Summary: Cytomegalovirus (CMV) is a common infection in immunocompromised patients, especially those undergoing allo-HSCT. Recent management strategies include pre-emptive treatment with frequent CMV PCR monitoring and the use of new drugs like letermovir. Treatment of CMV disease is challenging due to drug resistance, but promising results have been seen with maribavir and other alternative treatments such as immunotherapy, artesunate, and leflunomide.

Summary: This study found an increased risk of post-LTx mortality in recipients with previous allo-HSCT, especially related to infections. The 1-year survival rate post-LTx was similar to re-LTx recipients, but the conditional 1-year survival rate was lower. Therefore, optimization of post-LTx immunosuppressive and antimicrobial strategies should be considered in this patient population.

Summary: The high morbidity of HBV reactivation following allo-HSCT is partially due to complex therapeutic regimens and immunosuppression, which leads to HBV replication. Prophylactic antiviral therapy should be given to HBsAg positive recipients to reduce the risk of HBV reactivation.

Summary: This study conducted a retrospective cohort analysis on patients with severe aplastic anemia (SAA) who underwent allogeneic hematopoietic stem cell transplantation (allo-HSCT) to compare their survival between those with infection and those without infection. The study found that patients with infection before transplantation had inferior survival compared to those without infection, highlighting the importance of prophylaxis and complete control of infectious complications before transplantation among SAA patients.

Summary: This study investigated the clinical characteristics and mortality risk factors of mixed bacterial infections (MBI) in hematopoietic stem cell transplantation (HSCT) recipients. The results showed that MBI has become a serious problem after HSCT, with a high mortality rate. Time interval from diagnosis to transplantation, ICU admission after transplantation, engraftment period, continuous renal replacement therapy, and septic shock were identified as independent risk factors for MBI acquisition and mortality. Clinicians should pay attention to MBI and formulate monitoring and treatment plans to improve patient prognosis.

Summary: The study highlights the important role of MSC-derived extracellular vesicles (MSC-EVs) in hematopoietic regulation and immune response after transplantation. MSC-EVs directly fuse with hematopoietic stem cell membranes, affecting their fate, and regulate the proliferation and differentiation of hematopoietic stem cells through the release of various bioactive factors.

Summary: The comparison of oral and pathogenic microorganisms in bloodstream infections (BSIs) in allogeneic hematopoietic stem cell transplantation (allo-HSCT) showed that oral microorganisms caused 46.2% of BSIs in patients. The incidence rate of BSIs was 27%, with coagulase-negative Staphylococci being the predominant microorganism.

Summary: The non-use of fluoroquinolone prophylaxis during neutropenia can reduce antibiotic resistance in Gram-negative bacteria causing bloodstream infections (BSIs) in hematopoietic stem cell transplantation (HSCT) patients. However, it may also increase BSI incidence and mortality rates.

Summary: Autophagy is a fundamental process that helps cells adapt to metabolic stress by degrading and recycling intracellular components. It plays a crucial role in various aspects of allogeneic hematopoietic stem cell transplantation (HSCT), including the survival and function of hematopoietic stem cells, T-cell differentiation, antigen presentation, and responsiveness to cytokine stimulation. However, the overall role of autophagy in HSCT remains unclear, highlighting the need for further understanding to improve transplant outcomes.

Summary: Increased synthesis and release of inflammatory signalling proteins are common among individuals with hematologic malignancies undergoing hematopoietic cell transplantation (HCT). This study examined the relationship between markers of inflammatory activity and depression symptoms following HCT. The results showed that patients with elevated levels of IL-6 and IL-10 reported more severe depression symptoms post-HCT, especially in terms of neurovegetative symptoms.

Summary: Opportunistic infections remain a major cause of morbidity and mortality after pediatric HCT, with risk factors including patient's disease, treatment history, donor graft source, pre-transplant therapy, and immunosuppressive agents. Infection risk changes over time and is greatly influenced by the degree of immunosuppression, highlighting the need for careful monitoring.

Summary: Infection is a major complication after allo-HSCT, especially CMV infection and IFI. Taiwan has a high CMV seroprevalence area. This study aimed to evaluate the incidence, risk factors, impact on survival of CMV infection, and the association of CMV infection and IFI in recipients after allo-HSCT.

Summary: The self-renewal and differentiation of hematopoietic stem cells are tightly controlled by various cells and cytokines in the bone marrow microenvironment. Changes in the niche composition can lead to hematological malignancies, and processes like homing, proliferation, and differentiation of HSCs are crucial for the success of hematopoietic stem cell transplantation. Single-cell sequencing and in vivo imaging are powerful tools for studying the bone marrow microenvironment in diseases like leukemia and after transplantation.

Summary: The thymus plays a crucial role in T-cell development, but its function can be affected by various factors, including chemotherapy, conditioning regimen, graft-versus-host disease, and steroid therapy. Despite its remarkable capacity to recover after damage, the pathways governing its regeneration are poorly understood. Tools for measuring thymic output can help assess thymic function.

Summary: The study shows that acetylation-induced activation of PLZF affects the development of NKT cells, leading to a decrease in cell proportion and numbers, and blockage of transition from developmental stage 1 to stage 2. Acetylation of PLZF also alters NKT cell subset differentiation, inhibiting the development of NKT1 and NKT17 subsets while promoting the emergence of an NKT cell subset resembling cells in early developmental stages.
