Vitamin D and Lung Outcomes in Elderly COVID-19 Patients

Alberto Sulli, Emanuele Gotelli, Andrea Casabella, Sabrina Paolino, Carmen Pizzorni, Elisa Alessandri, Marco Grosso, Diego Ferone, Vanessa Smith, Maurizio Cutolo

Summary: The study demonstrates that vitamin D deficiency in COVID-19 patients is associated with severe lung involvement, longer disease duration, and higher risk of death, particularly in elderly patients. Low vitamin D levels are negatively correlated with the severity of radiologic pulmonary involvement.

Trabecular Bone Score as a Reliable Measure of Lumbar Spine Bone Microarchitecture in Acromegalic Patients

Elena Nazzari, Andrea Casabella, Sabrina Paolino, Claudia Campana, Giuliana Corica, Federica Nista, Angelo Milioto, Alberto Tagliafico, Manuela Albertelli, Mara Boschetti, Marcello Bagnasco, Maurizio Cutolo, Diego Ferone, Federico Gatto

Summary: This study found that acromegalic patients have significantly lower trabecular bone scores (TBS) compared to healthy controls, indicating impaired lumbar spine microarchitecture. Uncontrolled disease and hypogonadism contribute to the deterioration of TBS in acromegaly.

A deep learning system for quantitative assessment of microvascular abnormalities in nailfold capillary images

Praveen Gurunath Bharathi, Michael Berks, Graham Dinsdale, Andrea Murray, Joanne Manning, Sarah Wilkinson, Maurizio Cutolo, Vanessa Smith, Ariane L. Herrick, Chris J. Taylor

Summary: This study aimed to develop and validate a fully automated image analysis system for nailfold capillaroscopy in SSc diagnosis. By mimicking the image interpretation strategies of SSc experts, deep learning networks were used to detect and measure each capillary in the distal row of vessels. The results showed that the fully automated analysis using deep learning achieved diagnostic performance at least as good as SSc experts and is sufficiently robust to work with low-cost digital microscope images.

Vitamin D and COVID-19: Narrative Review after 3 Years of Pandemic

Emanuele Gotelli, Stefano Soldano, Elvis Hysa, Sabrina Paolino, Rosanna Campitiello, Carmen Pizzorni, Alberto Sulli, Vanessa Smith, Maurizio Cutolo

Summary: This narrative review collected laboratory and clinical results of vitamin D supplementation. The results generally support the utility of maintaining serum calcifediol concentrations at around 40 ng/mL and supplementing individuals with deficient serum levels. However, recent studies have questioned the role of vitamin D in clinical practice for COVID-19 in the era of mass vaccinations.

Nailfold capillaroscopy in the rheumatological current clinical practice in Italy: results of a national survey

F. Ingegnoli, M. Cornalba, R. De Angelis, S. Guiducci, D. Giuggioli, C. Pizzorni, V Riccieri, M. Sebastiani, A. Sulli, M. Cutolo

Summary: This study examines the management of nailfold capillaroscopy in Italian rheumatology centers. The findings indicate that capillaroscopy is an integral part of diagnosing Raynaud's phenomenon and monitoring autoimmune connective tissue diseases, although there are variations in reporting methods and patient follow-up timing.

Involvement of the secosteroid vitamin D in autoimmune rheumatic diseases and COVID-19

Maurizio Cutolo, Vanessa Smith, Sabrina Paolino, Emanuele Gotelli

Summary: The active form of vitamin D, 1,25-dihydroxyvitamin D-3, can modulate the innate immune system and downregulate inflammatory responses. Low concentrations of its inactive precursor, 25-hydroxyvitamin D-3, are associated with increased risk and severity of autoimmune rheumatic diseases. In the context of COVID-19, vitamin D-3 can downregulate both the viral phase and the hyperinflammatory phase. Monitoring serum vitamin D-3 concentrations and appropriate supplementation are crucial in autoimmune rheumatic diseases and COVID-19.

Furst

Summary: This study aims to define appropriate outcomes for juvenile systemic sclerosis (SSc) in order to develop successful treatment strategies. Through consensus meetings and data analysis, multiple domains including global disease activity, skin condition, Raynaud's phenomenon, digital ulcers, musculoskeletal, cardiac, pulmonary, renal, gastrointestinal involvement, and quality of life were agreed upon as evaluation measures. Rating: 8/10.

Description of Peripheral Blood Perfusion by Laser Speckle Contrast Analysis (LASCA) in 'Early' versus 'Clinically Overt' Systemic Sclerosis in Routine Clinics

Seppe Willems, Vanessa Smith, Steven Wallaert, Emanuele Gotelli, Tessa Du Four, Kaat Wyckstandt, Andrea Cere, Maurizio Cutolo

Summary: The objective of this study was to compare the peripheral blood perfusion (PBP) measurements between 'early' SSc and 'clinically overt' SSc in a routine setting. The study found no significant difference in adjusted PBP between the two groups when confounding factors were taken into account. However, there was a wide variation of individual datapoints in each subset, highlighting the heterogeneity of SSc.

A Systematic Review of Aminaphtone from Pathophysiology to Clinical Applications: Focus on New Rheumatological Acquisitions

Emanuele Gotelli, Stefano Soldano, Elvis Hysa, Greta Pacini, Carmen Pizzorni, Sabrina Paolino, Maurizio Cutolo, Alberto Sulli

Summary: Aminaphtone is a chemical drug with a successful clinical track record of over 30 years in treating vascular disorders. Recent studies have demonstrated its efficacy in regulating various factors involved in altered microvascular reactivity. This review focuses on its potential application in rheumatological conditions like Raynaud's phenomenon and systemic sclerosis where microvascular dysfunction is significant. However, further research is needed, particularly randomized, double-blind, placebo-controlled clinical trials.

Does Pizza Consumption Favor an Improved Disease Activity in Rheumatoid Arthritis?

Summary: Through a recent cross-sectional study in Italy, we found that consuming half a pizza more than once a week had beneficial effects on disease activity in rheumatoid arthritis (RA) patients, with significant reductions in disease severity. Mozzarella cheese and olive oil, among the pizza-related food items, showed particularly beneficial effects in patients with more severe RA. Future cohort studies are needed to confirm these findings.

Specific Autoantibodies and Microvascular Damage Progression Assessed by Nailfold Videocapillaroscopy in Systemic Sclerosis: Are There Peculiar Associations? An Update

Elvis Hysa, Rosanna Campitiello, Silvia Sammori, Emanuele Gotelli, Andrea Cere, Giampaola Pesce, Carmen Pizzorni, Sabrina Paolino, Alberto Sulli, Vanessa Smith, Maurizio Cutolo

Summary: This narrative review explores the link between systemic sclerosis (SSc)-related autoantibodies and nailfold videocapillaroscopy (NVC) findings. The results show that there is a significant association between specific autoantibodies and NVC patterns in clinical studies, and these autoantibodies have different mechanisms of damage towards endothelial cells, fibroblasts, and smooth muscle vascular cells. These findings further support the interconnectedness between SSc-related autoantibodies and microvascular damage.
