c-MYC expression and maturity phenotypes are associated with outcome benefit from addition of ixazomib to lenalidomide-dexamethasone in myeloma

Objectives In the TOURMALINE-MM1 phase 3 trial in relapsed/refractory multiple myeloma, ixazomib-lenalidomide-dexamethasone (IRd) showed different magnitudes of progression-free survival (PFS) benefit vs placebo-Rd according to number and type of prior therapies, with greater benefit seen in patients with >1 prior line of therapy or 1 prior line of therapy without stem cell transplantation (SCT). Methods RNA sequencing data were used to investigate the basis of these differences. Results The PFS benefit of IRd vs placebo-Rd was greater in patients with tumors expressing high c-MYC levels (median not reached vs 11.3 months; hazard ratio [HR] 0.42; 95% CI, 0.26, 0.66; P < .001) compared with in those expressing low c-MYC levels (median 20.6 vs 16.6 months; HR 0.75; 95% CI, 0.42, 1.2). Expression of c-MYC in tumors varied based on the number and type of prior therapy received, with the lowest levels observed in tumors of patients who had received 1 prior line of therapy including SCT. These tumors also had higher expression levels of CD19 and CD81. Conclusions PFS analyses suggest that lenalidomide and ixazomib target tumors with different levels of c-MYC, CD19, and CD81 expression, thus providing a potential rationale for the differential benefits observed in the TOURMALINE-MM1 study. Summary: Patients with multiple myeloma have a lower efficacy from COVID-19 vaccination and a high rate of mortality from COVID-19 in hospitalized patients. The study found a relatively low infection rate but a quarter of the COVID-19 infections were severe. Treatment with CD38 antibody, cardiac and pulmonary comorbidities were independent predictors for ICU admission. Cardiac comorbidity was an independent predictor of mortality, while MM/AL in remission was associated with lower mortality.

Summary: Waldenstrom macroglobulinemia (WM) is a rare, indolent lymphoma that mostly affects the elderly. A study compared young WM patients to their older counterparts and found that young patients have lower overall survival and higher death attributable to WM. The treatment strategies for WM have not improved the outcome of young patients, and the impact of Bruton tyrosine kinase inhibitors on this patient population is still unknown.

Summary: The outcome of early treatment with antivirals or monoclonal antibodies for COVID-19 in patients with haematological malignancies is uncertain. A retrospective study was conducted to assess the efficacy of these treatments in mild/moderate cases between March 2021 and July 2022. Among the 328 patients included, treatment failure occurred in 9.5% of cases and was associated with older age, fewer vaccine doses, and monoclonal antibody therapy. The mortality rate in patients who developed severe COVID-19 despite early treatment was 26%.

Summary: This study explored the prognostic factors of cytopenic myelofibrosis (MF) and found that high molecular risk mutations, intermediate 2/high Dynamic International Prognostic Score System, and intermediate 2/high Myelofibrosis Secondary to Polycythemia Vera and Essential Thrombocythemia Prognostic Model were associated with cytopenic MF. Patients with cytopenia received lower doses of ruxolitinib and had worse treatment outcomes, suggesting the need for alternative therapeutic strategies.

The toxicity of ide-cel was consistent with previous reports.

Multiple myeloma with acute light chain cast nephropathy

Summary: Light chain cast nephropathy (LCCN) is a significant cause of acute kidney injury (AKI) in patients with multiple myeloma (MM) and is now considered as a defining event of myeloma. While long-term prognosis has improved, short-term mortality remains higher in patients with LCCN if renal failure is not reversed. Rapid reduction of involved serum free light chain is critical for renal function recovery. Treatment algorithm for MM patients with biopsy-proven LCCN or ruling out other causes of AKI is provided based on randomized trial data whenever available. Clinical trial enrollment is recommended prior to following the treatment algorithm.

Summary: Triplet regimens with immunomodulatory drugs and proteasome inhibitors have improved outcomes for patients with relapsed/refractory multiple myeloma. In the ELOQUENT-3 trial, the addition of elotuzumab to pomalidomide and dexamethasone did not impact the health-related quality of life (HRQoL) of patients. HRQoL remained stable and there were no significant differences between the treatment arms.

Summary: Measurable residual disease (MRD) evaluation is helpful in determining treatment duration in multiple myeloma (MM). However, limited longitudinal data exists on MRD during maintenance. This study investigated the prognostic value of MRD dynamics and found that MRD status at randomization and MRD dynamics during maintenance impact progression-free survival (PFS) risk. These findings support the significance of MRD assessment during maintenance in MM.
