Summary: This study suggests that specific HLA-DQB1 eplet mismatch is significantly associated with T cell-mediated rejection in liver transplant recipients, serving as a predictive factor for risk assessment. Both DQB1-EpMM and DQB1-PIRCHE-II are related to de novo donor-specific antibody formation.

Summary: This study provides a comprehensive analysis of the natural course of dnDSA after kidney transplantation, demonstrates the impact of MFI evolution on graft outcomes, and identifies a subgroup of patients with a stable disappearance of dnDSA associated with better allograft survival.

Summary: The importance of antibodies, especially neutralizing antibodies, has been recognized for many years. Measuring the levels of antigen-specific antibodies is easier than measuring the T-cell response when examining immune responses to a pathogen after vaccination or infection, but it does not provide a complete picture. Determining the levels of neutralizing antibodies is more challenging, but it gives a better indication of the quality of the antibody response. Inducing long-lived antibody-secreting plasma cells is crucial for a persistent humoral immune response.

Summary: The study showed that blocking IL-21R can inhibit the generation of Tfh cells, promote Tfr differentiation, and inhibit DSA production. In a transplant model, alpha IL-21R significantly reduced Tfh/Tfr ratio and germinal center response, leading to a significant decrease in DSA levels after transplantation, indicating a potential therapeutic role in preventing chronic antibody-mediated rejection.

Summary: Autism Spectrum Disorder (ASD) is a complex neurodevelopmental disorder characterized by deficits in social communication, restricted interests, and repetitive behavior. The immune hypothesis plays a major role in ASD pathogenesis and explains the variability in clinical phenotypes and comorbidities. Evidence suggests a link between immune dysfunction and behavioral traits in ASD. This review summarizes current understanding of immune dysfunction in ASD, particularly the impact of maternal immune factors, comorbidities, and other relevant factors.

Summary: This study evaluated the efficacy of high-dose intravenous immunoglobulin (IVIG) and rituximab combination therapy for subclinical de novo donor-specific antibody (dnDSA). The results showed that preemptive treatment with high-dose IVIG combined with rituximab did not show a better dnDSA reduction compared with rituximab alone.

Summary: TFH cells and TFR cells play important roles in the formation of dnDSA after renal transplant. Activation of TFH cells is associated with the development of dnDSA, while TFR cells have an inhibitory effect on TFH cells.

Summary: This study revealed a positive correlation between de novo DSA and p-S6RP expression in pneumocytes and macrophages using IHC. p-S6RP was found to be relatively sensitive, specific, and had superior inter-rater reliability compared to C4d.

Summary: HLA-DPB1 eplet and TerEp mismatches have a significant negative impact on graft survival in kidney transplantation, while mismatches at allele or epitope level do not significantly affect the risk of developing dnDSA. Matching for HLA-DPB1 at epitope level instead of allele level may potentially improve graft survival.

Summary: Most transplant recipients with de novo DSA show histological signs of rejection, even in the absence of proteinuria and deterioration of graft function, indicating the need for routine allograft biopsy following DSA detection.

Summary: This study evaluated the dynamics of Torque teno virus (TTV) viral load in kidney transplant patients during the first year post-transplant and its relationship with infection and the formation of donor-specific antibodies (DSAs). The results showed that an increase in TTV viral load at 1 month post-transplant was associated with an increased risk of infection, while a decrease in TTV viral load at 12 months post-transplant was associated with the development of DSAs. Therefore, early evaluation of TTV viral load is an effective strategy for predicting the risk of infection after transplantation.

Summary: This study designed a novel regimen for the treatment of late or chronic active antibody-mediated rejection (AMR) in renal transplant recipients. The regimen included early intensive therapy with daratumumab plus plasmapheresis/IVIG and later maintenance therapy with daratumumab alone. The results showed that this daratumumab-based regimen had promising results in treating refractory late or chronic active AMR.

Summary: This study evaluated the association between 25-hydroxyvitamin D levels and rejection reactions, antibody-mediated rejection, and graft survival in kidney transplant recipients. They found that patients with sufficient 25(OH)D levels had fewer clinical rejection reactions and longer occurrence-free survival of donor-specific antibodies. Additionally, sufficient 25(OH)D levels were associated with longer graft survival in the first three years post-transplant.
