The effect of mood-stabilizing drugs on cytokine levels in bipolar disorder: A systematic review

Objectives: Cytokine level alterations suggest a role for the immune system in the pathophysiology of bipolar disorder (BD). Pharmacotherapy is an important confounding factor in clinical research on cytokine levels. In this systematic review we collate the evidence on blood cytokine levels in medication free BD and the effects of single mood-stabilizing drugs on these levels. Methods: A systematic review was conducted according to the PRISMA statement. We searched the Pubmed and Embase databases for clinical studies reporting either on cytokine levels in medication-free BD or on the effects of single mood-stabilizing drugs on cytokine levels in BD. Results: Of the 564 articles screened, 17 were included. Fourteen articles report on medication-free patients with BD and indicate state-related cytokine alterations. Six articles discuss the effect of lithium. Whereas no data on short-term effects of lithium were found, >= 2 months lithium use in euthymic populations is associated with normal cytokine levels. Two studies report no effect of valproate and no studies were found on carbamazepine, lamotrigine or antipsychotics. Limitations: The available studies are characterized by a broad methodological heterogeneity and limited replication between studies. Conclusions: This systematic review suggests the presence of state-related cytokine level alterations in medication-free BD with most evidence pointing to a proinflammatory cytokine response in mania. Euthymia and long-term lithium use are associated with normal cytokine levels. To improve our understanding of the impact of mood-stabilizing drugs on cytokine levels, longitudinal studies with medication-free baseline, randomized controlled single-drug treatment protocols and close mood state monitoring are needed. The Role of Kynurenines in Cognitive Dysfunction in Bipolar Disorder

Summary: In patients with bipolar disorder, lower levels of the KYN metabolite KYNA were associated with worse cognitive functioning, particularly in processing speed, only in the bipolar depression group. The manic group exhibited deficits in all cognitive domains, while throughout the follow-up period, both patient groups had significantly lower KYNA levels compared to controls. Further large-scale longitudinal studies are needed to confirm the role of KYN metabolites in cognitive impairment in BD patients and potential therapeutic implications.

Impaired Sensorimotor Adaption in Schizophrenia in Comparison to Age-Matched and Elderly Controls

Summary: The cognitive dysmetria hypothesis suggests disrupted communication between the cerebellum and cerebral cortex in schizophrenia, leading to sensorimotor and cognitive symptoms. This study found that sensorimotor adaptation is reduced in schizophrenia patients compared to age-matched and elderly controls. Patients with schizophrenia showed impairments in both explicit and implicit components of sensorimotor adaptation learning.

Summary: This study presents the clinical management and analysis of four new cases of Mitchell-Riley syndrome (MRS), a rare genetic disorder caused by mutations in the RFX6 transcription factor gene. The study highlights the impact of these mutations on RFX6 function and discusses the multidisciplinary approach to treatment that has improved the prognosis for these patients. The findings suggest that further understanding of RFX6 function could lead to new therapeutic interventions for MRS.

The combination of CGM metrics and clinical parameters revealed key clinical milestones of glucose homeostasis and remission status during the first year of type 1 diabetes in the pediatric cohort.

Summary: This study aimed to investigate the SARS-CoV-2 seroprevalence, antibody titers, and cytokine concentrations in newly admitted or hospitalized psychiatric inpatients without a known history of COVID-19. The results showed that psychiatric patients had a significantly higher seroprevalence and antibody titers compared to hospital caregivers. The study highlights the importance of monitoring and mitigating exposure to infectious agents among psychiatric patients.

The relationship between psychological pain and suicidality in patients with major depressive disorder: A meta-analysis

Summary: This study systematically reviewed the association between psychological pain and suicidality in patients with major depressive disorder (MDD). The results showed that psychological pain was a risk factor for suicidality in MDD patients, especially for those of advancing age. The role of family accommodation in child posttraumatic stress symptoms and functional impairment in the context of cognitive behavioral therapy

Summary: This study examined the relationship among family accommodation (FA), posttraumatic stress symptoms (PTSS), and functional impairment. The results showed that FA was significantly associated with PTSS and functional impairment. Baseline FA partially mediated the relationship between baseline PTSS and functional impairment. Changes in FA from pre- to post-treatment were associated with relevant outcome variables at post-treatment and 12-month follow-up.
