Oropharyngeal dysphagia (OD) affects older and neurological patients, causing malnutrition and dehydration and increasing the risk for aspiration pneumonia. There is evidence that sensory deficits in those populations are closely related to swallowing disorders, and several research groups are developing new therapies based on sensory stimulation of this area. More information on the sensory innervation participating in the swallow response is needed to better understand the pathophysiology of OD and to develop new treatments. This review focuses on the sensory innervation of the human oropharynx and larynx in healthy people compared with patients with swallowing disorders in order to unravel the abnormalities that may lead to the loss of sensitivity in patients with OD. We also hypothesize the pathway through which active sensory-enhancement treatments may elicit their therapeutic effect on patients with swallowing dysfunctions. As far as we know, this is the first time a review covers the anatomy, histology, ultrastructure, and molecular biology of the sensory innervation of the swallowing function.

Prognostic value of the nodal yield in elective neck dissections in patients with head and neck carcinomas

Summary: This study found that the nodal yield in elective neck dissections in patients without evidence of lymph node disease had prognostic capacity depending on the location of the primary tumor, with significant impact on disease-specific mortality for tumors located in the oral cavity, oropharynx, and hypopharynx. However, for tumors located in the larynx, the number of dissected nodes did not significantly influence the prognosis.

Causes of long-term mortality in patients with head and neck squamous cell carcinomas

Relationship between the transcriptional expression of PIM1 and local control in patients with head and neck squamous cell carcinomas treated with radiotherapy

Summary: This study analyzed the relationship between the transcriptional expression of PIM-1 and local response to radiotherapy in patients with head and neck squamous cell carcinoma (HNSCC). The results showed that patients with elevated transcriptional expression levels of PIM-1 had a significantly higher risk of local recurrence after radiotherapy.

Summary: This study demonstrated that assessing skeletal muscle mass (SMM) from the computed tomography image of the 3rd cervical vertebra (C3) can predict the risk of pharyngocutaneous fistula (PCF) formation after total laryngectomy (TL). Low SMM was significantly associated with a higher risk of PCF, and sarcopenia was more common in underweight patients, those undergoing extended surgeries, or presenting preoperative anemia or hypoalbuminemia.

Summary: Novel therapeutic strategies are urgently needed for HNSCC patients with therapy resistance. T22-PE24-H6 and T22-DITOX-H6 nanotoxins show promising potential for the treatment of HNSCC by specifically targeting CXCR4-overexpressing cells and triggering caspase-3/GSDME-dependent pyroptosis.

Pharyngocutaneous fistula in irradiated patients: systematic review and our experience

Summary: This study found that there were no significant differences in the pharyngocutaneous fistula rate after total laryngectomy depending on the previous treatment with radiotherapy or chemoradiotherapy, or depending on the interval between radiotherapy treatment and surgery. Patients who received chemoradiotherapy required surgical repair more frequently.

Summary: Endoscopic endonasal surgery is an effective treatment for selected cases of sinonasal cancers, but there is still debate in some cases. Based on the available literature, this surgery allows for safe resection of selected lesions with decreased morbidity, but when contraindicated, craniofacial and transfacial approaches remain the best option.

Summary: This study developed a novel protein nanoparticle, T22-DITOX-H6, which selectively delivers the diphtheria toxin cytotoxic domain to CXCR4(+) HNSCC cells. The nanoparticles effectively blocked tumor metastasis and recurrence without causing off-target toxicity.

Differential characteristics of patients with squamous cell carcinoma of the head and neck with no history of tobacco or alcohol use

Summary: This study aimed to analyze the differential characteristics of HNSCC patients without a history of toxic substance consumption. Through a retrospective study of 4,694 HNSCC patients treated in our center from 1985 to 2019, it was found that 7.7% of patients did not report a history of toxic substance consumption. Patients without a toxic history were older, had a higher proportion of women, a higher frequency of cases located in the oral cavity, a higher proportion of early-stage diagnoses, and a lower incidence of second neoplasms. The overall survival of patients without a history of toxic substance consumption was significantly higher than that of patients with toxic substance use.
