Heart failure (HF) is a major public health priority due to its epidemiological transition and the world's aging population. HF is typified by continuous loss of contractile function with reduced, normal, or preserved ejection fraction, elevated vascular resistance, fluid and autonomic imbalance, and ventricular dilatation. Despite considerable advances in the treatment of HF over the past few decades, mortality remains substantial. Pharmacological treatments including beta-blockers, angiotensin-converting enzyme inhibitors, angiotensin receptor blockers, and aldosterone antagonists have been proven to prolong the survival of patients with HF. However, there are still instances where patients remain symptomatic, despite optimal use of existing therapeutic agents. This understanding that patients with chronic HF progress into advanced stages despite receiving optimal treatment has increased the quest for alternatives, exploring the roles of additional pathways that contribute to the development and progression of HF. Several pharmacological targets associated with pathogenesis of HF have been identified and novel therapies have emerged. In this work, we review recent evidence from proposed mechanisms to the outcomes of experimental and clinical studies of the novel pharmacological agents that have emerged for the treatment of HF.

Examining Risk Factors Accelerating Time-to-Chronic Obstructive Pulmonary Disease (COPD) Diagnosis among Asthma Patients

Summary: This study found that male sex, older age (>40 years), smoking history, frequent asthma exacerbations, frequent emergency admissions, longer hospital stays, increased burden of comorbidities, obese male sex, SABA overuse, moderate and severe asthma all accelerated the time-to-COPD diagnosis among asthma patients. On the other hand, high medication adherence (MA >=0.80) was significantly associated with a delayed COPD diagnosis by 83%. This emphasizes the importance of medication adherence in delaying the onset of COPD among asthma patients.

Determining the optimal threshold for medication adherence in adult asthma patients: an analysis of British Columbia administrative health database in Canada

Summary: This study examined the relationship between medication adherence levels and asthma exacerbations in physician-diagnosed asthma patients. The results showed that maintaining a medication adherence threshold level greater than 0.80 can significantly reduce the likelihood of experiencing asthma exacerbations.

Thyrotoxic periodic paralysis together with thyrotoxic heart disease in a Ghanaian man: case report and literature review

Summary: Thyrotoxic periodic paralysis (TPP) is a rare symmetrical paralysis that primarily affects proximal muscles and occurs in hyperthyroidism with hypokalemia. It is more common in East Asian males and extremely rare in blacks. There is limited data on TPP in Africa, and no cases have been reported in Ghana. We present a case of a middle-aged Ghanaian man who experienced three episodes of paralysis in all four limbs, with the second and third episodes requiring hospitalization. He did not show signs of hyperthyroidism during the first hospital visit but developed clinical and biochemical evidence of hyperthyroidism during the second visit, with serum potassium levels of 1.9 mmol/L, leading to a diagnosis of TPP. His paralysis resolved after correcting the hypokalemia. It is important to consider less common differentials, such as TPP, in patients presenting with paralysis to ensure early diagnosis and treatment and prevent complications.
