Summary: Interstitial lung diseases are a complex group of diseases with unclear causes and mechanisms of progression. Recent research suggests that the lung microbiome may play a role in the development and progression of these diseases. However, the specific mechanisms are not yet understood. This review aims to discuss the role of altered lung microbiome in various interstitial lung diseases. Understanding the interaction between the host and microbiome in the lungs is a priority for research on these diseases.

Pathologic comparison of conventional video-assisted thoracic surgical (VATS) biopsy versus non-intubated/awake biopsy in fibrosing interstitial lung diseases

Summary: Non-intubated/awake surgical lung biopsy was compared to conventional VATS biopsy in patients with F-ILD. The two procedures showed similar identification of histopathological features, but awake biopsy had significantly higher patient age and diagnostic confidence. Both procedures had a 100% diagnostic yield and similar distribution of ILD diagnoses. Awake biopsy has the potential to become the standard surgical procedure in F-ILD patients, but larger studies are needed for validation.

Summary: This study aimed to create a large database for patients with idiopathic pulmonary fibrosis (IPF) and lung cancer in Europe. The results showed that 10.2% of IPF patients also had lung cancer, and IPF patients with lung cancer had a higher risk of all-cause mortality. Lung cancer patients who received antifibrotic treatment had lower all-cause mortality, and surgically treated IPF patients with operable lung cancer had lower all-cause mortality.

Summary: COVID-19 and sarcoidosis are two diseases with similar clinical manifestations, and they may interact with each other in terms of immune system dysregulation, inflammatory response, and disease course. Sarcoidosis patients are at a higher risk of developing severe COVID-19 pneumonia due to underlying lung disease and immunosuppressive treatment. This review discusses the interactions between these two diseases in terms of clinical manifestations, treatment, and pathogenesis.

Summary: Sarcoidosis is a disease where granulomas form in various organs, particularly the lungs. The course of the disease can be unpredictable, with some patients experiencing spontaneous remission while others develop chronic lung disease. The mechanism leading to fibrosis in advanced pulmonary sarcoidosis is still unknown, but recent studies have shed light on the genetic and immune factors involved. This review aims to summarize the clinical-prognostic characteristics and molecular pathways associated with the development of advanced pulmonary sarcoidosis.

Summary: Millions of workers, especially in developing countries, are exposed to substances that can cause occupational interstitial lung diseases (ILDs), but the burden of the disease could be underestimated due to under-recognition and under-reporting. The diagnosis of occupational ILD requires suspicion and a thorough occupational history, as it can be difficult to distinguish between occupational and non-occupational ILDs. Considering the potential occupational cause of ILD is important, as removing workplace exposure can lead to significant improvement.

Summary: Idiopathic pulmonary fibrosis (IPF) is a common and severe disease that primarily affects middle-aged and elderly males. In addition to aging, occupational and environmental exposures, as well as lifestyle factors such as smoking and diet, have been associated with an increased risk of IPF. Trigger factors may also lead to acute exacerbations and poor prognosis.
