Quantitative and Spatial Analysis of CD8+/PD-1 Tumor-Infiltrating Lymphocytes as a Predictive Biomarker for Clinical Response of Melanoma In-Transit Metastases to Topical Immunotherapy

Sophia Haywood, Jennifer Garioch, Arjun Ramaiya, Marc Moncrieff

Summary: The study found that BRAF mutation status, CD8 and PD-1 expression on tumor-infiltrating lymphocytes (TILs), and tumor PD-L1 expression were not significantly associated with response to DPCP in treating ITMs. Patients with BRAF wild-type tumors were more likely to experience a complete response to DPCP. Peritumoral TILs and PD-1 expressions may predict a better response to DPCP.

Extracapsular Spread in Melanoma Lymphadenopathy: Prognostic Implications, Classification, and Management

Michelle Lo, Alyss Robinson, Ryckie Wade, Howard Peach, Donald Dewar, Martin Heaton, Marc Moncrieff

Summary: ECS in melanoma patients is a significant prognostic indicator affecting disease burden and survival. The presence of ECS can lead to changes in nodal burden and disease severity in patients.

Effectiveness of SPECT/CT Imaging for Sentinel Node Biopsy Staging of Primary Cutaneous Melanoma and Patient Outcomes

Marc Moncrieff, Sarah Pywell, Andrew Snelling, Matthew Gray, David Newman, Clare Beadsmoore, Davina Pawaroo, Martin Heaton

Summary: This study confirms the increased accuracy of SPECT/CT for identifying SLN metastases, which appears to have a significant therapeutic benefit, although there is an increased risk of cancellation of the SNB procedure on the day of surgery.

Evaluation of the Indications for Sentinel Node Biopsy in Early-Stage Melanoma with the Advent of Adjuvant Systemic Therapy: An International, Multicenter Study

Marc D. Moncrieff, Serigne N. Lo, Richard A. Scolyer, Martin J. Heaton, Jenny P. Nobes, Andrew P. Snelling, Michael J. Carr, Carolyn Nessim, Ryckie Wade, A. Howard Peach, Rumi Kisyova, Jennifer Mason, Ewan D. Wilson, Grant Nolan, Rowan Pritchard Jones, Vernon K. Sondak, John F. Thompson, Jonathan S. Zager

Summary: Patients with early-stage melanoma have a low SN-positivity rate, with most patients being reclassified as AJCC stage IIIA after SNB. Adjuvant systemic therapy is not routinely recommended for most patients with AJCC stage IIIA melanoma. The indications for SNB in some patients with early-stage melanoma need to be reevaluated.

Follow-up Schedule for Patients With Sentinel Node-negative Cutaneous Melanoma (The MELFO Study) An International Phase III Randomized Clinical Trial

Marc D. Moncrieff, Esther Bastiaannet, Beverly Underwood, Anne Brecht Francken, Jennifer Garioch, Samantha Damude, Martin Heaton, Eric A. Deckers, Nakul Patel, Josette E. Hoekstra-Weebers, Harald J. Hoekstra

Summary: The MELFO study compared a reduced-intensity follow-up schedule with the current national guidelines for sentinel node-negative melanoma patients. The results showed no significant differences in patient-reported outcomes and survival outcomes between the two schedules. Patients had a high acceptance of the reduced-intensity schedule.

Summary: This study aimed to identify factors predicting SLNB positivity in patients with T1 melanoma. The results showed that Breslow thickness and mitotic rate were independent predictors of SLNB positivity in T1 melanoma patients.

Summary: The study proposes a classification system based on tumor burden and sentinel node ECS status, which identifies low-risk patients in the AJCC IIIB and IIIC subgroups who may be candidates for observation. This research has important clinical implications for the treatment decision for melanoma patients.
