Metformin and health outcomes: An umbrella review of systematic reviews with meta-analyses

The study found that observational evidence on metformin is largely unreliable, while randomized evidence shows benefits for preventing diabetes and in some gynaecological and obstetrical settings. However, almost all meta-analyses are of low or critically low quality according to AMSTAR 2 criteria.
Background The objective was to capture the breadth of outcomes that have been associated with metformin use and to systematically assess the quality, strength and credibility of these associations using the umbrella review methodology. Methods Four major databases were searched until 31 May 2020. Meta-analyses of observational studies and meta-analyses of randomized controlled trials (RCTs) (including active and placebo control arms) were included. Results From 175 eligible publications, we identified 427 different meta-analyses, including 167 meta-analyses of observational studies, 147 meta-analyses of RCTs for metformin vs placebo/no treatment and 113 meta-analyses of RCTs for metformin vs active medications. There was no association classified as convincing or highly suggestive from meta-analyses of observational studies, but some suggestive/weak associations of metformin use with a lower mortality risk of CVD and cancer. In meta-analyses of RCTs, metformin was associated with a lower incidence of diabetes in people with prediabetes or no diabetes at baseline; lower ovarian hyperstimulation syndrome incidence (in women in controlled ovarian stimulation); higher success for clinical pregnancy rate in poly-cystic ovary syndrome (PCOS); and significant reduction in body mass index in people with type 1 diabetes mellitus, in women who have obesity/overweight with PCOS and in obese/overweight women. Of 175 publications, 166 scored as low or critically low quality per AMSTAR 2 criteria. Conclusions Observational evidence on metformin seems largely unreliable. Randomized evidence shows benefits for preventing diabetes and in some gynaecological and obstetrical settings. However, almost all meta-analyses are of low or critically low quality according to AMSTAR 2 criteria.

Association between asthma and work absence in working adults in the United States

Summary: This study aimed to investigate the association between asthma and work absence in a large sample of US working adults, while controlling for several sociodemographic and health characteristics. The results showed that individuals with asthma were more likely to report at least one or three days of absence from work in the past 12 months than those without asthma. Furthermore, after adjusting for all control variables, asthma was positively and significantly associated with work absence.

Summary: This study aims to provide clinicians with evidence-based recommendations and implementation considerations for lifestyle-based mental health care for adults with Major Depressive Disorder (MDD). The study conducted systematic literature searches and formed nine recommendations, including physical activity, relaxation techniques, work-directed interventions, sleep, and mindfulness-based therapies. The study also emphasized the importance of implementing this approach with input from allied health professionals and support networks, and delivering interventions using a biopsychosocial-cultural framework.

Global burden of gout in 1990-2019: A systematic analysis of the Global Burden of Disease study 2019

Summary: Gout, a common rheumatic disease, has been understudied in non-industrialized countries. This study aimed to investigate the global burden of gout, including its associations with various factors. The findings revealed an increasing trend in the total number of gout patients and age-standardized prevalence rate from 1990 to 2019. Gout was most prevalent in Australasia and high-income North America, and higher sociodemographic index was associated with higher prevalence, incidence, and YLDs. High BMI and kidney dysfunction were identified as risk factors for gout, and gout showed correlations with other kidney diseases.

Summary: Despite progress in pharmacotherapy, many mental disorders remain insufficiently treated due to limited knowledge of pathophysiology, lack of biological markers, and challenges in drug development and testing. This paper discusses promising drugs and innovative mechanisms of action undergoing testing for various psychiatric disorders, as well as the clinical trial parameters that need to be considered for successful drug development. It also highlights the hurdles and perils in new drug development and testing and emphasizes the importance of considering these factors for increased chances of success.

Mental health differences between multiracial and monoracial college students in the United States: Emerging racial disparities

Effect of CD274 (PD-L1) overexpression on survival outcomes in 10 specific cancers: a systematic review and meta-analysis

Summary: This study aims to investigate the prognostic role of CD274 (PD-L1) overexpression in malignant tumors. The meta-analysis of eligible studies suggests that CD274 (PD-L1) overexpression is a potential biomarker for multiple types of cancers and is associated with worse prognosis. However, further studies are needed to confirm these findings and reduce heterogeneity.

Association Between Pollution and Frailty in Older People: A Cross-Sectional Analysis of the UK Biobank

Summary: This study aimed to investigate the potential association between air and noise pollution and frailty and prefrailty in older adults. The cross-sectional study of UK Biobank participants aged 60 and older found that increased exposure to nitrogen oxides (NOx) and particulate matter (PM2.5, PM2.5-10, PM10) was associated with a higher probability of being in the prefrail and frail category. These findings suggest that air pollution may contribute to frailty and highlight the importance of considering the impact of air pollutants in frailty prevention and intervention strategies.

Prevalence of tobacco smoking in people at clinical high-risk for psychosis: Systematic review and meta-analysis

Summary: This systematic review and meta-analysis examined the association between tobacco smoking and Clinical High Risk for Psychosis (CHR-P) status. The analysis showed that CHR-P individuals had a higher prevalence of smoking compared to healthy controls, but no causal association was found between smoking and the incidence of psychosis. Further research is needed.

Prevalence and outcomes of rapid cycling bipolar disorder: Mixed method systematic meta-review

Summary: This systematic review examines the definitions, prevalence, and clinical characteristics of rapid-cycling bipolar disorder (RC-BD). The results indicate that RC-BD has a high prevalence (22.3%-35.5% of BD cases) and is associated with inferior treatment response, suicide attempts, and unsatisfactory response to mood stabilizers. Factors such as childhood maltreatment, mixed features, female sex, and type-II BD also show associations with RC-BD.

Summary: This study aimed to estimate the main effect of two different physical activity (PA) programs, and a Low-Glycemic-Index Mediterranean Diet (LGIMD), or their combined effect on liver fibrosis parameters in subjects with NAFLD. The results showed that only subjects following LGIMD or LGIMD plus PA program experienced a significant reduction in liver fibrosis at 90 days.

The prevalence and clinical significance of loss of appetite in older patients with chronic kidney disease

Summary: This study aimed to determine predictors of loss of appetite among older adults with chronic kidney disease (CKD). Logistic regression analysis was performed using demographic and clinical data to identify the predictors of loss of appetite. The results showed that loss of appetite is common in older adults with CKD and is associated with insomnia and depressive mood.
