Summary: Breast cancer survivors often experience atrophic vaginitis due to persistent estrogen suppression treatments, but the use of local estrogen therapy is limited in these women due to treatment controversies. Non-hormonal treatments are the first-line therapy for vulvovaginal atrophy in this population, with concerns remaining about safety of alternative options like local estrogen, laser therapy, and local androgens.

Summary: The study aimed to assess whether CO2 laser treatment is more effective than sham application in relieving symptoms in women with genitourinary syndrome of menopause (GSM). The results showed that the treatment response after 12 weeks of laser application was comparable to that of the sham applications, and no serious adverse events were reported.

Summary: The postmenopausal state affects a large percentage of women's lives and many experience GSM symptoms such as vaginal dryness, itching, inflammations, lack of elasticity, or dyspareunia. This study found that fractional CO2 laser treatment effectively improved these symptoms and increased the quality of life for women. The treatment restored the correct structure and composition of the vaginal epithelium, leading to positive results in both objective and subjective evaluations.

Summary: This study aimed to assess the effectiveness of fractional CO2 laser therapy for GSM through a randomized, double-blind, sham-controlled trial. The results showed no significant difference in improvement proportion between the treatment and control groups, but both groups showed significant within-group improvements in VHI and VAS. Further research is needed to determine the therapeutic effect of this novel modality.

Summary: Menopause is a physiological phenomenon secondary to decreased ovarian follicular reserve, leading to vaginal symptoms. Topical hormone therapy is commonly used for postmenopausal vaginal symptoms, but not suitable for women with a history of breast cancer, oestrogen-sensitive tumors, and thromboembolism. Physical methods like laser and RF have been used for vaginal mucosa treatment.

Summary: The use of vaginal laser therapy for GSM has increased globally despite lacking endorsement from international societies. Studies suggest that it may improve VAS, FSFI, and VHI scores in the short term, but safety outcomes are underreported. Well-designed clinical trials with sham-laser control groups are necessary to provide better evidence on efficacy.

Summary: This narrative review focuses on the impact of cancer treatments on sexual function in female cancer survivors, and discusses the evaluation and management of sexual concerns in this population. Female cancer survivors commonly experience sexual dysfunction, however, it is often underdiagnosed and undertreated. The complex nature of sexual dysfunction after cancer calls for a multidisciplinary approach to care. Validating and addressing sexual health concerns, providing education and appropriate therapies, and timely referral to sexual health experts are crucial in optimizing care for female cancer survivors. Clinicians should be aware of various potential treatment modalities that may contribute to sexual health concerns and individualize management based on contributing factors.

Summary: This study evaluated the effects of fractional CO2 laser compared with sham treatment on postmenopausal vaginal epithelium. The results showed no significant difference in the histological effects on vaginal tissue between the laser and sham groups. Therefore, fractional CO2 laser should not be recommended for the treatment of postmenopausal vaginal symptoms.

Summary: The study aimed to describe the magnetic resonance characteristics of radial scars/complex sclerosing lesions (RS/CSL) of the breast using the current BI-BADS lexicon, and investigate the value of diffusion weighted imaging for predicting malignancy. The majority of RS/CSL showed enhancement at MR, predominantly presenting as non-mass, focal, heterogeneous internal enhancement. All cases associated with cancer showed non-mass enhancement.

Summary: This study reports the first documented case of primary membranous nephropathy (PMN) in a deceased kidney donor, transplanted to two different recipients, with their clinical and immunological evolution monitored through serial biopsies. The findings suggest that PMN changes can reverse in grafts, indicating the possibility of transplanting from asymptomatic deceased donors with PMN to patients.

Summary: This study analyzed the clinicopathological and genetic alterations in 15 cases of IVLBCL using next-generation sequencing. The most frequently mutated genes were PIM1, MYD88, and CD79B, and PD-L1 expression was observed in some cases. The results emphasize the importance of mutations in B-cell receptor/nuclear factor-kappa B signaling and immune escape pathways in IVLBCL, and identify novel driver alterations. Extravascular dissemination may also be considered as part of the spectrum of IVLBCL.

Summary: In early stage breast cancer patients without lymph node metastasis, the presence of positive sentinel lymph nodes after primary systemic treatment directs axillary lymph node dissection. This study aimed to identify factors associated with non-sentinel lymph node involvement and develop a predictive score. The results showed that non-sentinel lymph node involvement was infrequent (22.2%) and associated with progesterone receptor levels and macrometastatic sentinel lymph nodes. The developed predictive score accurately predicted the absence of non-sentinel lymph node involvement, suggesting that unnecessary axillary lymph node dissection could be safely avoided in most patients.
