Severe vitamin D deficiency is associated with emphysema progression in male patients with COPD

Background: Patients with chronic obstructive pulmonary disease (COPD) have an increased risk of vitamin D deficiency. Vitamin D levels also correlate with lung function in patients with COPD. However, there are few reports on vitamin D deficiency and emphysema severity in COPD. This study aimed to investigate the effects of plasma 25-hydroxyvitamin D (25-OHD) level on emphysema severity in male COPD patients. Methods: A total of 151 male subjects were selected from the Korean Obstructive Lung Disease (KOLD) cohort. Subjects were subdivided into four subgroups according to their baseline plasma 25-OHD level: sufficiency (>= 20 ng/ml), mild deficiency (15-20 ng/ml), moderate deficiency (10-15 ng/ml), and severe deficiency (<10 ng/ml). Results: Baseline computed tomography (CT) emphysema indices revealed significant differences among the subgroups (p = 0.034). A statistically significant difference was also observed among the subgroups regarding change in the CT emphysema index over 3 years (p = 0.047). The annual increase in emphysema index was more prominent in the severe deficiency group (1.34% per year) than in the other groups (0.41% per year) (p = 0.003). Conclusions: This study demonstrates that CT emphysema indices were different among the four subgroups and supports that severe vitamin D deficiency is associated with rapid progression of emphysema in male patients with COPD.

Summary: This study aimed to describe the profile of severe asthma patients eligible for both anti-IgE and anti-IL5/5R and compare the effectiveness of these two treatments in real-life settings. The results showed that anti-IL5/5R was superior to anti-IgE in reducing asthma exacerbations and long-term oral corticosteroid use, while there was some evidence suggesting a decrease in asthma-related hospitalizations with anti-IL5/5R treatment.

Evaluation of an air quality warning system for vulnerable and susceptible individuals in Korea: an interrupted time series analysis

Summary: This study investigates the effects of an air quality warning system (AQWS) implemented in Korea in 2015 on the incidence and exacerbation rates of environmental diseases. The results show that the AQWS has led to a gradual decrease in asthma incidence, as well as significant decreases in cardiovascular disease and stroke incidence. However, there was no immediate or gradual decrease in the exacerbation rate of any environmental disease. The findings suggest that tailoring the AQWS to demographic and sociological characteristics and providing enhanced education can effectively reduce air pollution-related health risks.

Tiotropium Bromide Improves Neutrophilic Asthma by Recovering Histone Deacetylase 2 Activity

Summary: This study found that tiotropium bromide (TIO) plays a significant role in neutrophilic asthma by recovering histone deacetylase 2 (HDAC2) activity and regulating the transcription of inflammatory cytokines. Experimental results from mouse models and asthma patients' cells showed that TIO can improve NF-KB activity, CXCL1 levels, and HDAC2 activity. These findings suggest that TIO modulates inflammation by restoring HDAC2 activity, possibly through the PLC gamma-1-IP3-IP3R-mediated intracellular calcium ion pathway.
