Mesenchymal stem cells in obesity: insights for translational applications

Obesity is now a major public health problem worldwide. Lifestyle modification to reduce the characteristic excess body adiposity is important in the treatment of obesity, but effective therapeutic intervention is still needed to control what has become an obesity epidemic. Unfortunately, many anti-obesity drugs have been withdrawn from market due to adverse side effects. Bariatric surgery therefore remains the most effective therapy for severe cases, although such surgery is invasive and researchers continue to seek new control strategies for obesity. Mesenchymal stem cells (MSCs) are a major source of adipocyte generation, and studies have been conducted into the potential roles of MSCs in treating obesity. However, despite significant progress in stem cell research and its potential applications for obesity, adipogenesis is a highly complex process and the molecular mechanisms governing MSC adipogenesis remain ill defined. In particular, successful clinical application of MSCs will require extensive identification and characterization of the transcriptional regulators controlling MSC adipogenesis. Since obesity is associated with the incidence of multiple important comorbidities, an in-depth understanding of the relationship between MSC adipogenesis and the comorbidities of obesity is also necessary to evaluate the potential of effective and safe MSC-based therapies for obesity. In addition, brown adipogenesis is an attractive topic from the viewpoint of therapeutic innovation and future research into MSC-based brown adipogenesis could lead to a novel breakthrough. Ongoing stem cell studies and emerging research fields such as epigenetics are expected to elucidate the complicated mechanisms at play in MSC adipogenesis and develop novel MSC-based therapeutic options for obesity. This review discusses the current understanding of MSCs in adipogenesis and their potential clinical applications for obesity.

Summary: This study aimed to evaluate the impact of stroke on the prognosis of patients with underlying ischemic heart disease (IHD) who underwent percutaneous coronary intervention (PCI) treatment. The study revealed that stroke patients were older and had more comorbidities compared to stroke-free patients. Patients with stroke had a higher incidence of clinical events and a poorer prognosis, highlighting the urgent need for comprehensive prevention of cardiovascular and cerebrovascular diseases.

Summary: This study investigated the significance of left ventricular apical sparing in patients with wild-type transthyretin amyloid cardiomyopathy (ATTRwt-CM). The results showed that approximately half of ATTRwt-CM patients did not exhibit left ventricular apical sparing at diagnosis. The relative apical longitudinal strain index (RapLSI) of ATTRwt-CM significantly changed over time, emphasizing the importance of repeated two-dimensional speckle tracking analysis for suspected ATTR-CM patients.

Summary: This study aimed to identify factors leading to future pacing device implantation (PDI) and the necessity of prophylactic PDI or implantable cardioverter-defibrillator (ICD) implantation in ATTR-CM patients. A retrospective study was conducted on 114 wild-type ATTR-CM and 50 hereditary ATTR-CM patients to compare patient backgrounds with and without future PDI and examine the incidence of PDI in each conduction disturbance. The study found that specific factors were significantly associated with future PDI in both ATTRwt-CM and ATTRv-CM patients.

Summary: This study aimed to evaluate the clinical course and therapeutic effect of tafamidis in patients with ATTR-CM. It found that tafamidis treatment significantly improved outcomes, reducing all-cause mortality and hospitalization due to heart failure. High-sensitivity cardiac troponin T may be a useful biomarker for evaluating the therapeutic effect of tafamidis.

Characterization of Immune Cell Populations of Cutaneous Neurofibromas in Neurofibromatosis 1

Summary: This study investigated the immune microenvironment of cutaneous neurofibromas (cNFs) in patients with neurofibromatosis 1 (NF1). The results showed that cNFs have substantial populations of T cells and macrophages, which may be tumor-specific. T cell populations in cNFs were found to be different from those in the skin, and cNFs exhibited lower expression of proteins related to T cell-mediated immunity compared to the skin.
