Summary: In this study, the impact of various risk factors on the pre-failure behavior of ascending aortic aneurysm (AsAA) was compared using biomechanical data obtained from equi-biaxial tensile tests. The results showed that age, smoking, diabetes, aortic stenosis, coronary artery disease, and aortic diameter significantly influenced aortic stiffness. Additionally, an increase in aortic diameter was associated with an increase in aortic wall thickness.

Summary: There was no difference in long-term survival between mechanical and biological prostheses for both age groups. Mechanical prostheses had a higher risk of bleeding in the 60-69-year group whereas biological valves had higher overall reintervention probability without an impact on long-term survival. It may be safe to use biological valves based on lifestyle choices for patients in the 50-69-year age group.

Summary: The objective of this study was to investigate the need for postoperative permanent pacemaker implantation (PPI) following sutureless and rapid-deployment aortic valve replacement (SuRD-AVR). The study retrospectively analyzed a large multicenter international registry (SURD-IR) from 2008 to 2019. The results showed a significant decrease in the PPI rate over time in the Su cohort, while the RD cohort showed no significant differences in PPI rate between the two time periods.

Summary: Sutureless aortic valve replacement (Su-AVR) is a viable option for patients with severe aortic valve stenosis, and the long-term outcomes and durability of the prosthesis were evaluated in this study. Age, EuroSCORE II, baseline dialysis, and postoperative acute kidney injury were found to be significantly associated with worse survival. Structural valve deterioration occurred in 4.2% of patients, with 19 individuals undergoing reintervention.

Summary: This study evaluated the diagnostic value of microbiological methods for detecting the causative microorganism of infective endocarditis (IE) and analyzed the spectrum of pathogens. The results showed that blood culture, microbiological culture, and 16S rDNA PCR play important roles in pathogen detection in IE. Blood culture is the most commonly used method, while 16S rDNA PCR can increase the diagnostic yield of blood culture-negative cases.

Summary: This study aimed to evaluate the impact of membranous interventricular septum (MIS) length and calcifications of the native aortic valve (AV) on postoperative atrioventricular block III (AVB/AVB III) and permanent pacemaker implantation in surgical aortic valve replacement (SAVR). Preoperative multidetector computed tomography (MDCT) scans and procedural outcomes were retrospectively analyzed. The results showed that patients with AVB had more severe calcifications in specific areas and a shorter MIS length. Therefore, we recommend including MDCT in preoperative diagnostic testing to further stratify the risk for patients undergoing surgical AVR.

Summary: This study compared the outcomes of Sutureless and Rapid Deployment Valves (SuRD-AVR) and TAVI in low surgical risk patients with isolated aortic stenosis. The results showed that TAVI patients had lower 5-year overall survival and survival freedom from major adverse cardiovascular and cerebrovascular events (MACCEs) compared to SuRD-AVR. TAVI was also associated with a higher incidence of postoperative permanent pacemaker implantation (PPI) and paravalvular leak grade >= 2 (PVL).

Summary: This study investigated the occurrence and clinical impact of platelet count reduction in patients receiving different types of aortic bioprosthesis valve implantation. The results showed that platelet count at discharge was lower than the baseline values in all groups. Patients with rapid deployment and stentless bioprostheses experienced a greater reduction in platelet count, which was associated with the size of the bioprosthesis. Platelet count variation was not directly proportional to the size of the stented bioprosthesis and was not affected by cardiopulmonary bypass or cross-clamp times. Platelet count reduction was associated with ischemic strokes.
