Uterine Fibroids (UFs), or leiomyomas, represent the most frequent pelvic tumor in reproductive-aged women. Although of benign origin, UFs decrease fertility and cause significant reproductive dysfunctions. Compared to normal myometrium, UFs are characterized by a clinical and molecular heterogeneity as demonstrated by the presence of multiple genetic alterations and altered signaling pathways. Recently, selective progesterone-receptor modulators (SPRM), as ulipristal acetate (UPA), have demonstrated their clinical benefits by reducing tumor growth and extracellular matrix deposition. For these reasons, UPA is used in the clinical practice as an intermittent treatment for women symptomatic for UFs or, sometimes, before a myomectomy. However, drug effects on signaling pathways frequently upregulated in UFs remain largely unknown. In fact, the mechanisms of action of the UPA on UFs and on the surrounding areas are not yet understood. To learn more about UPA molecular mechanisms, UF samples were treated ex vivo with UPA and profiled for drug effects on selected markers. During this preliminary ex vivo UPA administration, significant changes were observed in the expression levels of proteins related to cell cycle regulation, cytoskeleton remodeling, and drug resistance. The UPA administration reduced cofilin, Erk and Src phosphorylation, p27 and ezrin protein levels, but not Akt phosphorylation and cyclin D1 and beta-catenin levels. This preliminary ex vivo biological analysis provided new insights into the mechanism of action of UPA in the treatment of UFs, which could better explain the biological functioning of the drug on UFs.

Therefore, it is important to focus on the outcomes of adolescent pregnancies and provide family planning and baby care social trainings.

Intrapartum ultrasound for fetal head asynclitism: Is it possible to establish a degree of asynclitism to correlate to delivery outcome?

Summary: The study aimed to investigate the role of intrapartum ultrasound in diagnosing asynclitism and the importance of the degree of asynclitism in labor outcomes. A cohort study was conducted on 41 low-risk pregnant women with singleton-vertex fetus. Severe and moderate asynclitism was observed in 41.7% and 58.8% of women, respectively. Severe asynclitism was associated with increased operative birth and maternal-fetal complications. Detection of asynclitism degree by intrapartum ultrasound could be useful in alerting the obstetrics team to potential perinatal problems during delivery.

Prolonged Dystocic Labor in Neuraxial Analgesia and the Role of Enkephalin Neurotransmitters: An Experimental Study

The study compared LUS samples obtained from emergency cesarean section (CS) cases of 38 patients with PDL to elective CS cases of 37 patients. The results showed a significant reduction in En in the LUS of the PDL group, indicating that the drugs used during LNA may not effectively control the dystocic pain.

Local anesthesia for Altis® single incision sling in women with stress urinary incontinence

Summary: The aim of this study was to verify the efficacy and safety of local anesthesia during Altis (R) single incision placement for stress urinary incontinence. The results showed that local anesthesia had a good objective and subjective cure rate, with shorter operative time compared to spinal anesthesia. There were no significant differences in cure rate and postoperative complications between the two anesthesia methods. Therefore, local anesthesia may be considered a safe alternative to spinal anesthesia for Altis (R) implantation, allowing for outpatient procedures.

Myomectomy in adult women of reproductive age: a propensity score-matched study for pregnancy rates

The study included laparoscopic intracapsular myomectomy (LIM), open laparotomy (OIM), and non-surgical management as a control group. The results showed that there were no significant differences in reproductive outcomes between operated and non-operated patients, indicating that myomectomy in patients over 40 years old did not have a detrimental effect on future pregnancy rates compared to expectant management.

Summary: In this study, a green olive leaf extract obtained by water ultrasound-assisted extraction procedure (OLE) showed antioxidant and lipid-lowering effects on human hepatic HuH7 cells treated with high concentrations of free fatty acids (FFA). OLE reduced lipid accumulation, oxidative stress, and improved mitochondrial membrane potential and hepatic parameters by restoring the expression of enzymes involved in insulin signaling and lipid metabolism.
