Chronic exposure to high doses of selenium in the first trimester of pregnancy: Case report and brief literature review

BackgroundObstetricians usually prescribe supplements during pregnancy without actual indication. The use of selenium during pregnancy has increased, due to its function in several antioxidant mechanisms. CaseA pregnant woman received 200,000 micrograms (g) per day of a selenium galenic formulation, since gestational week (g.w.) 7 to 12, due to a prescription error. The patient experienced nausea, vomiting, hand and foot paresthesia, followed by fatigue, loss of fingernails and hair. The woman was referred to our Fetal and Maternal Medicine Unit for surveillance. The mother's blood selenium levels went back to normal 13 weeks after arrest and the baby was born at term, without complications. Clinical evaluation and imaging studies were normal at one month of age. ConclusionThis is the first case of severe chronic selenium intoxication during the first trimester of pregnancy. In the present case, no consequences of congenital defects or pregnancy complications occurred. However, since vitamins, minerals, and food supplements may be harmful and prescription errors occur, obstetricians should avoid prescribing supplements in the absence of maternal deficiency.

Summary: This study aimed to report the rate of additional anomalies detected exclusively at prenatal MRI in fetuses with isolated severe ventriculomegaly, finding that 18.1% of cases had additional structural anomalies detected only on MRI. The presence of additional anomalies found only on MRI was significantly higher in bilateral compared to unilateral ventriculomegaly cases.

Summary: A retrospective study compared the efficacy of three different treatment regimens for preventing mother-to-fetus transmission of Toxoplasma gondii infection. The combination of Spiramicyn with Trimethoprim-Sulfamethoxazole (Spy+TMP-SMX) was found to be more effective in reducing transmission risk compared to Spiramicyn alone, and it was not inferior to the standard-of-care treatment Pyrimethamine-Sulfadiazine (P/S).

Summary: The use of surgical masks by healthcare workers during the COVID-19 pandemic has led to noticeable negative reactions in infants aged 2-9 months, affecting their emotional behaviors and responses to caregivers. Longitudinal studies are necessary to propose alternative ways to assess pediatric patients in early infancy.

Gender Differences in the Psychopathology of Mixed Depression in Adolescents with a Major Depressive Episode

Summary: This study examines gender differences in the psychopathology of mixed depression among adolescents. The results show that depressive symptoms are more severe in females, while males exhibit higher levels of motor activity and pressured speech. Females also experience higher levels of anxiety and emotional dysregulation. Therefore, mixed depression among adolescents is more severe in females, with a focus on core affective symptoms and excessive fatigue, while males exhibit higher levels of psychomotor activation.

Role of fetal magnetic resonance imaging in fetuses with congenital cytomegalovirus infection: multicenter study

Summary: This study aimed to investigate the role of fetal brain MRI in detecting associated anomalies in fetuses with congenital CMV infection and normal neurosonography. The results showed that fetal brain MRI can detect additional anomalies in fetuses with congenital CMV infection and negative neurosonography, and the viral load in the amniotic fluid is an independent predictor of the risk of associated anomalies in these fetuses.

Summary: This study aimed to describe the use of COVID-19-related medicines during pregnancy and how it evolved between the early and late periods of the pandemic. The results showed that the proportion of pregnant women who used COVID-19-related medicines increased with disease severity. The use of antibiotics, antivirals, and hydroxychloroquine was lower during the late period of the pandemic.

Long term follow-up of scoliosis progression in type II SMA patients

Summary: The aim of this study is to retrospectively assess the onset and progression of scoliosis in untreated type II spinal muscular atrophy (SMA) patients. The results showed a positive correlation between age and scoliosis angles, with scoliosis progressively increasing with age. There was also a progressive increase in scoliosis angles with decreasing functional status scores. The longitudinal analysis revealed that age, functional status, and baseline scoliosis angle were predictive of scoliosis progression. The mean annual rate of increase in scoliosis angle was 5.63.

Urgent Psychiatric Consultations for Suicidal Ideation and Behaviors in Italian Adolescents during Different COVID-19 Pandemic Phases

Summary: Access to the emergency department for acute psychiatric problems, especially suicide attempts, has increased significantly after the COVID-19 pandemic. This study examined the increase in pediatric psychiatric consultations and found that consultations for any reason and suicide attempts increased during the pandemic compared to the pre-pandemic period. Young people who attempted suicide during the pandemic were more likely to use self-poisoning and less likely to use precipitation, and they were more likely to be diagnosed with major depressive disorder.

Congenital lung malformations: a novel application for lung ultrasound?

Summary: Congenital lung malformations (CLMs) are a group of different disorders that are increasingly detected due to the widespread use of antenatal ultrasonography (aUS) and prenatal magnetic resonance imaging (MRI). However, the best postnatal imaging approach is still not well defined, with newborns often undergoing multiple chest X-rays and computed tomography scans to confirm the diagnosis.
