Summary: Hand eczema is a common inflammatory skin disorder. This article aims to update the European Society of Contact Dermatitis guideline on the diagnosis, prevention, and treatment of hand eczema. Consensus was reached for several statements and management strategies through analysis of evidence for therapeutic and preventive interventions using a structured method of developing consensus. The updated guideline should improve the management of hand eczema.

Summary: This study aimed to examine the treatment patterns of atopic dermatitis (AD) in Asians, and found significant differences in treatment between pediatric and adult patients. Topical corticosteroids and antihistamines were the most commonly prescribed medications, but the use of topical corticosteroids decreased with age. Cyclosporine was the most frequently prescribed immunosuppressant. Pediatric patients were more likely to discontinue treatment, and restart of treatment for moderate-to-severe AD was earlier than overall AD.

A modified Delphi method was adopted for the consensus process, involving two rounds of email questionnaires and face-to-face meetings. Consensus was reached for the domains of classification, diagnostic evaluation, and treatment, developing a therapeutic ladder for chronic hand eczema.

Summary: Since the first approval for immune checkpoint inhibitors (ICIs) for the treatment of cutaneous melanoma more than a decade ago, immunotherapy has greatly transformed the treatment landscape of this chemotherapy-resistant disease. Combination regimens have gained regulatory approvals for the treatment of metastatic cutaneous melanoma, with the possibility of cure for some patients. However, traditional immunotherapy biomarkers have failed to predict response reliably and ICIs are associated with unique toxicity profiles. The clinical practice guideline on immunotherapy aims to provide guidance for healthcare professionals and improve patient care.

Summary: Chronic hand eczema (CHE) is a burdensome condition with limited treatment options. This review discusses the pathogenesis and currently available treatments for CHE, as well as emerging drugs being studied in clinical trials. Expert opinion suggests that immunomodulatory drugs such as JAK inhibitors and Th2-targeting antibodies hold promise for CHE treatment. Future management of CHE may involve more targeted therapies through clinical trials, moving away from broad immunosuppressants with more side effects.
