External beam radiotherapy (EBRT) has been shown to synergize with ICI to prompt remarkable tumor regression and even eradication, but is not suitable for widely disseminated disease. Targeted radiopharmaceutical therapy (TRT) selectively delivers radiation to both the primary tumor and the metastatic sites, and has shown promising results, leading to regulatory approval of certain agents.

Replication stress, which often leads to genomic instability and is closely associated with cancer, can be targeted to induce catastrophic cell proliferation. Radiation therapy, a commonly used treatment for cancer, can induce DNA lesions that affect replication. Combining replication stress regulators with radiation therapy has the potential to enhance tumor radiosensitivity.

Summary: This study investigated the combined efficacy of the tyrosine kinase inhibitor (gefitinib) and bevacizumab in opposing MAPF-induced angiogenesis. The results showed that cotreatment with gefitinib and bevacizumab produced better inhibition of MAPF-induced endothelial angiogenesis than gefitinib alone, particularly in the mutant EGFR subgroup. Protein analysis of MAPF-derived exosomes revealed abundant EGFR and p-EGFR components that implied possible transfer to endothelial cells. The addition of bevacizumab on gefitinib treatment could suppress MAPF-induced angiogenesis in lung adenocarcinoma patients, as indicated by Kaplan-Meier analysis and in vitro studies.

Summary: This study reports the final full clinical analysis of a phase I trial of radiation with ipilimumab in patients with melanoma. Radiotherapy followed by ipilimumab was well tolerated and yielded a response rate that compares favorably to ipilimumab alone. The recommended dose for subsequent phase 2 trials was 8 Gy x 3 doses, based on these results.
