Influence of Severe Vitamin D Deficiency on the Clinical Course of Inflammatory Bowel Disease

This study suggests that severe deficiency of 25(OH)D may be a marker of a more aggressive clinical course of IBD, leading to a higher risk of surgery in both Crohn's disease and ulcerative colitis patients.
Background Previous studies have shown vitamin D status to be associated with disease activity in patients with inflammatory bowel disease (IBD), but its influence on the clinical course of IBD has not been established. Aims We aimed to analyze whether the serum 25-hydroxyvitamin D3 [25(OH)D] status is associated with clinical characteristics and affects the risk of surgery in patients with IBD. Methods From the IBD registry of the Asan Medical Center, we identified all patients who had at least one 25(OH)D measurement; we then analyzed the association between clinical factors and 25(OH)D status. 25(OH)D was considered borderline deficient, deficient, and severely deficient at levels of < 30, < 20, and < 10 ng/mL, respectively. Results We included 711 Crohn's disease (CD) and 764 ulcerative colitis (UC) patients who had not undergone surgery before 25(OH)D was measured. Both in CD and in UC patients, reduced 25(OH)D was associated with higher disease activity scores and CRP levels (p < 0.001). Severe 25(OH)D deficiency was associated with ileocolonic disease and complicated behavior in CD (p < 0.05) and was relevant to the disease extent in UC (p < 0.001). Additionally, severe 25(OH)D deficiency was associated with CMV colitis in patients with UC (p < 0.001). In multivariable analysis, severe deficiency of 25(OH)D was an independent risk factor for surgery in both CD (HR 1.93, 95% confidence interval [CI] 1.38-2.70) and UC (HR 2.77, 95% CI 1.14-6.74). Conclusion Severe 25(OH)D deficiency may be a marker of a more aggressive clinical course of IBD.

Comparative Cost Analysis Between Endoscopic Resection and Surgery for Submucosal Colorectal Cancer

Therefore, endoscopic resection can be considered a cost-effective option for initial treatment of submucosal colorectal cancer.

Impact of Sarcopenia on Clinical Course of Inflammatory Bowel Disease in Korea

Summary: This study investigated the prevalence of sarcopenia at diagnosis and its clinical significance in Korean patients with IBD. The results showed that 56.8% of the population had sarcopenia, with higher prevalence in male patients. Sarcopenia may be associated with an increased risk of perianal surgeries in patients with CD.

Summary: This study investigated the transcriptional profile and compositional features of creeping fat (CF) in Crohn's disease (CD). The results showed that CF in CD patients had a large number of genes related to immune cell responses, while genes with a decremental pattern were associated with cell migration. Cell analysis revealed significant changes in cellular composition between CD-CF and Con-MAT groups.

Summary: This study compared the efficacy, tolerability, and safety of mini-OST and conventional OST. The results showed that there were no significant differences in preparation efficacy, satisfaction, and tolerability between the two groups. The first bowel movement occurred earlier in the mini-OST group. The occurrence of adverse events was comparable between the two groups. In conclusion, mini-OST showed similar efficacy, tolerability, and safety compared to conventional OST.

Influence of endoscopists & apos; expertise level on clinical outcomes after bridge-to-surgery stenting in obstructive colorectal cancer

Summary: This study aimed to investigate the effect of stenting-related factors, including endoscopists' expertise, on clinical outcomes after bridge-to-surgery (BTS) stenting for obstructive colorectal cancer (CRC). The results showed that the expertise of endoscopists was associated with better short-term outcomes, including high stenting success rate and low rate of stent-related complications after BTS stenting. The interval between stenting and surgery also played a role in postoperative complications.

Through radiologic, endoscopic, and histopathologic analysis, it was found that all patients exhibited focal stenotic regions and proximal bowel dilatation. Surgical treatment showed significant improvement in symptoms for these patients.

Treatment of inflammatory bowel disease-Asian perspectives: the results of a multinational web-based survey in the 8th Asian Organization for Crohn's and Colitis meeting

Summary: A web-based survey was conducted to evaluate the current treatment of inflammatory bowel disease (IBD) in Asian countries/regions. The results showed that anti-tumor necrosis factor (TNF) agents, anti-integrins, and anti-interleukin-12/23 agents were commonly used in Asian countries/regions. Treatment strategies were mostly similar among Asian doctors, but it is important to identify the most appropriate therapeutic strategy for Asian IBD patients.

Clinicopathologic Impact of Peptide Hormonal Expression in Rectal Neuroendocrine Tumors

Summary: This study aimed to evaluate the expression of multiple peptide hormones in rectal neuroendocrine tumors (NETs) and their clinical and prognostic implications. The results showed that several peptide hormones were expressed in rectal NETs, and tumors expressing serotonin or somatostatin were associated with lymphovascular invasion, chromogranin A expression, and shorter disease-free survival. The peptide hormonal expression status can classify rectal NETs into L-cell, enterochromaffin-cell, D-cell, null-expression, or mixed-expression type.
