Case report: biallelic DNMT3A mutations in acute myeloid leukemia

Summary: Mutations in the DNMT3A gene are common in de novo acute myeloid leukemia (AML) patients, with heterozygous variants being typical. However, biallelic variants are rare and associated with a worse prognosis. In this study, a unique AML case was presented with a complex DNMT3A molecular profile consisting of four different somatic variants mapping on different alleles. These mutations were predicted to significantly decrease or abolish DNMT3A activity, leading to global genomic hypermethylation. The unusual behavior of DNMT3A in this case highlights the need for further understanding of its role in AML development and persistence.

The impact of DNMT3A variant allele frequency and two different comutations on patients with de novo cytogenetically normal acute myeloid leukemia

Summary: In this study, we investigated the biological and prognostic significance of DNMT3A mutations in acute myeloid leukemia (AML) by analyzing 171 adult patients with de novo cytogenetically normal AML. DNMT3A(High) patients with a variant allele frequency (VAF) above 42% showed higher white blood cell count, higher lactate dehydrogenase level, lower complete remission rate, and shorter overall survival compared to DNMT3A(Low) patients. We also identified two different comutated genotypes, DNMT3A(mut)NPM1(mut)FLT3-ITDmut and DNMT3A(mut)IDH1/IDH2(mut), which showed different clinical outcomes. Multivariate analyses revealed that DNMT3A(High) and DNMT3A(mut)NPM1(mut)FLT3-ITDmut were independently associated with lower complete remission rate and shorter overall survival and relapse-free survival, respectively.

Summary: The study proposed an alternative method to estimate MRD based on AML-associated DNA methylation patterns, which could reliably distinguish malignant and healthy samples by identifying abnormal DNAm patterns. However, since healthy samples may also contain abnormal-classified DNAm reads, the approach does not yet reliably differentiate between MRD positive and negative samples.

Identification of two novel mutations in human acute myeloid leukemia cases

Summary: AML is an aggressive cancer with rapid progression, where cytogenetic and genetic analysis play crucial roles in diagnosis and prognosis. This study identified two novel deleterious mutations in DNMT3A and RUNX1 genes, providing additional information for patient diagnosis and prognosis. The exclusion and co-occurrence of specific gene mutation patterns in AML were also confirmed, offering insights into the mechanisms of AML leukaemogenesis.

Microenvironmental CXCL12 deletion enhances Flt3-ITD acute myeloid leukemia stem cell response to therapy by reducing p38 MAPK signaling

Summary: Using a mouse model of FLT3-ITD AML, researchers found that FLT3-ITD AML LSC were enriched within the ST-HSC population and had increased expression of CXCR4. Deletion of CXCL12 from the microenvironment enhanced targeting of AML cells by Flt3-TKI plus chemotherapy treatment, including enhanced LSC targeting. Inhibition of p38 signaling reduced the maintenance of Flt3-ITD AML LSC and enhanced their sensitivity to treatment.

Molecular and clinical significance of FLT3, NPM1, DNMT3A and TP53 mutations in acute myeloid leukemia patients

Summary: The aim of this study was to determine the relationship between genetic alterations and clinical-hematological parameters, prognostic factors and therapy in Iraqi patients with AML. Sanger Sequencing was used to detect mutations in 76 AML patients, and their prognosis was compared. Somatic mutations were found in 47.4% of patients, with FLT3, DNMT3A, NPM1, and TP53 being the most common mutated genes. The presence of mutations was significantly associated with disease progression and total leukocyte count after induction therapy.

Summary: This study found that AML cells with DNMT3A R882H mutation exhibited high proliferative and anti-apoptotic activities, as well as resistance to daunorubicin treatment. The NRF2 pathway played an important role in the regulation of cell apoptosis, and the expression of NRF2 was regulated by DNMT3A R882H mutation through protein stability rather than methylation. Targeting NRF2 might be a potential therapeutic strategy for AML patients with DNMT3A R882H mutation.

Leukemic stem cells and therapy resistance in acute myeloid leukemia

Summary: A major challenge in treating acute myeloid leukemia (AML) is the presence of therapy-resistant cells, known as leukemic stem cells (LSC), which can contribute to relapse after achieving remission. LSC display significant plasticity and can develop mechanisms to resist treatment, making them difficult to target with chemotherapy. The combination of venetoclax and azacitidine shows promise in targeting AML LSC, but it is still unclear which patients would benefit most from this treatment and how to overcome resistance mechanisms. Clinical trials are underway to investigate LSC susceptibility to first-line therapies and better understand the complex nature of AML. This review discusses the biology of LSC, resistance mechanisms, potential vulnerabilities, and ongoing clinical trial activities related to LSC-targeted therapies.

Summary: In Guatemala, children aged 0-6 with cHL have a high EBV expression rate of 97.5%, share similar immune regulatory mechanisms with adult cHL, and show increased CD8+ lymphocytes and tumor-associated macrophages in the tumor microenvironment. Despite high-risk disease in 25 patients, only four died from disease progression, relapse, or infection.
