Summary: In the human embryo, the genetic program responsible for germ cell specification involves a balance between epigenetic and transcriptional mechanisms, resulting in a unique cell population that can exhibit both germness and pluripotency features. Germ cell tumors, originating from fetal or neonatal germ cells, maintain this balance and can display a wide range of phenotypes with either pluripotent or germness characteristics. This review highlights the similarities in transcriptional and epigenetic programs between human primordial germ cells and germ cell tumors.

Summary: In this review, the current research progress on the pathogenesis, diagnosis, and treatment of extragonadal germ cell tumors (EGCTs) from an immunology perspective are summarized. EGCTs have a histological origin related to gonads but are located outside. They can occur in various areas and have great morphological variation. The unclear pathogenesis and extensive differential diagnosis of EGCTs pose challenges. This review provides ideas for applying immunology in combating these diseases, which is currently a hot topic.

Summary: We report a very rare case of immature gastric teratoma in an infant based on clinicoradiological and pathological findings. The patient was a 6-month-old male child born through full-term vaginal delivery. He presented with abdominal distension and a palpable mass, along with a history of Malena for the past 10 days. Computed tomography scan revealed a large well-defined multilobulated solid cystic lesion with multiple calcifications. Surgical exploration showed a tumor mass arising from the lesser curvature of the posterior wall of the stomach. Total excision of the mass was performed, and the child is currently doing well on regular follow-up visits.

Summary: This study describes a heterogeneous group of extragonadal germ cell tumors (EGCTs) and examines their clinicopathological correlation. The findings reveal that EGCTs predominantly occur in post-pubertal adolescents and young adults, with common sites being the mediastinum, sacrococcyx, retroperitoneum, and central nervous system. The different types of germ cell tumors were confirmed using immunohistochemistry markers. After treatment, tumor marker levels decreased significantly or remained normal.

Summary: This study aims to present the current evidence and development of studies on the management of extragonadal germ cell tumors (EGCT). The results show that the current insights on EGCT are limited, mainly based on case series and studies with small patient numbers. Non-seminomatous EGCT have a poorer prognosis compared to testicular germ cell tumors, while seminomatous EGCT have a similar prognosis to patients with metastatic testicular seminoma. Systemic treatment should be performed like in testicular metastatic germ cell tumors, but upfront dose intensification of chemotherapy should be considered for mediastinal non-seminomatous EGCT patients.

Summary: Overexpression of prostate-specific membrane antigen (PSMA) is demonstrated in various tumors, and radionuclide therapies directed at PSMA might be effective in treating PSMA-avid malignancies. A case study of a 46-year-old male with progressive metastatic testicular mixed germ cell tumor showed mixed response in Ga-68-PSMA PET/CT after Lu-177-PSMA therapy, suggesting the need for further optimization of treatment strategies.
