Chemotherapy in head and neck osteosarcoma: Adjuvant chemotherapy improves overall survival

Background: Osteosarcoma is an aggressive bone malignancy presenting uncommonly in head and neck sites. Surgery is mainstay in treatment. However; trials show an improved survival with addition of chemotherapy in the treatment of extremity osteosarcoma. The head and neck osteosarcomas(HNOs) were excluded in these trials because of atypical presentation and disease course. Further; sufficient numbers were not possible for a trial. We present the largest retrospective study from single institute investigating the role of chemotherapy in the management of HNOs. Patients and methods: The retrospective cohort of HNOs treated from 2007 to 2015 of a tertiary hospital were charted. The therapeutic and prognostic factors were analyzed for overall survival(OS), disease free survival (DFS), local control(LC) and metastasis(MT) in univariate and multivariate analysis. The minimum and median period of follow up was 12 months and 56.04 months respectively. Results: There was a total of 157 patients definitively treated with surgery in the time period. 7 patients had positive margins and all were maxillary or skull base tumors. The multivariate cox regression showed significance of tumor site(p = 0.034), margin status (p = 0.006), chemotherapy(p = 0.025), histological subtype (p = 0.012) as predictors of overall survival. The margin status(p = 0.002), Radiotherapy(p = 0.005) were significant predictors for local recurrence. The age and histology subtype(p = 0.058) were borderline significant predictors of metastasis(p = 0.065). The KM method for OS of different chemotherapy groups(p = 0.013), and survival with and without chemotherapy(p = 0.007) was significant. The OS was significantly better with adjuvant chemotherapy among various treatment plans(p = 0.034). Conclusion: Adjuvant chemotherapy improved OS while adjuvant radiotherapy improved local control of HNOs.

A novel clinical signature predicts the survival of elderly patients with oral squamous cell carcinoma

Summary: This study analyzed the risk factors for survival in elderly patients with oral squamous cell carcinoma and developed a novel clinical signature for prognosis. Results indicated that high N stage, low hemoglobin level, low BMI, and high NLR were associated with poor survival outcomes. The constructed nomogram showed a good predictive ability.

This study found that tumor size, location, and preoperative neutrophil-to-lymphocyte ratio may be associated with the risk of malignant transformation in HNIMTs. Postoperative radiotherapy can benefit HNIMT patients at high risk of malignant transformation.

Summary: This study provides further insights into the risk factors for esophageal squamous cell carcinoma (ESCC) in patients with a history of head and neck squamous cell carcinoma (HNSCC). The study found that HNSCC patients, especially drinkers with an alcohol flushing response and those with high Ki67 labeling index (Ki67 LI) in HNSCC tissue, were more likely to develop ESCC.

Cancer cells co-opt nociceptive nerves to thrive in nutrient-poor environments and upon nutrient-starvation therapies

Summary: This study reveals that cancer cells can utilize nociceptive nerves to survive in nutrient-poor environments. Cancer cells secrete NGF through ROS-triggered activation of c-Jun, which stimulates nerves to produce CGRP. The neurogenic CGRP induces cytoprotective autophagy in cancer cells through Rap1-mediated disruption of the mTOR-Raptor interaction. Blocking CGRP can inhibit the vicious cycle between cancer cells and nerves and therapeutically benefit nutrient-starvation therapies.

EVI2B Is a Prognostic Biomarker and Is Correlated with Monocyte and Macrophage Infiltration in Osteosarcoma Based on an Integrative Analysis

Summary: Osteosarcoma (OS), the most common malignant bone tumor, lacks updated treatment strategies. This study identified EVI2B as a significantly upregulated gene in OS tumor samples and demonstrated its association with chemokine signaling, cytokine-cytokine receptor interaction, and Human T-cell leukemia virus 1 infection. The prognostic prediction model including EVI2B, DOCK2, and CD33 indicated EVI2B as an independent protective prognostic marker in OS. The study also revealed the close relationship between EVI2B expression, monocyte and macrophage infiltration in OS, providing clear evidence for its prognostic value.

Yin Yang 1-Induced Long Noncoding RNA DUXAP9 Drives the Progression of Oral Squamous Cell Carcinoma by Blocking CDK1-Mediated EZH2 Degradation

Summary: A highly expressed lncRNA DUXAP9, found in oral squamous cell carcinoma (OSCC), is positively associated with poor prognosis. DUXAP9 promotes OSCC cell proliferation, migration, invasion, and tumor growth through the upregulation of N-cadherin, Vimentin, Ki67, PCNA, and EZH2 and downregulation of E-cadherin. Yin Yang 1 (YY1) activates the transcription of DUXAP9 in OSCC. Moreover, DUXAP9 interacts with EZH2 and inhibits its degradation, thereby blocking its translocation from the nucleus to the cytoplasm. DUXAP9 may serve as a promising therapeutic target for OSCC.

Characteristics of esophageal cancer in patients with head and neck squamous cell carcinoma

Summary: This study investigated the clinicopathological features of esophageal cancer in patients with a history of head and neck squamous cell carcinoma (HNSCC). Most cases were male patients aged 50-69 years, with higher proportion of early esophageal cancer and successful surgical treatment in the synchronous carcinoma group compared to the metachronous carcinoma group. Detection of esophageal cancer should be prioritized in HNSCC patients.

Comprehensive clinical, genome and transcriptomic analysis of primary ghost cell odontogenic carcinoma

Summary: This study presents a comprehensive clinical, genomic, and transcriptomic analysis of primary ghost cell odontogenic carcinoma (GCOC). The results provide the first comprehensive molecular atlas for primary GCOC, including previously identified CTNNB1 mutation and novel alterations in MAP3K, EP300, and the 22q11.21 region. The transcriptome analysis reveals significant involvement of cytokine-cytokine receptor interaction and the PI3K-Akt signaling pathway. More GCOC cases should be compared to validate these findings for accurate clinical guidance.

Transoral robotic surgery in oropharyngeal squamous cell carcinoma: A comparative study between da Vinci Single-Port and da Vinci Xi systems

Summary: This study compares the outcomes of transoral resections using the da Vinci SP and da Vinci Xi systems for oropharyngeal squamous cell carcinoma (OPSCC). The results indicate that the da Vinci SP system has advantages in terms of reduced docking and console times, as well as improved rates of wide negative resection margins. However, there were no significant differences in complication rates or postoperative functional outcomes between the two systems.
