveness, and promote tumor invasion. Additionally, mesothelial cells release EVs that contribute to tumor invasion and trigger pro-invasive systems in tumor cells.

Quality of life meta-analysis following coloanal anastomosis versus abdominoperineal resection for low rectal cancer

Summary: In low rectal cancers without sphincter involvement, coloanal anastomosis (CAA) can be chosen as an alternative to abdominoperineal excision (APR) to avoid permanent stoma and maintain better quality of life outcomes. This study analyzed comparative data on quality of life outcomes for patients undergoing CAA versus APR and found that CAA was associated with higher physical functioning and body image scores, while APR was associated with increased male sexual problems. Patients who had an APR reported more fatigue, dyspnea, and appetite loss, while those who had a CAA reported higher scores for constipation and diarrhea.

Anoplasty for anatomical anal stenosis: systematic review of complications and recurrences

Summary: This study aimed to determine the rates of complications and recurrence after anoplasty for anatomical anal stenosis (AS) and compare the outcomes for different techniques. The results showed that complications and recurrence were significantly lower after house flap compared with rhomboid/diamond and Y-V flap. Well-designed multicenter studies with longer follow-up are needed to confirm these findings.

Efficacy and safety of Propionibacterium extract gel versus glyceryl trinitrate ointment in the treatment of chronic anal fissure: a randomized controlled trial

Summary: The aim of this study was to compare the efficacy and safety of a Propionibacterium extract gel (PeG) and 0.4% glyceryl trinitrate ointment (GTN) in patients with chronic anal fissure (CAF). The results showed no difference in healing rates between the two treatments, but PeG was more cost-effective and associated with fewer adverse events.

Summary: This paper presents a new protocol using SPME-GC-MS to monitor changes in endogenous VOCs released by resected colonic tissues. The VOCs from breath samples of patients were analyzed and compared with tissue samples to find a possible correlation between exhaled methylbenzene and benzaldehyde levels and the presence of colonic adenomatous polypoid lesions.

Optimization of a Breath Analysis Methodology to Potentially Diagnose Transplanted Kidney Rejection: A Preclinic Study

Summary: Chronic kidney disease (CKD) can lead to end-stage renal disease and increased mortality. Kidney transplantation is the only definitive treatment, but organ rejection is a risk in the short-medium term. This preclinic study proposes the optimization of a non-invasive and cheap TD-GC-MS protocol to monitor CKD patients before and after surgery, with the aim of predicting kidney rejection. Using the ReCIVA((R)) device, the breath of healthy and pathological subjects was analyzed, identifying 67 molecules and quantifying 13 compounds with high prognostic values.
