Summary: Lung cancer is a major cause of cancer-related death, and early diagnosis and treatment are crucial for improving the survival rate. MiRNAs have emerged as promising biomarkers for lung cancer due to their high sensitivity and specificity. In the regulation of non-small cell lung cancer, miRNAs play important roles as oncogenic factors, tumor suppressors, and regulators. Dysfunctional miRNAs negatively regulate target genes involved in cell proliferation, invasion, apoptosis, cell cycle disorder, and angiogenesis. This review provides a summary of the biological process and recent advances in the mechanisms of miRNAs in tumor formation and development, with a focus on their potential for early diagnosis of lung cancer through circulating free and extracellular vesicles.

Summary: A machine learning-based platform, CIRI, was developed to predict the ICI response of patients with NSCLC based on peripheral blood cytokine profiles. The CIRI model accurately determines the patients who would benefit from anti-PD-1/PD-L1 therapy with prolonged overall survival, aiding in clinical decision-making.

Summary: This study aimed to identify predictive biomarkers for first-line pembrolizumab treatment in NSCLC patients. Patients with durable clinical benefit showed greater infiltration of immune cells, with CD8 T-cell and NK cell scores strongly associated with durable benefit. XCL1/2 demonstrated high performance in predicting durable response to pembrolizumab, outperforming PD-L1 expression levels. These predictive biomarkers can improve patient selection for optimized treatment outcomes.

Summary: Despite advancements in lung cancer treatment, the 5-year survival rate remains low. This study aimed to develop a risk score based on CT radiomics to predict survival in NSCLC patients. The risk score model outperformed clinical indicators, and could effectively stratify patients into high and low-risk groups for survival prediction.

Summary: Tumor-educated Platelets (TEPs) have been identified as rich sources of cancer-related RNA profiles in liquid biopsies, potentially useful for cancer detection. In this proof-of-concept study, we investigated whether the circRNA signature of platelets could be utilized as a liquid biopsy biomarker for the detection of non-small cell lung cancer (NSCLC). Our findings suggest that circRNAs in blood platelets, such as circNRIP1, may serve as potential diagnostic biomarkers for NSCLC detection using liquid biopsies.

Summary: The study aimed to find diagnostic and/or prognostic biomarkers in non-small cell lung cancer (NSCLC) patients based on circulating microRNAs in extracellular vesicles (EVs). Results showed significantly lower levels of miR-23a and miR-let7i in NSCLC patients compared to controls, with potential diagnostic value. ROC curve analysis demonstrated the potential of miR-23a and miR-let7i as non-invasive markers for NSCLC diagnosis.
