BackgroundThe rapid spread of coronavirus disease 2019 (COVID-19) was declared as an emerging public health threat by the World Health Organization. As various measures have been taken successfully to combat the epidemic caused by SARS-CoV-2, a growing number of fully recovered patients have been discharged from hospitals. However, some of them have relapsed. Little is known about the causes that triggered the relapse.Case presentationWe report a case of a 40years old man who suffered from recurrent pulmonary infection with progression of lesions on chest computed tomography (CT), elevated levels of ferritin and IL2R, reduced lymphocyte count and positive oropharyngeal swab test for SARS-CoV-2 again after 5 days discharge from hospital. The anti-SARS-CoV-2 antibody level of this patient was very low at the time of relapse, suggesting a weak humoral immune response to the virus. Total exon sequencing revealed mutations in TRNT1 gene, which may be responsible for B cell immunodeficiency. Therefore, uncleared SARS-CoV-2 at his first discharge was likely to lead to his recurrence. However, viral superinfection and non-infectious organizing pneumonia could not be completely excluded.ConclusionCOVID-19 relapse may occur in a part of discharged patients with low titers of anti-SARS-CoV-2 antibodies. These patients should be maintained in isolation for longer time even after discharge. A more sensitive method to detect SARS-CoV-2 needs to be established and serological testing for specific antibodies may be used as a reference to determine the duration of isolation.

Absence of the MFG-E8 gene prevents hypoxia-induced pulmonary hypertension in mice

Jun Wang, Jixing Wu, Xianying Zhu, Jinkun Chen, Jianping Zhao, Yongjian Xu, Jungang Xie

Summary: MFG-E8 plays a crucial role in hypoxia-induced PH, and its absence can attenuate the development of PH and vascular remodeling. MFG-E8 may serve as a potential therapeutic target or biomarker for PH.

MBD2 serves as a viable target against pulmonary fibrosis by inhibiting macrophage M2 program

Yi Wang, Lei Zhang, Guo-Rao Wu, Qing Zhou, Huihui Yue, Li-Zong Rao, Ting Yuan, Biwen Mo, Fa-Xi Wang, Long-Min Chen, Fei Sun, Jia Song, Fei Xiong, Shu Zhang, Qilin Yu, Ping Yang, Yongjian Xu, Jianping Zhao, Huilan Zhang, Weining Xiong, Cong-Yi Wang

Summary: Despite extensive studies, the mechanisms underlying PF remain poorly understood. This study found altered MBD2 expression in macrophages in lungs from different types of PF patients and mice with PF, and Mbd2 deficiency protected against BLM-induced PF. Mbd2 selectively influenced TGF-β1 production and M2 macrophage program through Ship promoter binding in macrophages.

Damaged lung gas exchange function of discharged COVID-19 patients detected by hyperpolarized 129Xe MRI

Haidong Li, Xiuchao Zhao, Yujin Wang, Xin Lou, Shizhen Chen, He Deng, Lei Shi, Junshuai Xie, Dazhong Tang, Jianping Zhao, Louis-S Bouchard, Liming Xia, Xin Zhou

Summary: The recovery process of COVID-19 patients remains unclear, with some experiencing ongoing respiratory issues. Xe-129 MRI analysis reveals higher ventilation defects and longer gas-blood exchange time in COVID-19 patients, indicating lung function impairment during recovery.

Preoperative risk grade predicts the long-term prognosis of intrahepatic cholangiocarcinoma: a retrospective cohort analysis

Jianping Zhao, Yao Chen, Jingjing Wang, Jian Wang, Ying Wang, Songshan Chai, Yuxin Zhang, Xiaoping Chen, Wanguang Zhang

Summary: The preoperative risk grade (PRG) integrating non-tumor-specific SIR markers like PLR and ALB was significantly associated with the overall survival of intrahepatic cholangiocarcinoma (ICC) patients, showing higher predictive accuracy compared to other systems. Incorporating PRG into a nomogram improved the predictive ability of ICC prognosis.

Optimizing Management to Reduce the Mortality of COVID-19: Experience From a Designated Hospital for Severely and Critically Ill Patients in China

Wei Zhu, Huaqiu Zhang, Yong Li, Zeyang Ding, Zhuo Liu, Yajun Ruan, Huan Feng, Ganxun Li, Bo Liu, Fan He, Ning Zhou, Jiangang Jiang, Zhixiang Wen, Gang Xu, Jianping Zhao, Bixiang Zhang, Daowen Wang, Zhouping Tang, Hui Wang, Jihong Liu

Summary: Limited data is available on the treatment for COVID-19, with the main treatment strategies including ventilatory support, multiple organ function preservation, glucocorticoid use, renal replacement therapy, anticoagulation, and restrictive fluid management. Early recognition and intervention, multidisciplinary collaboration, multi-organ function support, and personalized treatment might be the key for reducing mortality.

Hyperglycemia and Correlated High Levels of Inflammation Have a Positive Relationship with the Severity of Coronavirus Disease 2019

Wen Zhang, Chuanwei Li, Yu Xu, Binfeng He, Mingdong Hu, Guoqiang Cao, Li Li, Shuang Wu, Xia Wang, Chun Zhang, Jianping Zhao, Jungang Xie, Zihui Xu, Qi Li, Guansong Wang

Summary: This study found that COVID-19 patients with diabetes or hyperglycemia have higher inflammatory levels and more severe illness, suggesting that hyperglycemia is a risk factor for the increased severity of COVID-19. Initial measurement of plasma glucose levels after hospitalization may help identify patients predisposed to a worse clinical course.

Summary: The study showed that the combination of Shuanghuanglian (SHL) and standard care was safe and partially effective for treating COVID-19, with higher negative conversion rates of SARS-CoV-2 and promoted absorption of pneumonia inflammatory focus. No serious adverse events were reported in the SHL groups.

Increased epithelial galectin-13 expression associates with eosinophilic airway inflammation in asthma

Lingling Yi, Shuchen Zhang, Yuchen Feng, Wenliang Wu, Chenli Chang, Dian Chen, Shengchong Chen, Jianping Zhao, Guohua Zhen

Summary: Galectin-13 expression is significantly increased in asthma patients and is correlated with eosinophilic airway inflammation and Th2 signature genes. Inhaled corticosteroid treatment can reduce plasma galectin-13 levels, and galectin-13 may contribute to allergic airway eosinophilic inflammation by upregulating the expression of certain cytokines. Plasma galectin-13 levels may serve as a useful predictor for responses to ICS treatment.

Identification of Genetic Signature Associated With Aging in Pulmonary Fibrosis

Yanjiao Lu, Jinkun Chen, Shanshan Wang, Zhen Tian, Yan Fan, Meijia Wang, Jianping Zhao, Kun Tang, Jungang Xie

Summary: This study highlighted the significant impact of aging on idiopathic pulmonary fibrosis (IPF) and bleomycin-induced pulmonary fibrosis (BIPF) through comprehensive gene expression profile analysis. Aging was found to be closely associated with changes in differentially expressed genes (DEGs) and the severity of pulmonary fibrosis development. Four aging-associated core genes (Slc2a3, Fga, Hp, and Thbs1) were identified as key players in the pathogenesis of pulmonary fibrosis.
