Carcinomas, such as colon cancer, initiate their invasion by rescuing the innate plasticity of both epithelial cells and stromal cells. Although Snail is a transcriptional factor involved in the Epithelial-Mesenchymal Transition, in recent years, many studies have also identified the major role of Snail in the activation of Cancer-Associated Fibroblast (CAF) cells and the remodeling of the extracellular matrix. In CAFs, Platelet-derived growth factor (PDGF) receptor signaling is a major functional determinant. High expression of both SNAI1 and PDGF receptors is associated with poor prognosis in cancer patients, but the mechanism(s) that underlie these connections are not understood. In this study, we demonstrate that PDGF-activated fibroblasts stimulate extracellular matrix (ECM) fiber remodeling and deposition. Furthermore, we describe how SNAI1, through the FAK pathway, is a necessary factor for ECM fiber organization. The parallel-oriented fibers are used by endothelial cells as tracks, facilitating their activation and the creation of tubular structures mimicking in vivo capillary formation. Accordingly, Snail1 expression in fibroblasts was required for the coadjuvant effect of these cells on matrix remodeling and neoangiogenesis when co-xenografted in nude mice. Finally, in tumor samples from colorectal cancer patients a direct association between stromal SNAI1 expression and the endothelial marker CD34 was observed. In summary, our results advance the understanding of PDGF/SNAI1-activated CAFs in matrix remodeling and angiogenesis stimulation.

Detection of IDH1 Mutations in Plasma Using BEAMing Technology in Patients with Gliomas

Summary: Compared with other solid tumors, the detection of different biomarkers in the peripheral blood (PB) of patients with gliomas is still at a small scale research stage. A prospective study found that circulating tumor DNA (ctDNA) was detected in the PB of three out of six patients with isocitrate dehydrogenase 1 (IDH1)-mutant tumors, with a false-negative rate reaching 86%. Using Beads, Emulsion, Amplification and Magnetics (BEAMing) digital PCR technology, the IDH1 mutations in the plasma samples of IDH1-mutant patients can be accurately detected, with complete agreement with the mutant loci in the tumor. Larger prospective studies are needed to further validate these findings and explore the clearance of mutations in the blood of IDH1-positive patients during therapy.

Summary: Germline replication-repair deficient (gRRD) gliomas are rare and few patients have been treated with immune checkpoint inhibitors (ICIs). Unlike sporadic gliomas, gRRD glioma patients seem to benefit from PD-1 inhibitors, such as nivolumab or pembrolizumab, in terms of durable responses or survival. This study analyzed T-cell quantity and clonarity before and after nivolumab treatment in a Lynch syndrome-associated glioma patient with a germline hMLH1 mutation to understand the role of acquired immunity in gRRD gliomas. The lack of objective response to anti-PD-1 therapy may be explained by the scant T-cell infiltrate, and the change in TCR clonality reflects the molecular heterogeneity in a highly pre-treated disease. A review of the literature on the role of ICIs in sporadic and gRRD gliomas was also conducted.

Summary: The awareness of chronic kidney disease (CKD) is low among the public, physicians, and health authorities, with many still confusing it with chronic kidney insufficiency or failure. There is a prevailing view that CKD is mainly associated with kidney replacement therapy, but in reality, its main burden is accelerated aging and premature death from all causes, especially cardiovascular diseases. Researchers are calling for increased investment in CKD research to prevent it from becoming the second leading cause of death globally by 2040.
