Background: Surgical scars are crucial cosmetic problem, especially when in exposed areas such as the anterior neck following thyroidectomy. Objective: To evaluate the impact of post-thyroidectomy scars on quality of life (QoL) of thyroid cancer patients and identify the relationship between scar characteristics and QoL. Methods: Patients with post-thyroidectomy scars on the neck were recruited. QoL was measured using the Dermatology Life Quality Index (DLQI). Scar characteristics were graded according to Vancouver scar scale (VSS) score. Results: Ninety-seven patients completed a battery of questions at the time of enrollment. Post-thyroidectomy scars were classified according to morphology as linear flat scars, linear bulging scars, hypertrophic scars or adhesive scars. There were 32 patients (33.0%), 9 patients (9.3%), 41 patients (42.3%) and 15 patients (15.5%), respectively, in each group. The mean total DLQI score was 9.02. Domain 2 (daily activities, 2.87 points), which includes questions about clothing, was the most greatly impacted among patients. The total DLQI scores of patients who have experienced scar-related symptoms were significantly higher than those of patients without symptoms (p < 0.05). The VSS scores were 3.09 for linear flat scars, 6.89 for linear bulging scars, 6.29 for hypertrophic scars and 5.60 for adhesive scars. However, the DLQI scores did not significantly differ among scar types or VSS scores. Conclusion: Post-thyroidectomy scars on the neck affect the QoL of thyroid cancer patients regardless of scar type. Therefore, clinicians should pay attention to the psychological effects of scars on patients and take care to minimize post-thyroidectomy scar.

Summary: Fine needle aspiration biopsy (FNAB) has limited diagnostic accuracy in large thyroid nodules with Bethesda Cat. 2 result. In nodules >= 4 cm, there is a high false-negative rate with benign cytology, and these nodules often have suspicious ultrasound findings with a high malignancy rate.

Risk of recurrence of nail unit melanoma after functional surgery versus amputation

Byung Ho Oh, Solam Lee, Jung Won Park, Ju Yeong Lee, Mi Ryung Roh, Kyoung Ae Nam, Kee Yang Chung

Summary: This study retrospectively reviewed patients with Minimally invasive nail unit melanoma (NUM) and identified risk factors for recurrence. Male sex, greater Breslow thickness, amelanotic color, ulcers, and nodules were associated with higher risk of recurrence or distant disease. A Breslow thickness of 0.8 mm was determined to be the optimal cut-off point for predicting recurrence risk after surgery.

Summary: Thyroid cancer can be curable but may recur after treatment. In this study, a novel SERCA inhibitor was identified in sorafenib-resistant thyroid cancer cells, which showed significant tumor shrinkage in a xenograft tumor model. These findings have important clinical implications for developing new strategies to target refractory cancer cells.

Summary: Diffuse sclerosing variant papillary thyroid carcinoma (DSVPTC) is a rare variant of papillary thyroid carcinoma that mainly affects young patients and has distinct clinical, pathological, and molecular characteristics compared to classical PTC. Our study revealed significant age-related differences in DSVPTC, with more aggressive behavior observed in pediatric patients. Through targeted next-generation sequencing, we identified BRAF, KRAS, and TERT as the most important genes in DSVPTC with age-specific differences.

Various Applications of Purse-String Suture and Its Cosmetic Outcome in Cutaneous Surgical Defects

Sujin Park, Yeongjoo Oh, Jong Won Lee, Sooyie Choi, Kyoung Ae Nam, Mi Ryung Roh, Kee Yang Chung

Summary: This study retrospectively reviewed patients who underwent purse-string sutures between January 2015 and December 2019. The results showed that multilayered purse-string suture had the shortest repair duration and effectively minimized the scar size. The average Vancouver scar scale measured at least 6 months postoperatively was 1.62, with a risk of hypertrophic scarring of 8.6%. There was no significant difference in scar assessment and risk of hypertrophic scarring between different surgical method groups.
