Prognostic effects of TERT promoter mutations are enhanced by coexistence with BRAF or RAS mutations and strengthen the risk prediction by the ATA or TNM staging system in differentiated thyroid cancer patients

BACKGROUNDRecent reports suggest that mutations in the promoter of the gene encoding telomerase reverse transcriptase (TERT) affect thyroid cancer outcomes. METHODSIn all, 551 patients with differentiated thyroid cancer (DTC) enrolled in this study. The median follow-up duration was 4.8 years (interquartile range, 3.4-10.6 years). RESULTSTERT promoter mutations were detected in 25 DTCs (4.5%): 2.8% in neither BRAF-mutated nor RAS-mutated tumors, 4.8% in BRAF-mutated tumors, and 11.3% in RAS-mutated tumors. Moreover, they were frequently observed in American Thyroid Association (ATA) high-risk and TNM stage III/IV groups (9.1% and 12.9%, respectively). The coexistence of BRAF or RAS with TERT promoter mutations increased aggressive clinicopathologic features, recurrence (hazard ratio [HR] for BRAF, 4.64; 95% confidence interval [CI], 1.42-15.18; HR for RAS, 5.36; 95% CI, 1.20-24.02), and mortality (HR for BRAF, 15.13; 95% CI, 1.55-148.23; HR for RAS, 14.75; 95% CI, 1.30-167.00), even after adjustments for the age at diagnosis and sex, although the significance was lost after additional adjustments for pathologic characteristics. Furthermore, TERT promoter mutations significantly increased the risk of both recurrence and mortality in the ATA high-risk (HR for recurrence, 5.79; 95% CI, 2.07-16.18; HR for mortality, 16.16; 95% CI, 2.10-124.15) and TNM stage III/IV groups (HR for recurrence, 3.60; 95% CI, 1.19-10.85; HR for mortality, 9.06; 95% CI, 2.09-39.26). CONCLUSIONSThe coexistence of BRAF or RAS mutations enhanced the prognostic effects of TERT promoter mutations. Furthermore, TERT promoter mutations strengthened the predictions of mortality and recurrence by the ATA and TNM staging systems, particularly for high-risk patients with DTC. Characteristics of germline mutations in Korean patients with pheochromocytoma/paraganglioma

Summary: This study investigated the characteristics of germline mutations in a large group of Korean patients with PPGL. The results showed that approximately 21% of sporadic PPGLs harbored germline mutations, which were associated with younger age at diagnosis and the presence of bilateral and multifocal tumors. The study also identified a specific missense mutation in Asian patients that was associated with unilateral pheochromocytoma with dominant epinephrine production.

A Randomized Study of Lenvatinib 18 mg vs 24 mg in Patients With Radioiodine-Refractory Differentiated Thyroid Cancer

Summary: This study compared the efficacy and toxicity of lenvatinib starting at doses of 24 mg/day and 18 mg/day in patients with RR-DTC. The results showed that the objective response rate was 57.3% in the 24 mg/day arm and 40.3% in the 18 mg/day arm, indicating that the starting dose of 18 mg/day did not demonstrate noninferiority. However, there was no clinically relevant difference in the safety profile between the two arms.

Changes in Patterns of Physical Activity and Risk of Heart Failure in Newly Diagnosed Diabetes Mellitus Patients

Summary: Among individuals with newly diagnosed T2DM, the risk of heart failure was reduced in those with higher levels of physical activity after diagnosis. Our results suggest that either increasing or maintaining the frequency of physical activity after the diagnosis of T2DM may lower the risk of heart failure.

Effect of postoperative radiotherapy for patients with differentiated thyroid cancer

Summary: In this retrospective study, the efficacy and safety of postoperative radiotherapy (PORT) for differentiated thyroid cancer (DTC) with high risk features were evaluated. Results showed that PORT can reduce locoregional recurrence in DTC patients without severe toxicity. Further studies are needed to identify the patients who can benefit from PORT.

Risk of thyroid cancer in a lung cancer screening population of the National Lung Screening Trial according to the presence of incidental thyroid nodules detected on low-dose chest CT

Summary: This study evaluated the risk of thyroid cancer in a lung cancer screening population based on the presence of incidental thyroid nodules detected on low-dose chest computed tomography (LDCT). The results showed that the incidence of thyroid cancer was higher in the ITN-positive group compared to the ITN-negative group, but all cancers were papillary thyroid microcarcinomas (PTMCs). Therefore, thorough screening for thyroid incidentalomas may not be necessary in patients with a heavy smoking history.

Acromegaly and the long-term fracture risk of the vertebra and hip: a national cohort study

Summary: The importance of molecular pathology tests has increased in the past decade, leading to a demand for efficient training in this field. The Korean Society of Pathologists established a task force to develop a refined educational curriculum in molecular pathology. An online educational session was conducted, and the understanding of the participants significantly increased in various learning objectives, particularly in the field of next-generation sequencing. The feedback received on the refined curriculum was predominantly positive.

User Experience of Augmented Reality Glasses-based Tele-Exercise in Elderly Women

Summary: This study aimed to compare the user experience differences of elderly women participating in remote sports exercise programs using tablet and augmented reality (AR) glasses. The results showed no significant differences in attractiveness, stimulation, and novelty between the two groups, but there were significant differences in efficiency, perspicuity, and dependability. Therefore, when developing AR glasses-based exercise programs for the elderly, the efficiency, clarity, and stability of the program should be considered to meet participants' needs.

Bilateral axillo-breast approach robotic thyroidectomy in pediatric patients with thyroid disease and cancer

Summary: This study retrospectively evaluated the clinical characteristics and surgical outcomes of 26 pediatric patients aged 5-18 years who underwent BABA robotic thyroidectomy. The operation time and surgical completeness were similar to robotic thyroidectomy in adults, and most complications were minor and transient. Therefore, robotic thyroidectomy can be considered as a surgical option regardless of patient age or diagnosis.
