When Gross Total Resection of a Glioblastoma Is Possible, How Much Resection Should Be Achieved?

OBJECTIVE: The efficacy of extensive resection on prolonging survival for patients with glioblastoma (GBM) is controversial because prior studies have included tumors with dissimilar resection capabilities. The true isolated effect of increasing resection on survival for GBM therefore remains unclear. METHODS: Adult patients who underwent surgery of an intracranial newly diagnosed GBM at an academic tertiary-care institution from 2007 to 2011 were reviewed. Preoperative images were reviewed by 3 neurosurgeons independently. Tumors considered amenable to gross total resection based on preoperative imaging by all neurosurgeons were included. Multivariate proportional hazards regression analysis was used to identify if an association existed between residual volume (RV) and extent of resection (EOR) with survival. RESULTS: Of the 292 patients with newly diagnosed GBM, 84 (29%) were amenable to gross total resection. The median (interquartile range) pre and postoperative tumor volumes were 27 (13.8-54.4) and 0.9 (0-2.7) cm(3), respectively. The mean percent resection was 91.7% +/- 1.3%. In multivariate analysis, after controlling for age, functional status, and adjuvant therapies, RV (hazards ratio [HR] [95% confidence interval (CI)] = 1.114 [1.033-1.193], P = 0.006) and EOR (HR [95% CI] = 0.959 [0.934-0.985], P = 0.003) were each independently associated with survival. The RV and EOR with the greatest reduction in the risk of death was <2 cm(3) and >95%, respectively. Likewise, RV (HR [95% CI] = 1.085 [1.010-1.178], P = 0.01) and EOR (HR [95% CI] = 0.962 [0.930-0.998], P = 0.04) each remained independently associated with recurrence. CONCLUSION: This is the first study to evaluate RV and EOR in a more uniform population of patients with tumors of similar surgical capabilities. This study shows that achieving a decreased RV and/or an increased EOR is independently associated with survival and recurrence in those patients with tumors with similar resection capacities.

Further long-term studies are needed to determine the utility of remote monitoring for assessing gait improvement following VPS placement.

CSF dynamics in long-standing overt ventriculomegaly in adults

Summary: This study found a differential cerebrospinal fluid (CSF) dynamics pattern between patients with Long-standing Overt Ventriculomegaly in Adults (LOVA) and those with idiopathic normal pressure hydrocephalus (iNPH). A higher proportion of LOVA patients showed isolated compliance impairment. Therefore, CSF dynamics analysis plays a significant role in evaluating LOVA patients, and future research may help define diagnosis and treatment algorithms for LOVA based on CSF dynamics analysis.

Myo1e overexpression in lung adenocarcinoma is associated with increased risk of mortality

Summary: The study aimed to assess the impact of MYO1E overexpression on non-small cell lung carcinoma (NSCLC) and investigate the association between DNA methylation, RNA expression of MYO1E, and survival/mortality risk in NSCLC patients. The results show a negative correlation between DNA methylation and RNA expression of MYO1E in both LUAD and LUSC subpopulations. Late-stage LUAD patients had lower DNA methylation and higher RNA expression of MYO1E, while no significant differences were observed in LUSC patients. Low DNA methylation and high RNA expression of MYO1E were associated with shorter median survival time and increased mortality risk in LUAD, but not in LUSC. This study suggests that changes in MYO1E methylation and expression play a crucial role in the pathogenesis of lung cancer, particularly in LUAD, and highlights the potential utility of MYO1E DNA methylation and RNA expression as predictors of survival for LUAD patients. Furthermore, MYO1E DNA methylation could be used as a circulating tumor marker in liquid biopsies due to its low normal expression in blood cells.

INPH and parkinsonism: A positive shunt response with a negative tap test

Summary: The aim of this study was to compare the clinical and functional performances of INPH patients with and without parkinsonism at different stages of evaluation and surgery. The results showed that patients with parkinsonism performed worse than those without parkinsonism. Unsatisfactory response to the CSF tap test should not exclude INPH patients with parkinsonism from VPS surgery.

Summary: This study used a rat model of traumatic SCI to identify transcriptome changes and potential therapeutic value of Tamoxifen. Transcriptome analysis revealed differential gene expression related to SCI and inflammatory response. Tamoxifen was found to regulate the expression changes of Ccr2 and Mmp12 genes. These findings contribute to our understanding of Tamoxifen as a potential therapeutic resource for traumatic SCI during the acute phase.

Modic Changes Increase the Cage Subsidence Rate in Spinal Interbody Fusion Surgery: A Systematic Review and Network Meta-Analysis

Summary: This study aimed to compare the effect of different Modic changes (MC) grades on the cage subsidence rate after spinal interbody fusion surgery. A meta-analysis was conducted on relevant randomized controlled trials and cohort studies. The results showed that MC may be associated with a higher cage subsidence rate. With the increase in MC grades, the incidence of subsidence decreased gradually, but it was always higher than that in the non-Modic changes (NMC). Oblique lumbar interbody fusion may be a better treatment choice for lumbar degenerative disease with MC.

A Medical Student's Guide to Online Resources in Neurosurgery
