Clinical characteristics and long-term prognosis of elderly onset ulcerative colitis

Background and AimThe aim of this study was to investigate the clinical characteristics and prognosis of patients with elderly onset ulcerative colitis (EOUC), a new growing subgroup of UC. MethodsThis study retrospectively analyzed 3060 South Korean UC patients diagnosed between 1977 and 2014. The clinical characteristics and prognosis of EOUC, defined as UC in those aged 60years at diagnosis, were compared with those of non-EOUC (NEOUC). ResultsAmong the 3060 patients, 226 were diagnosed with EOUC (7.4%, median age at diagnosis 65.9years [interquartile range, 62.9-68.7years], 58.4% male). The frequency of EOUC increased from 3.9% in the interval 1977-1999 to 9.7% in the interval 2008-2014 (P<0.001). There were more ex-smokers in the EOUC than in the NEOUC group (44.2% vs 19.9%, P<0.001). In the EOUC group, extensive colitis at diagnosis, and the maximum extent thereof, was less than in the NEOUC group (13.7% vs 22.6%, P=0.002, and 34.5% vs 42.5%, P=0.011, respectively). The 10-year cumulative colectomy rate was significantly higher in the EOUC than in the NEOUC group (12.6% vs 7.7%, P=0.015). UC-related and all-cause mortality were higher in the EOUC than in the NEOUC group (3.5% vs 0.6%, P<0.001, and 12.4% vs 1.8%, P<0.001, respectively). ConclusionElderly onset ulcerative colitis patients are likely to exhibit distinct features both at diagnosis and during follow-up. It is necessary to pay more attention to, and to conduct further studies on, this particular group of patients.

Comparative Cost Analysis Between Endoscopic Resection and Surgery for Submucosal Colorectal Cancer

Summary: This retrospective observational study analyzed the cost of endoscopic resection and surgical resection for submucosal colorectal cancer. The study found that endoscopic resection had a lower total cumulative cost and comparable oncologic outcomes as surgical resection. Therefore, endoscopic resection can be considered a cost-effective option for initial treatment of submucosal colorectal cancer.

Summary: This study investigated the prevalence of sarcopenia at diagnosis and its clinical significance in Korean patients with IBD. The results showed that 56.8% of the population had sarcopenia, with higher prevalence in male patients. Sarcopenia may be associated with an increased risk of perianal surgeries in patients with CD.

Summary: This study evaluated the visualization ability of ICG fluorescence endoscopy in colitis-associated colon cancer and used a deep learning network to predict the development of colorectal cancer. The results showed that ICG could detect carcinoma, adenoma, and dysplasia with high accuracy, and there was a positive correlation between ICG and iNOS. The mechanism of ICG retention in cancer cells was related to intracellular nitric oxide concentration.

Comparison of a novel mini-oral sulfate tablet and the conventional oral sulfate tablet in bowel preparation for colonoscopy: a prospective, randomized, investigator-blinded, multicenter, non-inferior, phase 3 trial

Summary: This study compared the efficacy, tolerability, and safety of mini-OST and conventional OST. The results showed that there were no significant differences in preparation efficacy, satisfaction, and tolerability between the two groups. The first bowel movement occurred earlier in the mini-OST group. The occurrence of adverse events was comparable between the two groups. In conclusion, mini-OST showed similar efficacy, tolerability, and safety compared to conventional OST.

Influence of endoscopists & apos; expertise level on clinical outcomes after bridge-to-surgery stenting in obstructive colorectal cancer

Summary: This study aimed to investigate the effect of stenting-related factors, including endoscopists' expertise, on clinical outcomes after bridge-to-surgery (BTS) stenting for obstructive colorectal cancer (CRC). The results showed that the expertise of endoscopists was associated with better short-term outcomes, including high stenting success rate and low rate of stent-related complications after BTS stenting. The interval between stenting and surgery also played a role in postoperative complications.

Summary: This study examined the features of adult-onset megacolon with focal hypoganglionosis. Through radiologic, endoscopic, and histopathologic analysis, it was found that all patients exhibited focal stenotic regions and proximal bowel dilatation. Surgical treatment showed significant improvement in symptoms for these patients.

Clinicopathologic Impact of Peptide Hormonal Expression in Rectal Neuroendocrine Tumors

Summary: This study aimed to evaluate the expression of multiple peptide hormones in rectal neuroendocrine tumors (NETs) and their clinical and prognostic implications. The results showed that several peptide hormones were expressed in rectal NETs, and tumors expressing serotonin or somatostatin were associated with lymphovascular invasion, chromogranin A expression, and shorter disease-free survival. The peptide hormonal expression status can classify rectal NETs into L-cell, enterochromaffin-cell, D-cell, null-expression, or mixed-expression type.

HZ in IC/AID patients leads to higher medical costs.
