Early outcomes in re-do operation after acute type A aortic dissection: results from the multicenter REAAD database

This study provides early results of re-operations after the prior surgical treatment of acute type A aortic dissection (AAD) and identifies risk factors for mortality. Between May 2003 and January 2014, 117 aortic re-operations after an initial operation for AAD (a mean time from the first procedure was 3.98 years, with a range of 0.1-20.87 years) were performed in 110 patients (a mean age of 59.8 +/- 12.6 years) in seven European institutions. The reoperation was indicated due to a proximal aortic pathology in ninety cases: twenty aortic root aneurysms, seventeen root re-dissections, twenty-seven aortic valve insufficiencies and twenty-six proximal anastomotic pseudoaneurysms. In fifty-eight cases, repetitive surgical treatment was subscripted because of distal aortic pathology: eighteen arch re-dissections, fifteen arch dilation and twenty-five anastomotic pseudoaneurysms. Surgical procedures comprised a total of seventy-one isolated proximals, thirtyone isolated distals and fifteen combined interventions. In-hospital mortality was 19.6 % (twenty-three patients); 11.1 % in patients with elective/urgent indication and 66.6 % in emergency cases. Mortality rates for isolated proximal, distal and combined operations regardless of the emergency setting were 14.1 % (10 pts.), 25.8 % (8 pts.) and 33.3 % (5 pts.), respectively. The causes of death were cardiac in eight, neurological in three, MOF in five, sepsis in two, bleeding in three and lung failure in two patients. A multivariate logistic regression analysis revealed that risk factors for mortality included previous distal procedure (p = 0.04), new distal procedure (p = 0.018) and emergency operation (p < 0.001). New proximal procedures were not found to be risk factors for early mortality (p = 0.15). This multicenter experience shows that the outcome of REAAD is highly dependent on the localization and extension of aortic pathology and the need for emergency treatment. Surgery in an emergency setting and distal re-do operations after previous AAD remain a surgical challenge, while proximal aortic re-operations show a lower mortality rate. Foresighted decision-making is needed in cases of AAD repair, as the results are essential preconditions for further surgical interventions.

Summary: Our data suggest that in patients with an increased risk of bleeding, the use of cardiopulmonary bypass is associated with higher morbidity. These patients may benefit from off-pump surgery if complete revascularization can be ensured.

Summary: The objective of this study was to investigate the need for postoperative permanent pacemaker implantation (PPI) following sutureless and rapid-deployment aortic valve replacement (SuRD-AVR). The study retrospectively analyzed a large multicenter international registry (SURD-IR) from 2008 to 2019. The results showed a significant decrease in the PPI rate over time in the Su cohort, while the RD cohort showed no significant differences in PPI rate between the two time periods.

Ten years' experience with the sutureless aortic valve replacement: incidence and predictors for survival and valve durability at follow-up

Summary: Sutureless aortic valve replacement (Su-AVR) is a viable option for patients with severe aortic valve stenosis, and the long-term outcomes and durability of the prosthesis were evaluated in this study. Age, EuroSCORE II, baseline dialysis, and postoperative acute kidney injury were found to be significantly associated with worse survival. Structural valve deterioration occurred in 4.2% of patients, with 19 individuals undergoing reintervention.

Microbiologic diagnostics and pathogen spectrum in infective endocarditis of surgically treated patients: a five-year, retrospective, monocentric study

Summary: This study evaluated the diagnostic value of microbiological methods for detecting the causative microorganism of infective endocarditis (IE) and analyzed the spectrum of pathogens. The results showed that blood culture, microbiological culture, and 16S rDNA PCR play important roles in pathogen detection in IE. Blood culture is the most commonly used method, while 16S rDNA PCR can increase the diagnostic yield of blood culture-negative cases.

Baseline CT-Based Risk Factors for Atrioventricular Block after Surgical AVR

Summary: This study aimed to evaluate the impact of membranous interventricular septum (MIS) length and calcifications of the native aortic valve (AV) on postoperative atrioventricular block III (AVB/AVB III) and permanent pacemaker implantation in surgical aortic valve replacement (SAVR). Preoperative multidetector computed tomography (MDCT) scans and procedural outcomes were retrospectively analyzed. The results showed that patients with AVB had more severe calcifications in specific areas and a shorter MIS length. Therefore, we recommend including MDCT in preoperative diagnostic testing to further stratify the risk for patients undergoing surgical AVR.

Sutureless and Rapid Deployment vs. Transcatheter Valves for Aortic Stenosis in Low-Risk Patients: Mid-Term Results

Summary: This study compared the outcomes of Sutureless and Rapid Deployment Valves (SuRD-AVR) and TAVI in low surgical risk patients with isolated aortic stenosis. The results showed that TAVI patients had lower 5-year overall survival and survival freedom from major adverse cardiovascular and cerebrovascular events (MACCEs) compared to SuRD-AVR. TAVI was also associated with a higher incidence of postoperative permanent pacemaker implantation (PPI) and paravalvular leak grade >= 2 (PVL).

Summary: This study compared the mid- to long-term outcomes of patients with isolated aortic stenosis and an intermediate-risk profile who underwent aortic valve replacement using a sutureless valve versus transcatheter aortic valve implantation. The study found that sutureless aortic valve replacement had significantly lower rates of all-cause mortality, cardiac-related death, permanent pacemaker implantation, and paravalvular leak compared to transcatheter aortic valve implantation.

Platelet Reduction after Aortic Bioprosthesis Implantation: Results from the PORTRAIT Study

Summary: This study investigated the occurrence and clinical impact of platelet count reduction in patients receiving different types of aortic bioprosthesis valve implantation. The results showed that platelet count at discharge was lower than the baseline values in all groups. Patients with rapid deployment and stentless bioprostheses experienced a greater reduction in platelet count, which was associated with the size of the bioprosthesis. Platelet count variation was not directly proportional to the size of the stented bioprosthesis and was not affected by cardiopulmonary bypass or cross-clamp times. Platelet count reduction was associated with ischemic strokes.
