Antimicrobial-coated sutures to decrease surgical site infections: a systematic review and meta-analysis

To investigate the effectiveness of antimicrobial-coated sutures compared with non-coated sutures in reducing surgical site infection (SSI) and develop recommendations for World Health Organization (WHO) SSI prevention guidelines. We searched Medline, Embase, Cinahl, Cochrane Central Register of Controlled Trials, and WHO Global Health from 1990-16/02/2015 with language restricted to English, Spanish, and French. Meta-analysis was performed with a random-effects model. Meta-regression analysis assessed whether the effect of antimicrobial coating changed according to the type of suture and surgery. Subgroup analyses were based on types of sutures. Quality of the retrieved evidence was assessed using the Grading of Recommendations Assessment, Development, and Evaluation. Thirteen randomized controlled trials (RCTs) and five observational studies (OBSs) met the inclusion criteria. Antimicrobial sutures significantly reduced SSI risk (for RCTs: OR 0.72, 95 % CI 0.59-0.88, p = 0.001, I-2 = 14 %; for OBSs: OR 0.58, 95 % CI 0.40-0.83, p = 0.003, I-2 = 22 %). Only Vicryl Plus vs Vicryl revealed consistent results in favor of antimicrobial sutures (for seven RCTs: OR 0.62, 95 % CI 0.44-0.88, p = 0.007, I-2 = 3 %; for four OBSs: OR 0.58, 95 % CI 0.37-0.92, p = 0.02, I-2 = 41 %). The effect of antimicrobial coating was similar between different suture, wound, and procedure types. Quality of RCT evidence was moderate, and OBS evidence was very low quality. Triclosan-coated sutures may reduce SSI risk. However, the available evidence is of moderate/low quality, and many studies had conflicts of interest.

Marine-inspired molecular mimicry generates a drug-free, but immunogenic hydrogel adhesive protecting surgical anastomosis

Summary: This study demonstrates a novel strategy for generating an adhesive by mimicking marine animals and shows that the Da-g-Xan adhesive can significantly improve protective effects on colonic anastomosis in a rat model. The structure of the Da-g-Xan hydrogel is maintained by dynamic intermolecular hydrogen bonds to allow quick release of dopamine-conjugated xanthan gum, which can regulate inflammatory status and induce type 2 macrophage polarization. The adhesive creates an appropriate microenvironment for tissue healing by promoting chemokine and growth factor secretion from M2 macrophages, enhancing fibroblast migration and proliferation, collagen synthesis, and epithelial vascularization.

Intestinal mucosa-derived DNA methylation signatures in the penetrating intestinal mucosal lesions of Crohn's disease

Summary: This study comprehensively evaluated the genome-wide DNA methylation changes in intestinal mucosa tissue of adult patients with Crohn's disease. Abnormal DNA methylation sites were identified and correlated with genes related to apoptosis, IL-8 production, and inflammatory pathways. The degree of abnormal methylation of MUC1 was negatively correlated with the disease activity of Crohn's disease, suggesting a potential role in disease progression.

Microfluidic encapsulated manganese organic frameworks as enzyme mimetics for inflammatory bowel disease treatment

Summary: Metal organic frameworks (MOFs) are proposed as attractive materials for their adjustable structures and physicochemical properties, with a focus on developing functional materials with catalytic capabilities for biomedical values. Biocompatible manganese metal organic framework (Mn-MOF)-based catalase mimetics with microfluidic microcapsule encapsulation were presented for intravital inflammatory bowel disease (IBD) treatment. These Mn-MOF-encapsulated microcapsules showed high performance in treating spontaneous IBD in interleukin-10-deficient mice by relieving oxidative stress and reducing inflammation.

Circulating mitochondrial DNA-triggered autophagy dysfunction via STING underlies sepsis-related acute lung injury

Summary: The STING pathway and its induction of autophagy play a crucial role in immune defense response against pathogenic DNA, but excessive activation of STING can exacerbate organ damage. Increased circulating mtDNA levels and STING activation in sALI patients contribute to lung injury by triggering an inflammatory response and disrupting autophagy. STING activation interferes with lysosomal acidification in an IFN-dependent manner, worsening sepsis.

The interaction between STING and NCOA4 exacerbates lethal sepsis by orchestrating ferroptosis and inflammatory responses in macrophages

The interaction between STING and NCOA4 is found to be critical in this process. The selective 20-HETE synthase inhibitor, HET0016, is shown to reduce STING-induced ferroptosis and mortality in septic mice. These findings provide insights into sepsis and potential therapeutic targets.
