Transanal reinforcement of low rectal anastomosis versus protective ileostomy after total mesorectal excision for rectal cancer. Preliminary results of a randomized clinical trial

Summary: The study aimed to investigate whether transanal reinforcement of the suture line could prevent anastomotic leakage after rectal cancer surgery. Preliminary data indicated that transanal reinforcement of the suture line in selected patients had a similar anastomotic leakage rate to covering ileostomy, suggesting the potential for avoiding ileostomy in this group of patients.

The Interaction between Reactive Peritoneal Mesothelial Cells and Tumor Cells via Extracellular Vesicles Facilitates Colorectal Cancer Dissemination

Summary: Evolving evidence suggests that cancer-derived extracellular vesicles (EVs) play a crucial role in facilitating directional metastasis in colorectal cancer to the peritoneal surface. These EVs can induce cell apoptosis, reduce invasiveness, and promote tumor invasion. Additionally, mesothelial cells release EVs that contribute to tumor invasion and trigger pro-invasive systems in tumor cells.

Quality of life meta-analysis following coloanal anastomosis versus abdominoperineal resection for low rectal cancer

Barry Maguire, Cillian Clancy, Tara M. Connelly, Brian J. Mehigan, Paul McCormick, Donato F. Altomare, Martijn Pieter Gosselink, John O. Larkin

Summary: In low rectal cancers without sphincter involvement, coloanal anastomosis (CAA) can be chosen as an alternative to abdominoperineal excision (APR) to avoid permanent stoma and maintain better quality of life outcomes. This study analyzed comparative data on quality of life outcomes for patients undergoing CAA versus APR and found that CAA was associated with higher physical functioning and body image scores, while APR was associated with increased male sexual problems. Patients who had an APR reported more fatigue, dyspnea, and appetite loss, while those who had a CAA reported higher scores for constipation and diarrhea.

Anoplasty for anatomical anal stenosis: systematic review of complications and recurrences

Gaetano Gallo, Arcangelo Picciariello, Gian Luca Di Tanna, Patrizia Pelizzo, Donato Francesco Altomare, Mario Trompetto, Giulio Aniello Santoro, Franco Roviello, Carla Felice, Ugo Grossi

Summary: This study aimed to determine the rates of complications and recurrence after anoplasty for anatomical anal stenosis (AS) and compare the outcomes for different techniques. The results showed that complications and recurrence were significantly lower after house flap compared with rhomboid/diamond and Y-V flap. Well-designed multicenter studies with longer follow-up are needed to confirm these findings.

Efficacy and safety of Propionibacterium extract gel versus glyceryl trinitrate ointment in the treatment of chronic anal fissure: a randomized controlled trial

Gaetano Gallo, Mario Trompetto, Serena Fulginiti, Marco La Torre, Simone Tierno, Francesco Cantarella, Paolo Vanini, Giovanni Tomasicchio, Donato Francesco Altomare, Marcella Rinaldi, Giulio Aniello Santoro, Giuseppe Curro, Ugo Grossi

Summary: The aim of this study was to compare the efficacy and safety of a Propionibacterium extract gel (PeG) and 0.4% glyceryl trinitrate ointment (GTN) in patients with chronic anal fissure (CAF). The results showed no difference in healing rates between the two treatments, but PeG was more cost-effective and associated with fewer adverse events.

Summary: This paper presents a new protocol using SPME-GC-MS to monitor changes in endogenous VOCs released by resected colonic tissues. The VOCs from breath samples of patients were analyzed and compared with tissue samples to find a possible correlation between exhaled methylbenzene and benzaldehyde levels and the presence of colonic adenomatous polypoid lesions.

Optimization of a Breath Analysis Methodology to Potentially Diagnose Transplanted Kidney Rejection: A Preclinic Study

Nicoletta De Vietro, Antonella Maria Aresta, Arcangelo Picciariello, Donato Francesco Altomare, Giuseppe Lucarelli, Alessia Di Gilio, Jolanda Palmisani, Gianluigi De Gennaro, Carlo Zambonin

Summary: Chronic kidney disease (CKD) can lead to end-stage renal disease and increased mortality. Kidney transplantation is the only definitive treatment, but organ rejection is a risk in the short-medium term. This preclinic study proposes the optimization of a non-invasive and cheap TD-GC-MS protocol to monitor CKD patients before and after surgery, with the aim of predicting kidney rejection. Using the ReCIVA((R)) device, the breath of healthy and pathological subjects was analyzed, identifying 67 molecules and quantifying 13 compounds with high prognostic values.
