Summary: Lung cancer has high morbidity and mortality rates, with tumor metastasis being a significant risk factor for death. Even after complete removal of a tumor at an early stage, there is still a risk of recurrence. Lymph node micrometastasis has been found to be associated with poor prognosis, but predicting prognosis and choosing the appropriate surgical and postoperative adjuvant chemotherapy options remain challenging for clinicians.

Summary: This study aimed to discuss the underlying oncological issues in staging of mediastinal lymph node metastasis in patients with left lung cancer who underwent extended radical lymphadenectomy. The results showed that cN status and adjuvant chemotherapy were significant prognostic factors for left lung cancer with mediastinal lymph node metastasis, rather than pN status. These findings broaden the discussion of oncological issues in the staging of mediastinal lymph node metastasis of left lung cancer.

Summary: Personalized treatment of metastatic non-squamous NSCLC requires comprehensive molecular characterization of the tumor, including detection of driver mutations and PD-L1 expression. Different sampling techniques may be necessary to obtain sufficient tumor material. This case report highlights the importance of biopsy technique for precise tumor characterization. Only EBUS-TBLNC provided complete characterization, demonstrating PD-L1 expression without actionable genetic alterations.

Summary: The anatomical locations of non-small cell lung carcinomas are closely related to metastatic lymph nodes, with the apicoposterior segment of the left superior lobe showing a significant association with mediastinal nodes. 18F-FDG PET/CT demonstrated diagnostic power in detecting mediastinal nodes, although there was no statistically significant relationship between tumor location and TNM staging. Further investigation in larger study groups is needed to explore this relationship.

Summary: This study aimed to investigate suitable patients for adjuvant chemoradiation therapy (CRT) in non-small cell lung cancer (NSCLC) with heterogeneous pathological N2 (pN2) diseases. The study retrospectively reviewed data of pN2 NSCLC patients in Shanghai Pulmonary Hospital. The results showed that adjuvant CRT significantly improved the prognosis of pN2 patients, regardless of single or multiple lymph node metastasis.

The results showed that ACT did not provide any survival benefit, and patients with older age and early-stage disease may not benefit from ACT after nCRT followed by TME plus LLND.

Summary: Through whole-exome sequencing and RNA sequencing analysis of GGO-like lung adenocarcinoma, we identified differences compared to solid nodule lung adenocarcinoma, including lower TP53 mutation frequency and less active cell proliferation-related pathways. We also found differences between GGO-like lung adenocarcinoma and minimally invasive adenocarcinoma in terms of EGFR mutation frequency, CNV load, and immune cell abundance. Finally, we developed a transcriptomic signature that predicts the progression from GGO to solid nodule and patients' prognosis.
