Correlation between Resection Margin and Disease Recurrence with a Restricted Cubic Spline Model in Patients with Resected Hepatocellular Carcinoma

Background: The relationship between resection margin (RM) and recurrence of resected hepatocellular carcinoma (HCC) is unclear. Methods: We reviewed clinical data for 419 patients with HCC. The oncologic outcomes were compared between 2 groups of patients classified according to the inflexion point of the restricted cubic spline plot. Results: The patients were divided according to an RM of <1 cm (n = 233; narrow RM group) or >= 1 cm (n = 186; wide RM group). The 5-year recurrence-free survival (RFS) rate was lower (34.8 vs. 43.8%, p = 0.042) and recurrence near the resection site was more frequent (4.7 vs. 0%, p= 0.010) in the narrow RM group. Patients with multiple lesions, or prior transarterial chemo-embolization (TACE) or radiofrequency ablation (RFA) were excluded from subgroup analyses. In patients with a 2-5 cm HCC, the 5-year RFS was greater in the wide RM group (54.4 vs. 32.5%, p = 0.036). Narrow RM (hazard ratio 1.750, 95% CI 1.029-2.976, p = 0.039) was independently associated with disease recurrence. Conclusion: In patients with a single 2-5 cm HCC without prior TACE/RFA, an RM of >= 1 cm was associated with lower risk of recurrence after liver resection. (C) 2018 S. Karger AG, Basel

Comparative long-term outcomes of laparoscopic hepatectomy and radiofrequency ablation for hepatocellular carcinoma located in the anterolateral segments of the liver

Achievement of textbook outcomes and comparisons with benchmark values after laparoscopic left lateral sectionectomy

Summary: Laparoscopic left lateral sectionectomy (LLLS) is a standard approach for patients, and achieving textbook outcomes (TOs) is crucial. This study identified factors associated with achieving TOs after LLLS. The main determinant for achieving a TO was prolonged hospital stay, while other factors such as year of surgery, ASA score, BMI score, inflow clamping time, and operation time were also significant. Our outcomes were similar to benchmark values for most criteria, except for the transfusion rate, which was worse in our patients. However, we achieved better outcomes in terms of conversion rate, transfusion requirement, inflow clamping, complication rate, reoperation rate, and positive margin.

Summary: This study evaluated the factors affecting the achievement of textbook outcomes (TO) for laparoscopic liver resection (LLR) and its impact on survival. The study found that Model for End-stage Liver Disease (MELD) score, thrombocytopenia, and hypoalbuminemia were independent risk factors influencing the achievement of TO. The TO group had significantly higher overall and recurrence-free survival rates compared to the non-TO group.

A scoring system to predict the risk of major complications after laparoscopic liver resection in elderly patients with hepatocellular carcinoma

Summary: This study proposes a new simplified scoring system based on the Geriatric Nutritional Risk Index (GNRI) to predict major complications after laparoscopic liver resection (LLR) in elderly patients with hepatocellular carcinoma (HCC). The study found that the GNRI, Child-Turcotte-Pugh score, major liver resection, and intraoperative transfusion were independent predictors of major postoperative complications. The scoring system showed good discrimination in predicting major complications.

Extranodal extension influences prognosis in pancreatic head cancer: A retrospective cohort study

Laparoscopic versus open pancreaticoduodenectomy with major vein resection for pancreatic head cancer

The results showed no significant differences in complications, tumor recurrence, pathological outcomes, and disease-free survival between the two groups. The surgical method was not an independent prognostic factor for disease-free survival.

Summary: This study investigated the role of salvage hepatectomy (SH) as a rescue therapy for recurrent hepatocellular carcinoma (HCC) after radiofrequency ablation (RFA). The study found that SH showed safety and effectiveness equivalent to primary resection in terms of short-term and long-term outcomes. However, due to the higher grade and more aggressive behavior of recurrent tumors, more extensive resections with wide surgical margins are necessary to prevent recurrence.

Summary: A novel difficulty scoring system (nDSS) was developed and validated in this study for predicting the surgical difficulty and outcomes of laparoscopic liver resection (LLR) for hepatolithiasis. The nDSS score was found to be significantly associated with the rates of red blood cell transfusion, postoperative hospital stay, and major complications. Patients in the high nDSS group had longer operation time, greater estimated blood loss, higher rates of red blood cell transfusion, longer postoperative hospital stay, and higher major complication rate compared to those in the low nDSS group.
