Schizophrenia (SCZ) is a severe psychiatric disorder affecting approximately 23 million people worldwide. It is considered the eighth leading cause of disability according to the Wood Health Organization and is associated with a significant reduction in life expectancy. Antipsychotics represent the first-choice treatment in SCZ, but approximately 30% of patients fail to respond to acute treatment. These patients are generally defined as treatment-resistant and are eligible for clozapine treatment. Treatment-resistant patients show a more severe course of the disease, but it has been suggested that treatment-resistant schizophrenia (TRS) may constitute a distinct phenotype that is more than just a more severe form of SCZ. TRS is heritable, and genetics has been shown to play an important role in modulating response to antipsychotics. Important efforts have been put into place in order to better understand the genetic architecture of TRS, with the main goal of identifying reliable predictive markers that might improve the management and quality of life of TRS patients. However, the number of candidate gene and genome-wide association studies specifically focused on TRS is limited, and to date, findings do not allow the disentanglement of its polygenic nature. More recent studies implemented polygenic risk score, gene-based and machine learning methods to explore the genetics of TRS, reporting promising findings. In this review, we present an overview on the genetics of TRS, particularly focusing our discussion on studies implementing polygenic approaches.

Summary: This study identified 6 new genetic variants associated with Alzheimer's disease risk through a large-scale genetic association study. In men, treatment with PPIs and the CYP2C19 poor metabolizer status were linked to a higher incidence of chronic migraine.

Summary: Bipolar disorder is associated with inflammation-triggered tryptophan degradation, and a poorer response to lithium is associated with higher levels of kynurenine and quinolinic acid, indicating a pro-inflammatory state and increased degradation towards the neurotoxic branch. The response to valproate and lamotrigine is not affected by tryptophan metabolite levels. These findings support the anti-inflammatory properties of lithium and suggest a potential personalized medicine approach for bipolar disorder.

RNA Biomarkers in Bipolar Disorder and Response to Mood Stabilizers

Summary: In this narrative review, the potential use of messenger RNAs and noncoding transcripts as peripheral markers of bipolar disorder (BD) and/or response to lithium and other mood stabilizers are summarized. Studies have investigated specific targets or pathways and there is heterogeneity in the type of cells or biofluids included. However, there is a growing number of hypothesis-free designs and integration of coding and noncoding RNAs data. Studies conducted in cellular models provide promising findings for investigating the molecular determinants of BD and clinical response.

Pharmacokinetic Markers of Clinical Outcomes in Severe Mental Illness: A Systematic Review

Summary: The term severe mental illness (SMI) refers to psychiatric disorders that have the highest clinical burden and socio-economic impact on individuals and their communities. Pharmacogenomic (PGx) approaches show potential in personalizing treatment and improving outcomes for SMI, but lack of standardization and limited evidence hinder interpretation. A systematic review of literature and analysis of 42 articles suggest that PGx testing may be cost-effective in specific settings and have modest clinical benefits, but efforts are needed to improve standardization, knowledge, and guidelines in clinical practice for screening recommendations.

Few studies have explored sex-related differences in antidepressant treatment response, revealing a lack of omics studies in this area.
