Summary: A new in vivo hemopoietic stem cell gene therapy approach has been developed without the need for myelosuppressive conditioning and autologous HSC transplantation, involving HSC mobilization and a helper-dependent adenovirus vector system. The mobilization regimen using truncated GRO-beta and plerixafor resulted in robust HSC mobilization with less toxicity. In a mouse model, in vivo HSC transduction after mobilization led to complete correction of thalassemia, demonstrating the potential for long-term HSC repopulation.

The quantity of CD34(+) cells mobilized by plerixafor varies among SCD patients for unknown reasons, and factors influencing this yield include age, baseline and pre-apheresis CD34(+) cell counts, and disease severity markers unique to SCD.
