Era of universal testing of microsatellite instability in colorectal cancer

Colorectal cancer (CRC) incidence and mortality are constantly decreasing, but CRC still remains the third most prevalent cancer and the third most common cause of cancer death in both males and females in the United States. Recent rapid declines in CRC incidence rates have largely been attributed to increases in screening that can detect and remove precancerous polyps, and the decrease in death rates for CRC largely reflects improvements in early detection, treatment and the understanding of molecular/genetic basis of CRC. One of the important molecular/genetic findings is the presence of microsatellite instability (MSI) in CRCs. Many studies have shown the importance of MSI testing in diagnosing Lynch syndrome and predicting prognosis and response to chemotherapeutic agents in CRCs. Increased emphasis has been placed on the importance of MSI testing for all newly diagnosed individuals with CRCs. Both immunohistochemical staining (IHC) and polymerase chain reaction (PCR)-based MSI testing show high sensitivity and specificity in detecting MSI. The current clinical guidelines and histopathology features are indicative of, but not reliable in diagnosing Lynch syndrome and CRCs with MSI. Currently, there are evidences that universal testing for MSI starting with either IHC or PCR-based MSI testing is cost effective, sensitive, specific and is getting widely accepted. PSPH promotes melanoma growth and metastasis by metabolic deregulation-mediated transcriptional activation of NR4A1

Summary: Metabolic deregulation in cancer cells promotes tumor growth and metastasis. In this study, PSPH, a key enzyme in serine metabolism pathway, was found to be upregulated in melanoma samples. Knocking down PSPH resulted in inhibition of melanoma growth and metastasis through suppression of 2-HG levels and subsequent activation of pro-oncogenic gene expression pathways.

Can primary hepatocellular carcinoma histomorphology predict extrahepatic metastasis?

Summary: This study compared the histomorphologic features and phenotypic heterogeneity between primary and metastatic hepatocellular carcinoma (HCC) using 39 cases with both primary and metastatic tissues. The findings revealed significant intratumoral heterogeneity and histomorphologic discordance between primary and metastatic HCCs, with certain histologic patterns and subtypes being closely associated with metastasis. Further studies on a larger cohort are needed to better understand the mechanisms of HCC metastasis.

In select patients, hepatic ablation may offer a less invasive but equally effective treatment option compared to hepatectomy.

Hepatocellular Carcinoma Does the Background Liver With or Without Cirrhosis Matter?

Summary: The study revealed that HCCs in noncirrhotic livers have larger tumor size, more macrotrabecular histologic pattern, higher grade and stage, higher rate of vascular invasion, and more anaplastic tumor cells compared to those in cirrhotic livers. Further research is needed to explore the oncogenesis pathways in noncirrhotic livers for better understanding of HCC pathogenesis.

Pathological characteristics of liver sinusoidal thrombosis in COVID-19 patients: A series of 43 cases

Clinicopathological significance of neutrophil-rich colorectal carcinoma

Summary: This study investigates the prognostic significance of tumor-infiltrating neutrophil (TIN) in colorectal carcinomas (CRCs). The results show that high TIN is associated with higher histological grade, right-sided tumor location, advanced TNM stage, and higher rate of MMR-deficient CRCs. Patients with neutrophil-rich (NR) CRCs have significantly poorer 5-year recurrence-free survival compared to neutrophil-intermediate (NI) or neutrophil-poor (NP) CRCs. High TIN remains an independent risk factor for poor prognosis. NR CRCs are more commonly seen in MMR-deficient CRCs, and MMR-deficient NR CRCs have similar 5-year recurrence-free survival compared to MMR-proficient NR CRCs.
