Background: The causes of many cases of preterm birth (PTB) remain enigmatic. Increased understanding of how epigenetic factors are associated with health outcomes has resulted in studies examining DNA methylation (DNAm) as a contributing factor to PTB. However, few studies on PTB and DNAm have included African American women, the group with the highest rate of PTB. Methods: The objective of this review was to systematically analyze the existing studies on DNAm and PTB among African American women. Results: Studies (N = 10) were limited by small sample size, cross-sectional study designs, inconsistent methodologies for epigenomic analysis, and evaluation of different tissue types across studies. African Americans comprised less than half of the sample in 50% of the studies reviewed. Despite these limitations, there is evidence for an association between DNAm patterns and PTB. Conclusions: Future research on DNAm patterns and PTB should use longitudinal study designs, repeated DNAm testing, and a clinically relevant definition of PTB and should include large samples of high-risk African American women to better understand the mechanisms for PTB in this population.

Epigenetic Age Acceleration Markers Are Associated With Physiologic Frailty and All-Cause Mortality in People With Human Immunodeficiency Virus

Krisann K. Oursler, Vincent C. Marconi, Zeyuan Wang, Ke Xu, Monty Montano, Kaku So-Armah, Amy C. Justice, Yan Sun

Summary: The associations between epigenetic age acceleration markers and physiologic frailty and mortality in people with HIV provide insight into accelerated aging and support the development of interventions for preventing and treating age-related diseases in this population.

A comparison of conceptual frameworks to examine health inequities in End-of-Life care

Jung A. Kang, Veronica Barcelona

Summary: This paper discusses existing conceptual frameworks that can be applied to the examination of health inequities in end-of-life care and health outcomes. The researchers emphasize the need for solid conceptualizations to address the complex nature of factors influencing these inequities. Three frameworks were identified and reviewed, with the Conceptual Framework of Minority Access to End-of-Life Care found to be more applicable when adapted to include fundamental characteristics such as sex and gender.

Summary: This study incorporates a diverse cohort of individuals to explore the genetic landscape of coronary artery disease. The researchers discovered novel loci associated with the disease in different ancestral groups, and identified common haplotypes that contribute to risk stratification. They also found genetic overlaps between coronary artery disease and clinical coronary atherosclerosis, and related these genetic factors to insulin resistance and other traits.

Associations between Paternal Co-residence and Child Health among African American Children

Veronica Barcelona, Ambre Flowers, Billy A. Caceres, Cindy A. Crusto, Jacquelyn Y. Taylor

Summary: This study investigated the associations between paternal co-residence and asthma, obesity, and blood pressure among children aged 3-5 years. While children who lived with their fathers were found to have a lower likelihood of asthma, this association became non-significant after adjusting for other factors. Paternal co-residence was not significantly associated with child obesity or blood pressure. Further research is needed to understand the impact of different family living situations on child health.

A qualitative analysis of stigmatizing language in birth admission clinical notes

Veronica Barcelona, Danielle Scharp, Betina R. R. Idnay, Hans Moen, Dena Goffman, Kenrick Cato, Maxim Topaz

Summary: The presence of stigmatizing language in electronic health records (EHR) was studied to identify implicit biases in healthcare disparities. A qualitative analysis was conducted on 1117 birth admission EHR notes, revealing various categories of stigmatizing language, such as disapproval, patient credibility questioning, difficult patients, stereotyping, and unilateral decisions. Additionally, a new category of stigmatizing language based on power and privilege was identified. Clinicians from different disciplines recorded stigmatizing language in medical records, particularly in birth admission triage notes. The inconsistent documentation of favorable patient traits indicated a power/privilege language bias. Further research can focus on tailored interventions to improve perinatal outcomes for all birthing people and their families.

Associations of inflammation-related proteome with demographic and clinical characteristics of people with HIV in South Africa

Summary: Elevated levels of inflammation in HIV-infected individuals are a major contributor to age-related diseases among this population. This study found multiple inflammatory proteins associated with sex, CD4+ cell count, and age in HIV-infected individuals. Many of these age-associated proteins have been previously linked to age in the general population and are also associated with chronic diseases such as cardiovascular disease and neurocognitive decline. Proteomics was shown to be a valuable tool for evaluating and characterizing the inflammatory status of HIV-infected individuals.

Stress Overload and DNA Methylation in African American Women in the Intergenerational Impact of Genetic and Psychological Factors on Blood Pressure Study

Jolaade Kalinowski, Yunfeng Huang, Martin A. Rivas, Veronica Barcelona, Michelle L. Wright, Cindy Crusto, Tanya Spruill, Yan Sun, Jacquelyn Y. Taylor

Summary: Experiencing psychosocial stress, such as perceived stress overload, may influence DNA methylation, leading to increased risk for cardiovascular disease in African American women. However, this study did not find a direct association between self-reported stress levels and DNA methylation, but identified differentially methylated CpG sites related to heart disease-related genes. This study provides foundational knowledge for future exploration of epigenetic associations with stress and other psychosocial measures in African Americans.
