Circulating branched-chain amino acids and incident heart failure in type 2 diabetes: The Hong Kong Diabetes Register

Aim Levels of branched-chain amino acids (BCAAs, namely, isoleucine, leucine, and valine) are modulated by dietary intake and metabolic/genetic factors. BCAAs are associated with insulin resistance and increased risk of type 2 diabetes (T2D). Although insulin resistance predicts heart failure (HF), the relationship between BCAAs and HF in T2D remains unknown. Methods In this prospective observational study, we measured BCAAs in fasting serum samples collected at inception from 2139 T2D patients free of cardiovascular-renal diseases. The study outcome was the first hospitalization for HF. Results During 29 103 person-years of follow-up, 115 primary events occurred (age: 54.8 +/- 11.2 years, 48.2% men, median [interquartile range] diabetes duration: 5 years [1-10]). Patients with incident HF had 5.6% higher serum BCAAs than those without HF (median 639.3 [561.3-756.3] vs 605.2 [524.8-708.7] mu mol/L; P = .01). Serum BCAAs had a positive linear association with incident HF (per-SD increase in logarithmically transformed BCAAs: hazard ratio [HR] 1.22 [95% CI 1.07-1.39]), adjusting for age, sex, and diabetes duration. The HR remained significant after sequential adjustment of risk factors including incident coronary heart disease (1.24, 1.09-1.41); blood pressure, low-density lipoprotein cholesterol, and baseline use of related medications (1.31, 1.14-1.50); HbA(1c), waist circumference, triglyceride, and baseline use of related medications (1.28, 1.11-1.48); albuminuria and estimated glomerular filtration rate (1.28, 1.11-1.48). The competing risk of death analyses showed similar results. Conclusions Circulating levels of BCAAs are independently associated with incident HF in patients with T2D. Prospective cohort analysis and randomized trials are needed to evaluate the long-term safety and efficacy of using different interventions to optimize BCAAs levels in these patients.

Summary: This study utilized single-cell RNA sequencing to explore the transcriptional landscape and heterogeneity of dysfunctional endothelial cells in atherosclerotic aortae. SOX4 was identified as a novel marker for atherosclerosis and was shown to promote atherogenesis and endothelial-to-mesenchymal transition. Hyperlipidemia-associated cytokines and oscillatory blood flow were found to upregulate SOX4 expression, while the anti-diabetic drug metformin suppressed SOX4 levels in endothelial cells.

Summary: This study explored the usage patterns of oral glucose-lowering drugs (OGLDs), particularly sulphonylureas (SUs), in Asian patients with type 2 diabetes (T2D). The findings showed that SUs, often combined with metformin, are a popular class of OGLDs in Asia. They are associated with good glycemic control and safety profiles.

Cardiorenal diseases in type 2 diabetes mellitus: clinical trials and real-world practice

Summary: Patients with type 2 diabetes mellitus (T2DM) often have comorbidities and premature mortality. Traditional drugs and certain inhibitors have organ-protective effects, while SGLT2i, GLP1RA, and nonsteroidal mineralocorticoid receptor antagonists provide additional cardiorenal benefits. However, there are care gaps due to detection issues, therapeutic inertia, and poor adherence.

Age-specific population attributable risk factors for all-cause and cause-specific mortality in type 2 diabetes: An analysis of a 6-year prospective cohort study of over 360,000 people in Hong Kong

Summary: This study examined the risk factors for mortality in people with type 2 diabetes and found that these factors have different impacts on different age groups. The study found that suboptimal blood pressure control was the leading risk factor for mortality in younger individuals, while chronic kidney disease (CKD) was the leading risk factor for older individuals. Therefore, early control of blood pressure is crucial for reducing premature mortality in young people and preventing CKD and related mortality in older individuals.

Higher dietary magnesium and potassium intake are associated with lower body fat in people with impaired glucose tolerance

Summary: This study found that the dietary intake of minerals was negatively correlated with body fat in people with impaired glucose tolerance (IGT). People with obesity had lower daily consumption of iron, magnesium, and potassium compared to overweight and normal weight individuals. Higher dietary magnesium and potassium intake were associated with lower body fat in people with IGT. Inadequate dietary mineral intake may contribute to obesity and metabolic disorders independent of macronutrients and fiber consumption.

Summary: This study compared the risk of diabetes-related complications and mortality in Chinese adults with type 1 diabetes onset and type 2 diabetes onset. The results showed that adults with type 1 diabetes onset had higher risks of diabetic ketoacidosis, severe hypoglycemia, end-stage kidney disease, cardiovascular disease, and mortality compared to those with youth-onset type 1 diabetes and type 2 diabetes presenting at the same age.

Summary: This study aimed to investigate whether early treatment intensification using dipeptidylpeptidase 4 inhibitors (DPP4i) delays insulin initiation in Chinese patients diagnosed with type 2 diabetes for less than 5 years. The results showed that early DPP4i intensification and low glycated haemoglobin (HbA1c) variability score (HVS) were associated with delayed insulin initiation.

Metabolic Dysfunction-Associated Steatotic Liver Disease (MASLD): A State-of-the-Art Review

Summary: Metabolic dysfunction-associated steatotic liver disease (MASLD) is the latest term for steatotic liver disease associated with metabolic syndrome. It is the most common cause of chronic liver disease and liver-related morbidity and mortality. In tackling the public health threat of obesity and obesity-related diseases, including MASLD, it is important for all stakeholders to be involved. Simple and clear assessment and referral pathways using non-invasive tests are crucial to identifying and referring patients with severe MASLD to specialist care, while managing patients with milder disease in primary care.
