Epidemiological evidence has shown that abdominal obesity is closely associated with the development of cardiovascular (CV) disease, suggesting that it might be considered as an independent CV risk factor. However, the pathophysiological mechanisms responsible for the association between these 2 clinical entities remain largely unknown. Adipocytes are considered able to produce and secrete chemical mediators known as adipokines that may exert several biological actions, including those on heart and vessels. Of interest, a different adipokine profile can be observed in the plasma of patients with obesity or metabolic syndrome compared with healthy controls. We consider the main adipokines, focusing on their effects on the vascular wall and analyzing their role in CV pathophysiology.

Summary: This study aimed to evaluate the prognostic implications of transient or in-hospital persistent acute kidney injury (AKI) in patients with acute coronary syndrome (ACS). The results showed that among ACS patients undergoing invasive management, in-hospital persistent AKI was associated with an increased risk of adverse cardiovascular events at 30 days.

Summary: Aortic stenosis (AS) commonly coexists with other valvular diseases or is combined with aortic regurgitation (AR), resulting in unique pathophysiological conditions. The combination of affected valves varies widely, leading to a lack of standardized diagnostic or therapeutic approaches. Echocardiography, although crucial, may be challenging due to the hemodynamic interactions between combined valvular defects. Hence, a multimodality imaging approach, such as computed tomography or cardiac magnetic resonance, is often crucial in diagnosing and understanding valvular pathology.

Summary: The role of coronary calcification in patients presenting with acute coronary syndromes (ACS) and different revascularization strategies has been investigated. This study evaluates the role of coronary calcification, detected by coronary angiography, in the overall spectrum of patients with acute ACS. The results suggest that patients with severe coronary calcification have worse clinical outcomes regardless of the management strategy.

Summary: Echocardiography is the most important diagnostic tool for evaluating aortic stenosis, and a standardized and stepwise approach is necessary for a comprehensive evaluation. Technical innovations in echocardiography have improved accuracy and detailed evaluation of aortic valve anatomy. Early diagnosis is crucial for prompt treatment and improved patient outcomes. Detailed evaluation of left ventricle function changes has prognostic significance, and efforts have been made to identify accurate cut-off values. Artificial intelligence is emerging as a promising tool for aortic stenosis screening and diagnosis, reducing misdiagnosis rates.

Summary: Acute decompensation often heralds the onset of symptoms and signs of acute heart failure in severe degenerative aortic stenosis (AS), resulting in poor survival and quality of life. The management of this condition lacks a standardized approach. Medical therapy is limited due to potential worsened hemodynamics in the presence of AS. Urgent aortic valve replacement is usually performed through transcatheter rather than surgical approach, and percutaneous balloon valvuloplasty has gained renewed interest as a bridge to definitive therapy. This review provides insights into the pathophysiological aspects of acute advanced AS and summarizes the current evidence on its management.

Summary: This study aims to evaluate the practice of right heart catheterization (RHC) among the interventional cardiology community. The survey results show that most centers perform few RHC procedures per year and lack a dedicated cardiologist. The most common indication for RHC is the evaluation of pulmonary hypertension, followed by the diagnostics of valvular diseases and advanced heart failure/heart transplantation.
