Poorly water soluble (PWS) drugs lack from good oral absorption, which limits their oral administration, the most common drug delivery route. The design of appropriate innovative formulations for PWS drugs will allow for adequate bioavailability, reduction of the required dose and oral delivery of drugs that would otherwise need the parenteral route. Among the formulations under study, Nanostructured Lipid Carriers (NLCs), made of biocompatible mixtures of solid and liquid lipids, represent a good choice. NLCs enhance the apparent solubility and dissolution of PWS drugs in the gastrointestinal tract, and/or modulate the drug permeability and fate across the intestinal barrier, opening new perspectives in the field of innovative medicines for oral administration of PWS molecules. Recently, NLCs have also emerged as a promising anti-inflammatory drug delivery system for inflammatory bowel diseases. The present paper is focused on the state of the art of NLCs for oral delivery of PWS drugs. The design, the main preparation techniques and the mechanisms through the drug oral delivery is improved by NLCs are presented, highlighting the influence of the most relevant technological and formulation factors on the properties of the NLCs, which condition their efficacy as oral delivery formulations. Summary: The ability of mesenchymal stromal cells (MSCs) to release immunomodulatory factors makes them promising candidates for treating inflammatory bowel diseases (IBD). However, nude MSC administration has limitations such as loss of immunomodulatory phenotype and low persistence. In this study, a licensing hydrogel system was designed to overcome these limitations and achieve continuous delivery of bioactive factors. The results showed that encapsulated MSCs in the hydrogel limited the progression of IBD in mice.

Exploiting the biological effect exerted by lipid nanocapsules in non-alcoholic fatty liver disease

Summary: Non-alcoholic fatty liver disease (NAFLD) affects a significant portion of the adult population worldwide and no approved pharmacologic therapy is currently available. A novel drug delivery system called lipid nanocapsules (LNCs) has shown promise in treating NAFLD by inducing the secretion of glucagon-like peptide 1 (GLP-1). This study aimed to compare the efficacy of the LNCs system with subcutaneous injection of a GLP-1 analog in two mouse models of NAFLD. The results demonstrated that chronic administration of the LNCs system improved glucose homeostasis and insulin resistance, and prevented disease progression to a greater extent than subcutaneous injection in both models.

Pharmacokinetic/Pharmacodynamic Analysis of Oral Calcium Fosfomycin: Are Urine Levels Sufficient to Ensure Efficacy for Urinary Tract Infections?

Summary: This study evaluated the pharmacokinetics of calcium fosfomycin in urine after oral administration and assessed its effectiveness for the treatment of urinary tract infections (UTIs) caused by Escherichia coli. The results showed that oral calcium fosfomycin at a dose of 1000 mg every 8 hours provided sufficient urine concentrations to ensure efficacy in UTI treatment in women.

The findings suggest that Dysosmobacter welbionis abundance is influenced by metformin treatment and is associated with liver health and glucose metabolism in individuals with obesity and diabetes. This highlights the potential role of Dysosmobacter welbionis in metabolic health.

Ceftaroline and Avibactam Removal by Continuous Renal Replacement Therapies: An in vitro Study

The results showed that all three modalities effectively eliminated ceftaroline and avibactam, suggesting that dose adjustment may be necessary in CRRT-treated patients.

Novel 3D integrated microfluidic device: Design, construction, and application to the preparation of liposomes for vaccine delivery
