Summary: Solid organ transplant recipients frequently experience tremor, and this study investigates the impact of tremor on activities of daily living and health-related quality of life (HRQoL). Results show that whole blood tacrolimus trough concentration is an independent determinant of mild tremor among these recipients. Severe tremor is strongly associated with lower physical and mental HRQoL. These findings emphasize the need for further research on the effects of tacrolimus on tremor.

Summary: Coccidioidomycosis is a fungal infection that is prevalent in certain regions of the United States, Mexico, and Central and South America. It can cause severe infections in immunocompromised patients, such as solid organ transplant recipients. The timely and accurate diagnosis of coccidioidomycosis in these patients is challenging due to limitations of diagnostic methods, making it important to explore alternative approaches. This review discusses available diagnostic modalities and the role of early diagnosis in guiding antifungal therapy for reducing complications in SOT recipients with coccidioidomycosis.

Summary: The incidence of pancreatic cancer is higher in solid organ transplant recipients, particularly in liver transplant recipients. Compared with the general population, pancreatic cancer is more commonly diagnosed in transplant recipients, especially in cases arising in the head of the pancreas and those diagnosed at localized stage. Risk factors for pancreatic cancer in transplant recipients are consistent with those in the general population, including male sex, older age, non-O blood type, and history of diabetes.

Summary: The current literature on hypertension (HTN) following pediatric solid organ transplant (SOTx) was reviewed, including definition, prevalence, risk factors, outcomes, and treatment. Several new guidelines for pediatric HTN have been published, but no specific recommendations for SOTx recipients. HTN is highly prevalent, yet underdiagnosed and undertreated in kidney transplant recipients, especially with ambulatory blood pressure monitoring. Little data is available regarding its prevalence in other SOTx recipients. HTN in this population is multifactorial and associated with pre-transplant HTN, demographic factors, weight status, and immunosuppression protocol. HTN is associated with subclinical cardiovascular (CV) end-organ damage, but no recent data on long-term outcomes are available. There are no updated recommendations for the optimal management of HTN in this population.

Summary: This study investigated whether patient registries could be used to emulate control cohorts in randomized controlled trials (RCTs) for pediatric kidney transplantation. The results showed that there were no significant differences in various outcome measures between the emulated control cohort and the actual control cohort of the RCT. Therefore, the usage of Real-World Data from patient registries could facilitate the conduct of clinical trials in pediatric kidney transplantation.

Summary: Primary hyperoxaluria (PH) is an inherited disorder that causes overproduction of oxalate, resulting in kidney stones, nephrocalcinosis, and kidney failure. Diagnosis and management of PH are challenging due to its rarity and variable clinical expression. However, recent developments in outcome data and therapies, including RNA interference, have led to the need for updated guidelines on PH. A consensus group formulated and graded practical statements on the diagnosis and management of PH based on existing evidence.

Summary: This study reports two cases of acute tubulointerstitial nephritis (ATIN) in children, one caused by leptospirosis in a 12-year-old boy and the other caused by hantavirus in a 10-year-old girl. Both patients presented with jaundice, elevated serum creatinine, and thrombocytopenia, with urine analysis revealing tubular proteinuria. Kidney biopsy confirmed tubulointerstitial inflammation. Both children showed excellent clinical and biochemical response to glucocorticoid treatment.

However, there is a lack of guidelines for monitoring and managing post-transplant hypertension.
